    Comparison of nutritional status of university students of two Asian countries

    Purpose – The purpose of this paper is to compare energy and macro-nutrients intake in university hostel students in two countries of Asia. Design/methodology/approach – Female students from the hostels of International Islamic University, Malaysia (IIUM), Kuantan Campus and NWFP, Agricultural University Peshawar, Pakistan were assessed for energy and macro-nutrients intake. A total of 140 students were registered who volunteered to participate in this study. The age range of the registered students was 22-26 years. On the day of the registration, age, height and weight were recorded; also, food frequency questionnaires (FFQs) were provided. The participants were asked to record alternately for three days whatever they ate during the prescribed week. Out of 140 students 139 returned the FFQs. From the anthropometry, the BMI was used to assess the under, ideal, over-weight and obese students. From the FFQs, energy and nutrient intakes were calculated using the food composition tables for Malaysia and Pakistan and compared with the recommended nutrients intakes (RNIs). Findings – The body weight for the required height among the Malaysian students was lower by 7.81 per cent than the reference value, whereas the body weight of Pakistani students matched to the reference weight for height. The Malaysian students were 28.0, 61.0, 5.5 and 0.9 per cent under, ideal, over-weight and obese, respectively, whereas Pakistani students were 100 per cent in the category of ideal-body weight. The total energy consumption was higher among Malaysian students by 9.93 per cent compared to the reference requirements, whereas the Pakistani students claimed to meet the requirements. In terms of nutrients balancing the Malaysian students were having the ideal combination of the macro-nutrients and it was within the recommended range of 55-60, 15-20 and 25-30 per cent for carbohydrates, protein and fat, respectively; whereas the balancing of the macro-nutrients was poor among the Pakistani students and met the energy requirements at the expense of fat consumption. Originality/value – The paper suggests that there is imbalance in macro-nutrients intake among the students

    3D animation as an alternative mean of da'wah for children

    3D animation cartoons has prodigious influence on children's mind, because children love watching cartoon. It is proved to influences family functioning in varied ways and has shaped our culture and social system. It affects the new generation with more aggressive personality and negative minds in sociality. Hence, it obstructed their learning of right behaviors. Concerned that this problem might become more severe, we tried relieve these problems by using a more proper 3D animation cartoon for children's entertainment as it is the closest media to the children. We have been motivated to propose a proselytizing or preaching of Islam (Da'wah) to prevent negative influences towards the children by proposing a suitable guideline for 3D animator in producing da'wah animation media more effectively

    Knowledge and attitude towards HIV/AIDS among transsexuals in Kuantan, Pahang

    Introduction: Lack of knowledge and negative attitude towards HIV/AIDS may be the risk factors for HIV infection among transsexuals. Research on knowledge and attitude towards HIV infection in transsexual communities is very limited at both local and international levels. This study aimed to assess the knowledge and attitude towards HIV infection among the male-to-female transsexual community in Kuantan, Pahang. Methods: A cross-sectional study was carried out from July to August 2014 among 33 male-to-female transsexuals in Kuantan, Pahang. Convenience sampling was used. Participants who gave consent answered a self-administered questionnaire. Data obtained was analyzed with descriptive statistics, χ2-test, and independent sample t test. Results: The majority of the subjects in this study were 29 years and below (48.5%), Muslims (93.9%), and had completed up to secondary education (60.6%). Most of them were sex workers (60.6%), and had relatively low income (no income to RM 3000, mean of RM1528). A total of 87.9% of the subjects demonstrated good knowledge and also positive attitude towards HIV/AIDS. Level of education was significantly associated with scores in knowledge (p=0.01). Conclusions: Despite the positive outcome from this study, misconceptions towards HIV/AIDS still exist among transsexuals. Education and interventions from multiple directions on HIV/AIDS are essential to deliver the correct information to this population, so as to emphasize prevention, early detection, and holistic medical care. Transsexuals also require attention from religious bodies and non-governmental organizations to help them in employment, financial, spiritual, and psycho-social issues

    Role of hyaluronan in human adipogenesis : evidence from in-vitro and in-vivo studies

    Hyaluronan (HA), an extra-cellular matrix glycosaminoglycan, may play a role in mesenchymal stem cell differentiation to fat but results using murine models and cell lines are conflicting. Our previous data, illustrating decreased HA production during human adipogenesis, suggested an inhibitory role. We have investigated the role of HA in adipogenesis and fat accumulation using human primary subcutaneous preadipocyte/fibroblasts (PFs, n = 12) and subjects of varying body mass index (BMI). The impact of HA on peroxisome proliferator-activated receptor gamma (PPARγ) expression was analysed following siRNA knockdown or HA synthase (HAS)1 and HAS2 overexpression. PFs were cultured in complete or adipogenic medium (ADM) with/without 4-methylumbelliferone (4-MU = HA synthesis inhibitor). Adipogenesis was evaluated using oil red O (ORO), counting adipogenic foci, and measurement of a terminal differentiation marker. Modulating HA production by HAS2 knockdown or overexpression increased (16%, p < 0.04) or decreased (30%, p = 0.01) PPARγ transcripts respectively. The inhibition of HA by 4-MU significantly enhanced ADM-induced adipogenesis with 1.52 ± 0.18- (ORO), 4.09 ± 0.63- (foci) and 2.6 ± 0.21-(marker)-fold increases compared with the controls, also increased PPARγ protein expression (40%, (p < 0.04)). In human subjects, circulating HA correlated negatively with BMI and triglycerides (r = −0.396 (p = 0.002), r = −0.269 (p = 0.038), respectively), confirming an inhibitory role of HA in human adipogenesis. Thus, enhancing HA action may provide a therapeutic target in obesity

