Adapting a Medication Reconciliation Process to the Family Medicine Outpatient Setting
The Institute of Healthcare Improvement states that “medication reconciliation is the process of creating the most accurate list possible of all medications a patient is taking” so that physicians may know what medications the patient is taking, at what dose, and to identify any discrepancies between prescriptions during the patient encounter. However, the medication reconciliation rates of the DHR Family Medicine Center are not optimal as many med reconciliations remain uncompleted. An adapted protocol from a previous quality improvement project done at DHR Women’s Clinic in 2020 was implemented to seek to improve medication reconciliation rates.
This Quality Improvement Project was conducted as a retrospective analysis of the medication reconciliation compliance rates by family medicine residents at the UTRGV-DHR Family Medicine Center from January to March 2022. Medication reconciliation rates were determined number of mediation reconciliations completed for the total number of patients seen by the resident. Residents underwent an education intervention on the medication reconciliation process and attendings/preceptors were to remind residents during sign out starting at the end of January to compare pre and post intervention rates of completion. UTRGV-SOM Biostatistics department was consulted for analysis of the data.
We included 21 residents from them, in first year we had 8 residents, in second year 8 residents and third year were 5, with mean (SD) ages: 30 (3.6), 30.4 (2.2) and 28.2 (1.3) years old, respectively. We evaluated 1,724 registered consults to assess the rate of reconciliation in January (1: Pre-intervention), and February (2: post intervention), and March (3: endurance). Residents of 1st years evaluated 13% (n=217) of total consults, residents of 2nd year 54% (n=935) and 3rd year evaluated 33% (n=572). Panel analysis showed increase in the probability of reconciliation (p=0.006). The global analysis showed the rate reconciliation success had an initial median 94% which increased to 100 at the third month. The attributable increase due to the intervention was 9%.
The standardized protocol for medication reconciliation education and reminders are effective in increasing the rate in during patient encounters
FIRST DISCOVERY OF MIDDLE TOURNAISIAN CONODONTS IN THE GRIOTTE-TYPE NODULAR PELAGIC LIMESTONES, ISTANBUL AREA, NW TURKEY
The uppermost layers of the Ayineburnu member of the Büyükada Formation in the Istanbul-Gebze area are represented by Griotte-type pelagic nodular limestones. They yielded conodont elements (Bispathodus stabilis and Siphonodella lobata) that can identify the interval from the upper part of the sandbergi Zone through the isosticha- Upper crenulata Zone - middle Tournaisian - Early Carboniferous. This is the first middle Tournaisian conodont record published from an autochthonous succession in the Istanbul Terrane, NW Turkey. This level characterizes the very last carbonate deposition and is followed by lydites and black siliceous shales and finally by the accumulation of flysch-type deposits during the late Tournaisian and early Visean, that mark the onset of Variscan events to the north of the Istanbul Terrane. A correlation with the Lower Carboniferous successions in the easterly-located Zonguldak area suggests that Istanbul and Zonguldak terranes were in quite different palaeogeographic positions during this time interval
Elevated serum levels of macrophage migration inhibitory factor and stem cell growth factor β in patients with idiopathic and systemic sclerosis associated pulmonary arterial hypertension.
