    Human amniotic fluid glycoproteins expressing sialyl Lewis carbohydrate antigens stimulate progesterone production in human trophoblasts in vitro

    Background: Progesterone is thought to mediate immune modulator effects by regulating uterine responsiveness. The aim of the study was to clarify the effect of transferrin and glycodelin A (former name PP14) as sialyl Lewis X-expressing glycoproteins on the release of progesterone by trophoblast cells in vitro. Methods: Cytotrophoblast cells were prepared from human term placentas by standard dispersion of villous tissue followed by a Percoll gradient centrifugation step. Trophoblasts were incubated with varying concentrations (50-300 mug/ml) of human amniotic fluid- and serum-transferrin as well as with glycodelin A. Culture supernatants were assayed for progesterone, human chorionic gonadotropin (hCG) and cortisol by enzyme immunometric methods. Results: The release of progesterone is increased in amniotic fluid transferrin- and glycodelin A-treated trophoblast cell cultures compared to untreated trophoblast cells. There is no relation between transferrin and the hCG or cortisol production of trophoblast cells. Conclusion: The results suggest that sialyl Lewis carbohydrate antigen-expressing amniotic fluid glycoproteins modulate the endocrine function of trophoblasts in culture by upregulating progesterone production.     Factors contributing to reported medication administration incidents in patients' homes - A text mining analysis

    AIMS: To describe the characteristics of medication administration (MA) incidents reported to have occurred in patients' own homes (reporters' profession, incident types, contributing factors, patient consequence, and most common medications involved) and to identify the connection terms related to the most common contributing factors based on free text descriptions. DESIGN: A retrospective study using descriptive statistical analysis and text mining. METHODS: Medication administration incidents (N = 19,725) reported to have occurred in patients' homes between 2013-2018 in one district in Finland were analysed, describing the data by the reporters' occupation, incident type, contributing factors, and patient consequence. SASŸ Text Miner was used to analyse free text descriptions of the MA incidents to understand contributing factors, using concept linking. RESULTS: Most MA incidents were reported by practical (lower level) nurses (77.8%, N = 15,349). The most common category of harm was 'mild harm' (40.1%, N = 7,915) and the most common error type was omissions of drug doses (47.4%, N = 9,343). The medications most commonly described were Marevan [warfarin] (N = 2,668), insulin (N = 811), Furesis [furosemide] (N = 590), antibiotic (N = 446), and Panadol [paracetamol] (N = 416). The contributing factors most commonly reported were 'communication and flow of information' (25.5%, N = 5,038), 'patient and relatives' (22.6%, N = 4,451), 'practices' (9.9%, N = 1,959), 'education and training' (4.8%, N = 949), and 'work environment and resources' (3.0%, N = 598). CONCLUSION: There is need for effective communication and clear responsibilities between home care patients and their relatives and health providers, about MA and its challenges in home environments. Knowledge and skills relating to safe MA are also essential. IMPACT: These findings about MA incidents that have occurred in patients' homes and have been reported by home care professionals demonstrate the need for medication safety improvement in home care

    Factors Related to Medication Administration Incidents in England and Wales Between 2007 and 2016: A Retrospective Trend Analysis

    OBJECTIVES The aims of the study were to describe medication administration incidents reported in England and Wales between 2007 and 2016, to identify which factors (reporting year, type of incident, patients' age) are most strongly related to reported severity of medication administration incidents, and to assess the extent to which relevant information was underreported or indeterminate. METHODS Medication administration incidents reported to the National Reporting & Learning System between January 1, 2007, and December 31, 2016 were obtained. Characteristics of the data were described using frequencies, and relationships between variables were explored using cross-tabulation. RESULTS A total of 517,384 incident reports were analyzed. Of these, 97.1% (n = 502,379) occurred in acute/general hospitals, mostly on wards (69.1%, n = 357,463), with medicine the most common specialty area (44.5%, n = 230,205). Medication errors were most commonly omitted doses (25.8%, n = 133,397). The majority did not cause patient harm (83.5%, n = 432,097). When only incidents causing severe harm or death (n = 1,116) were analyzed, the most common type of error was omitted doses (24.1%). Most incidents causing severe harm or death occurred in patients aged 56 years or older. For the 10-year period, the percentage of incidents with “no harm” increased (74.1% in 2007 to 86.3% in 2016). For some variables, data were often missing or indeterminate, which has implications for data analysis. CONCLUSIONS Medication administration incidents that do not cause harm are increasingly reported, whereas incidents reported as severe harm and death have declined. Data quality needs to be improved. Underreporting and indeterminate data, inaccuracies in reporting, and coding jeopardize the overall usefulness of these data