    An auto lifting device to lift manhole cover with ergonomics consideration

    The sewerage and maintenance process of manhole is important to ensure that the underground pipelines and other systems are in good condition. The manhole is covered with manhole cover to prevent people, animals or any objects fall into it. The manhole cover has one pocket which is to lock it and ensure that nobody open it. A questionnaire survey is distributed to the workers at Indah Water Konsortium Sdn Bhd, Kuala Lumpur and interviews are conducted with Indah Water Konsortium (IWK) and Jabatan Perkhidmatan Pembentungan (JPP), Selangor to access the user requirements and needs for an effective manhole cover lifter. Currently, the workers used a T-hook to unlock the heavy manhole cover and lift it. However, this method affected some of the workers because they experienced back, legs, shoulders and arm injuries. The T-hook is also quite heavy and it is difficult to bring anywhere. Based on the result of questionnaire survey, four concepts are designed to create new manhole cover lifter with automated operation. The best concept is finalized by using concept screening and scoring method. The chosen concept is applied pulley chain mechanism and it is selected as the best concept because it is easy and safe to use, low cost maintenance, low time consuming and affordable price. The design of the device is suitable for Malaysian male and secured them from musculoskeletal disorder. The worker's posture is analyzed by using Rapid Upper Limb Analysis (RULA) ergonomics analysis. The stability and strength of the device is analyzed by calculation and Finite Element Analysis (FEA) is used to analyze the screw cylinder

    Combining micro-RNA and protein sequencing to detect robust biomarkers for Graves’ disease and orbitopathy

    Graves’ Disease (GD) is an autoimmune condition in which thyroid-stimulating antibodies (TRAB) mimic thyroid-stimulating hormone function causing hyperthyroidism. 5% of GD patients develop inflammatory Graves’ orbitopathy (GO) characterized by proptosis and attendant sight problems. A major challenge is to identify which GD patients are most likely to develop GO and has relied on TRAB measurement. We screened sera/plasma from 14 GD, 19 GO and 13 healthy controls using high-throughput proteomics and miRNA sequencing (Illumina’s HiSeq2000 and Agilent-6550 Funnel quadrupole-time-of-flight mass spectrometry) to identify potential biomarkers for diagnosis or prognosis evaluation. Euclidean distances and differential expression (DE) based on miRNA and protein quantification were analysed by multidimensional scaling (MDS) and multinomial regression respectively. We detected 3025 miRNAs and 1886 proteins and MDS revealed good separation of the 3 groups. Biomarkers were identified by combined DE and Lasso-penalized predictive models; accuracy of predictions was 0.86 (±0:18), and 5 miRNA and 20 proteins were found including Zonulin, Alpha-2 macroglobulin, Beta-2 glycoprotein 1 and Fibronectin. Functional analysis identified relevant metabolic pathways, including hippo signaling, bacterial invasion of epithelial cells and mRNA surveillance. Proteomic and miRNA analyses, combined with robust bioinformatics, identified circulating biomarkers applicable to diagnose GD, predict GO disease status and optimize patient management

    Expression of endogenous putative TSH binding protein in orbit

    Thyroid stimulating antibodies (TSAB) cause Graves’ disease and contribute to Graves’ Orbitopathy (GO) pathogenesis. We hypothesise that the presence of TSH binding proteins (truncated TSHR variants (TSHRv)) and/or nonclassical ligands such as thyrostimulin (α2β5) might provide a mechanism to protect against or exacerbate GO. We analysed primary human orbital preadipocyte-fibroblasts (OF) from GO patients and people free of GO (non-GO). Transcript (QPCR) and protein (western blot) expression levels of TSHRv were measured through an adipogenesis differentiation process. Cyclic-AMP production by TSHR activation was studied using luciferase-reporter and RIA assays. After differentiation, TSHRv levels in OF from GO were significantly higher than non-GO (p = 0.039), and confirmed in ex vivo analysis of orbital adipose samples. TSHRv western blot revealed a positive signal at 46 kDa in cell lysates and culture media (CM) from non-GO and GO-OF. Cyclic-AMP decreased from basal levels when OF were stimulated with TSH or Monoclonal TSAB (M22) before differentiation protocol, but increased in differentiated cells, and was inversely correlated with the TSHRv:TSHR ratio (Spearman correlation: TSH r = −0.55, p = 0.23, M22 r = 0.87, p = 0.03). In the bioassay, TSH/M22 induced luciferase-light was lower in CM from differentiated GO-OF than non-GO, suggesting that secreted TSHRv had neutralised their effects. α2 transcripts were present but reduced during adipogenesis (p < 0.005) with no difference observed between non-GO and GO. β5 transcripts were at the limit of detection. Our work demonstrated that TSHRv transcripts are expressed as protein, are more abundant in GO than non-GO OF and have the capacity to regulate signalling via the TSHR

    Longitudinal characterization of autoantibodies to the thyrotropin receptor (TRAb) during alemtuzumab therapy; evidence that TRAb may precede thyroid dysfunction by many years.