Pulmonary arterial hypertension (PAH) can be idiopathic or secondary to autoimmune diseases, and it represents one of the most threatening complications of systemic sclerosis (SSc). Macrophage migration inhibitory factor (MIF) is a pleiotropic cytokine with proinflammatory functions that appears to be involved in the pathogenesis of hypoxia-induced PH. In SSc patients, high serum levels of MIF have been associated with the development of ulcers and PAH. Stem cell growth factor β (SCGF β) is a human growth factor that, together with MIF, is involved in the pathogenesis of chronic spinal cord injury. The aim of our study was to measure serum levels of MIF in patients with idiopathic and SSc-associated PAH. We enrolled 13 patients with idiopathic PAH and 15 with SSc-associated PAH. We also selected 14 SSc patients without PAH and 12 normal healthy controls, matched for sex and age. PAH was confirmed by right hearth catheterism (mPAP>25 mmHg). MIF and SCGF β levels were measured by ELISA. We found significantly higher circulating levels of MIF and of SCGF β in patients with idiopathic PAH (P=0.03 and P=0.004) and with PAH secondary to SSc (P=0.018 and P=0.023) compared to SSc patients without PAH. Higher levels of MIF were found in those patients with an higher New York Heart Association (NYHA) class (P=0.03). We can hypothesize that MIF and SCGF β are able to play a role in PAH, both idiopathic or secondary, and in the future they may be evaluated as useful biomarkers and prognostic factors for this serious vascular disease
Machine Understandable Policies and GDPR Compliance Checking
The European General Data Protection Regulation (GDPR) calls for technical
and organizational measures to support its implementation. Towards this end,
the SPECIAL H2020 project aims to provide a set of tools that can be used by
data controllers and processors to automatically check if personal data
processing and sharing complies with the obligations set forth in the GDPR. The
primary contributions of the project include: (i) a policy language that can be
used to express consent, business policies, and regulatory obligations; and
(ii) two different approaches to automated compliance checking that can be used
to demonstrate that data processing performed by data controllers / processors
complies with consent provided by data subjects, and business processes comply
with regulatory obligations set forth in the GDPR

Rapid Recovery of a Coral Reef at Darwin Island, Galapagos Islands
Surveys at Darwin Island in 2006 and 2007 have demonstrated that this northernmost Galapagos Islands coral reef has recovered significantly since the 1982–3 El Niño event. When first surveyed in 1975, this structural reef exhibited actively accreting frameworks of pocilloporid and poritid corals. The coral suffered severe mortality in 1983, resulting in the near total loss of pocilloporids and extensive partial mortality of poritid corals. Large sections of the reef had not recovered by 1992 and dead frameworks were subject to bio-erosion, although small numbers of sexual recruits of pocilloporid corals and numerous recruits plus regenerating patches of Porites lobata were present in some areas. An increase in live coral cover and recruitment was apparent through 2000 and 2002. Recent sampling at three sites along the reef has demonstrated mean (± 1 SD) live coral cover of 21.9 ± 1.7 % with P. lobata as the predominant species. Pocillopora spp. were present, but not so abundant as in earlier surveys. In spite of moderate erosion by echinoid and fish grazers, much of the original coral framework remained intact, providing a substrate for coral regeneration and recruitment. Recovery can be attributed to the original reef structure remaining intact, asexual regrowth of surviving tissues and sexual recruitment of poritid corals from surviving source populations
Kata Netizen tentang Kesetaraan Gender dalam Sentimen Warganet Twitter
Abstract: Gender equality is one of the goals in the Sustainable Development Goals. However, until now Indonesia is still having difficulties in achieving this goal. According to the United Nations Development Program (UNDP) data, Indonesia's Gender Inequality Index (GII) is ranked 107 out of 189 countries. In addition, according to The Global Gender Gap Index 2021 data by the World Economic Forum (WEF), Indonesia is ranked 105th out of 153 countries. This shows that Indonesia is still lagging behind in terms of gender equality. Therefore, this study aims to analyze the sentiments of Indonesian twitter netizens regarding gender equality in 2018-2021 and its accuracy. Data was collected from primary data, scraping twitter data with the keywords #kesetaraan and #gender in Indonesian. The method used is Lexicon-based Sentiment Analysis with AFINN-111 dictionary translated into Indonesian. The results obtained are that the percentage of positive sentiments tends to decrease from year to year except for 2021. On the contrary, the negative sentiments of Twitter tend to increase. This is due to controversial articles in RKUHP, RUU Cipta Kerja, Covid-19 pandemic, and the online gender-based violence. This shows that the gender equality in Indonesia is still minimal and needs to be improved.