    Tannins, flavonoids and stilbenes in extracts of African savanna woodland trees Terminalia brownii, Terminalia laxiflora and Anogeissus leiocarpus showing promising antibacterial potential

    Terminalia laxiflora, Terminalia brownii and Anogeissus leiocarpus are used as decoctions, macerations, infusions and fumigations in East and West African traditional medicine for treatment of infectious diseases and their symptoms. Using this ethnopharmacological information as a guideline for our research and owing to the fact that these species have not been subjected to in depth antibacterial and phytochemical studies, thirty-nine extracts of various polarities of the stem bark, stem wood and roots were studied for growth inhibitory effects against the human pathogenic bacteria Staphylococcus epidermidis, Staphylococcus aureus, Micrococcus luteus and Pseudomonas aeruginosa. Our results indicate that the studied species contain antibacterial compounds of a wide range of polarities. All polar root extracts of T. laxiflora and various polar extracts of T. brownii roots, including hot water decoctions, gave broad-spectrum antibacterial effects and low MIC values of 39 mu g/ml. The main ellagitannins in an ethyl acetate extract of the root of T. laxiflora were found to be corilagin and its derivative and punicalagin. A methanol extract of the roots of T. brownii contained methyl-(S)-flavogallonate and its derivative as the main identified ellagitannins. Moreover, both Terminalia species were found to contain ellagic acid xylopyranoside and methyl ellagic acid xyloside and pure ellagic acid was present in T. brownii. Pure punicalagin did not give as low MIC as an ethyl acetate extract of the roots of T. laxiflora, containing punicalagin as one of its main compounds, although this ellagitannin totally inhibited the growth of S. aureus at 125 mu g/ml and P. aeruginosa at 500 mu g/ml. Similarly, pure ellagic and gallic acid gave higher MIC values than the methanolic root extract of T. brownii against S. aureus and P. aeruginosa. Moreover, a Sephadex LH-20 fraction of the methanolic extract of the roots of T. brownii, enrichedwithmethyl-(S)-flavogallonate and its isomer, gave higher MIC values than the crude methanolic extract. These results suggest that the polyphenols in the extracts might act synergistically with each other. A methanolic soxhlet extract of the roots of A. leiocarpus, containing ampelopsin, aromadendrin, taxifolin, pinosylvin and 4'-methylpinosylvin gave a low MIC value of 39 mu g/ml against all bacterial strains used in this investigation. Our results demonstrate that the roots, stem bark and stem wood of T. brownii, T. laxiflora and A. leiocarpus are rich sources of (new) antimicrobial compounds and justify the uses of these plants for treatment of infections in African traditional medicine.Peer reviewe

    RIG-I signaling Is essential for influenza B virus-induced rapid interferon gene expression