    BACKGROUND Thyroid autoimmunity, especially Graves’ disease or hypothyroidism with positive autoantibodies (TRAb) to the thyrotropin receptor (TSHR), occurs in 30-40% of patients with relapsing multiple sclerosis (MS) following treatment with alemtuzumab (ALTZ). ALTZ therapy therefore provides a unique opportunity to study the evolution of TRAb prior to clinical presentation. TRAb can stimulate (TSAb), block (TBAb) or not affect (“neutral”: TNAb) the TSHR function, causing hyperthyroidism, hypothyroidism or euthyroidism, respectively. METHODS We conducted a longitudinal retrospective analysis of TRAb bioactivity over a period of 9 years in 45 MS patients receiving ALTZ using available stored serum; 31 developed thyroid dysfunction (TD) and 14 remained euthyroid despite being followed for a minimum of 5 years (NO-TD). The presence of TRAb was evaluated at standardized time points: A) pre-ALTZ, B) latest time available post-ALTZ and before TD onset, C) post-ALTZ during/after TD onset. Serum TRAb were detected by published in-house assays (ihTRAb): flow cytometry (FC) detecting any TSHR-binding TRAb and luciferase bioassays (LB) detecting TSAb/TBAb bioactivity. Purified IgGs were used to verify TSAb/TBAb in selected hypothyroid cases. Standard clinical automated measurements of TRAb (autTRAb), anti-thyroid peroxidase autoantibodies (TPOAb), thyroid stimulating hormone, free-thyroxine and free-triiodothyronine were also collected. RESULTS Pre-ALTZ, combined ihTRAb (positive with FC and/or LB), but not autTRAb, were present in 5/16 (31.2%) TD versus 0/14 (0%) NO-TD (p=0.017). Detectable ihTRAb preceded TD development in 9/28 (32.1%) and by a median of 1.2 years (range 28 days – 7.3 years). Combination testing of ihTRAb and TPOAb at baseline predicted 20% of subsequent cases of hyperthyroidism and 83% of hypothyroidism. CONCLUSIONS We present evidence that TRAb measured with custom-made assays can be detected prior to any change in thyroid function in up to a third of cases of ALTZ-related TD. Furthermore, The presence of ihTRAb prior to ALTZ treatment was strongly predictive of subsequent TD. Our findings suggest that a period of affinity maturation of TRAb may precede clinical disease onset in some cases. Combined testing of TPOAb and ihTRAb may increase our ability to predict those who will develop thyroid dysfunction post ALTZ

    Distinctive features of orbital adipose tissue (OAT) in Graves’ Orbitopathy

    Depot specific expansion of orbital-adipose-tissue (OAT) in Graves’ Orbitopathy (GO) is associated with lipid metabolism signaling defects. We hypothesize that the unique adipocyte biology of OAT facilitates its expansion in GO. A comprehensive comparison of OAT and white-adipose-tissue (WAT) was performed by light/electron-microscopy, lipidomic and transcriptional analysis using ex vivo WAT, healthy OAT (OAT-H) and OAT from GO (OAT-GO). OAT-H/OAT-GO have a single lipid-vacuole and low mitochondrial number. Lower lipolytic activity and smaller adipocytes of OAT-H/OAT-GO, accompanied by similar essential linoleic fatty acid (FA) and (low) FA synthesis to WAT, revealed a hyperplastic OAT expansion through external FA-uptake via abundant SLC27A6 (FA-transporter) expression. Mitochondrial dysfunction of OAT in GO was apparent, as evidenced by the increased mRNA expression of uncoupling protein 1 (UCP1) and mitofusin-2 (MFN2) in OAT-GO compared to OAT-H. Transcriptional profiles of OAT-H revealed high expression of Iroquois homeobox-family (IRX-3&5), and low expression in HOX-family/TBX5 (essential for WAT/BAT (brown-adipose-tissue)/BRITE (BRown-in-whITE) development). We demonstrated unique features of OAT not presented in either WAT or BAT/BRITE. This study reveals that the pathologically enhanced FA-uptake driven hyperplastic expansion of OAT in GO is associated with a depot specific mechanism (the SLC27A6 FA-transporter) and mitochondrial dysfunction. We uncovered that OAT functions as a distinctive fat depot, providing novel insights into adipocyte biology and the pathological development of OAT expansion in GO