    Influenza B virus causes annual epidemics and, along with influenza A virus, accounts for substantial disease and economic burden throughout the world. Influenza B virus infects only humans and some marine mammals and is not responsible for pandemics, possibly due to a very low frequency of reassortment and a lower evolutionary rate than that of influenza A virus. Influenza B virus has been less studied than influenza A virus, and thus, a comparison of influenza A and B virus infection mechanisms may provide new insight into virus-host interactions. Here we analyzed the early events in influenza B virus infection and interferon (IFN) gene expression in human monocyte-derived macrophages and dendritic cells. We show that influenza B virus induces IFN regulatory factor 3 (IRF3) activation and IFN-λ1 gene expression with faster kinetics than does influenza A virus, without a requirement for viral protein synthesis or replication. Influenza B virus-induced activation of IRF3 required the fusion of viral and endosomal membranes, and nuclear accumulation of IRF3 and viral NP occurred concurrently. In comparison, immediate early IRF3 activation was not observed in influenza A virus-infected macrophages. Experiments with RIG-I-, MDA5-, and RIG-I/MDA5-deficient mouse fibroblasts showed that RIG-I is the critical pattern recognition receptor needed for the influenza B virus-induced activation of IRF3. Our results show that innate immune mechanisms are activated immediately after influenza B virus entry through the endocytic pathway, whereas influenza A virus avoids early IRF3 activation and IFN gene induction. IMPORTANCE Recently, a great deal of interest has been paid to identifying the ligands for RIG-I under conditions of natural infection, as many previous studies have been based on transfection of cells with different types of viral or synthetic RNA structures. We shed light on this question by analyzing the earliest step in innate immune recognition of influenza B virus by human macrophages. We show that influenza B virus induces IRF3 activation, leading to IFN gene expression after viral RNPs (vRNPs) are released into the cytosol and are recognized by RIG-I receptor, meaning that the incoming influenza B virus is already able to activate IFN gene expression. In contrast, influenza A (H3N2) virus failed to activate IRF3 at very early times of infection, suggesting that there are differences in innate immune recognition between influenza A and B viruses

    Potential Anti-Tuberculosis Activity of the Extracts and Their Active Components of Anogeissus leiocarpa (DC.) Guill. and Perr. with Special Emphasis on Polyphenols

    In Sudanese traditional medicine, decoctions of the stem bark of Anogeissus leiocarpa are used for the treatment of tuberculosis (TB). However, this plant has not been investigated before for its antimycobacterial effects. Our screening results show, for the first time, that many extracts of various parts of A. leiocarpa exhibit growth inhibitory activity against Mycobacterium smegmatis. Minimum inhibitory concentration (MIC) values ranged between 625 and 5000 ”g/mL, with an ethyl acetate extract of the root showing the lowest MIC value. The good antimycobacterial effects of the root part could be due to its high concentration of ellagic acid derivatives, ellagitannins, and flavonoids. Thin layer chromatography (TLC) fractionation resulted in some fractions with better activity than the starting point crude methanol extract (MIC 2500 ”g/mL). Those fractions with the lowest MIC values contained a high number of antioxidant compounds. Fractions 3 and 4 (MIC 1500 and 1000 ”g/mL, respectively) contained high concentrations of di-methyl ellagic acid ([M-H]− 329.0318). Fraction 6 (MIC 2000 ”g/mL) contained a lower concentration of di-methyl ellagic acid and was not as growth inhibitory as fractions 3 and 4. Moreover, in fraction 3, an acetylated ellagic acid derivative ([M-H]− 343.0477) and di-methyl-ellagic acid xyloside ([M-H]− 461.0739) were tentatively characterized. Di-methyl ellagic acid xyloside was also present in fraction 4 and could strongly contribute to the antimycobacterial effect of this fraction. Additionally, protocatechuic acid ([M-H]− at m/z 153.0196) was present in fraction 4. Our antimycobacterial results obtained from this research justify the use of A. leiocarpa in Sudanese folk medicine against cough related to TB. Roots, stem bark, and leaves of A. leiocarpa are sources for new potent anti-TB drug lead compounds

    TiivistelmÀ. Tutkimuksessa tarkastellaan sattumanvaraisesti valittujen 6.-luokkalaisten lasten koulukaverisuhteita. Kaverisuhteet ovat kaikille lapsille tÀrkeitÀ tuottaen ryhmÀÀn kuulumisen tunnetta. Vertaissuhteet ovat koulumaailmassa merkityksellisiÀ ja vaikuttavat lapsen kokonaisvaltaiseen kasvuun ja kehitykseen. Koulu on lapsuuden keskeinen ympÀristö ja kaverisuhteiden luominen on yksi tÀrkeimmistÀ saavutuksista koulussa. Tutkimuksen tavoite oli selvittÀÀ 6.-luokkalaisten lasten luokan sisÀisiÀ kaverivalintoja sekÀ oppilaiden perusteluita valinnoille. Aineistonkeruu tehtiin kahteen erikokoiseen kouluun. Tutkimusluokat edustivat isoa ja pientÀ koulua. Kyseiseen ratkaisuun pÀÀdyttiin kahden koulun tulosten vertailun saamiseksi. MenetelmÀksi valittiin sosiometriikka ja aineisto kerÀttiin kyselylomakkeella. Sosiometriikkalla tarkoitetaan sosiaalisten suhteiden mittaamista. SosiometrisillÀ menetelmillÀ tutkitaan ryhmÀn sisÀllÀ tapahtuvia sosiaalisia valintoja, kuten kaverisuhteita. Tutkimuksen aineistona toimi yhteensÀ 32 oppilaan kyselylomakkeiden vastaukset. Tutkimuksessa keskeisiÀ teemoja olivat sukupuolen sekÀ ulkoisten ja sisÀisten tekijöiden merkitys koulukaveruudelle. Oppilaiden sosiaaliset roolit luokassa nÀkyivÀt ulkopuolisuuden ja sisÀpuolisuuden erilaisiana esiintyvyyksinÀ. Tarkastelun kohteena olivat myös erot ja yhtÀlÀisyydet koulujen ryhmÀdynamiikkassa sekÀ oppilaiden vastauksissa. Oppilaita pyydettiin muun muassa valitsemaan 18 vaihtoehdosta hÀnelle tÀrkeimmÀt tekijÀt kaverissa. Vaihtoehdot kyselylomakkeeseen muodostettiin Salmivallin (1998) ominaispiirteitÀ kÀsittelevÀn teorian pohjalta. LisÀksi teoreettisenÀ lÀhtökohtana tutkimuksessa toimivat Sheelyn ja Gutsteinin (2002) tutkimus osaamisalueista sekÀ Ojasen (2006) sosiaalista kÀyttÀytymistÀ kÀsittelevÀ teoria. Verrattaessa ison ja pienen koulun oppilaiden valintoja, painottui pienessÀ koulussa enemmÀn kiltteyden merkitys ja isossa koulussa se, ettÀ kaverin kanssa ollaan usein samaa mieltÀ. Tutkimuksen molempien koulujen 6.-luokkalaisille oppilaille tÀrkeimmÀksi tekijÀksi koulukaverissa ilmeni hauskuus. LisÀksi pinnalle nousivat riitelemÀttömyys, avuliaisuus sekÀ ystÀvÀllisyys, jotka tutkimustuloksissa yhdistettiin yhteistyötaidoiksi. Tutkimustulokset ovat sidoksissa kyseisen kulttuurin, yhteiskunnan, luokan sekÀ sen oppilaiden yksilöllisiin kokemuksiin. Tutkimus on ajankohtainen, sillÀ mukaan tutkimukseen otettiin kulutusyhteiskunnan, jonka kulutustuotteet lisÀÀvÀt eriarvoisuutta kouluissa, mahdollinen vaikuttavuus koulukaveruuteen

    The role of individual and social variables in task performance.

    This paper reports on a data-based study in which we explored - as part of a larger-scale British-Hungarian research project - the effects of a number of affective and social variables on foreign language (L2) learners’ engagement in oral argumentative tasks. The assumption underlying the investigation was that students’ verbal behaviour in oral task situations is partly determined by a number of non-linguistic and non-cognitive factors whose examination may constitute a potentially fruitful extension of existing task-based research paradigms. The independent variables in the study included various aspects of L2 motivation and several factors characterizing the learner groups the participating students were members of (such as group cohesiveness and intermember relations), as well as the learners’ L2 proficiency and ‘willingness to communicate’ in their L1. The dependent variables involved objective measures of the students’ language output in two oral argumentative tasks (one in the learners’ L1, the other in their L2): the quantity of speech and the number of turns produced by the speakers. The results provide insights into the interrelationship of the multiple variables determining the learners’ task engagement, and suggest a multi-level construct whereby some independent variables only come into force when certain conditions have been met
