Transzglutaminázok vizsgálata a proliferatív vitreoretinopátiában és a neutrofil extracelluláris csapda képződésében
A transzglutaminázok (TGázok) széles körben vizsgált Ca2+-függő enzimek, melyek változatos funkciókkal rendelkeznek. Számos esszenciális fiziológiás és patológiás folyamatban játszanak szerepet azáltal, hogy katalizálják az ɛ-(γ-glutamil)-lizin izopeptid kötések létrejöttét a lizin és glutamin aminosav oldalláncok között.
Munkám első felében az volt a célom, hogy egerek üvegtesti mintáiban kétdimenziós gélelektroforézis és tömegspektrometria alapú fehérje azonosítás alkalmazásával olyan fehérjéket azonosítsak, amelyek összefüggésben állnak a proliferatív vitreoretinopátiával (PVR). TG2 jelenlétében és hiányában egyaránt sikerült kimutatnom a PVR-hez köthető fehérjéket az egér üvegtesti mintákban. A diszpáz kezelés hatására a krisztallinok különböző formáinak mennyiségében figyeltem meg a legjelentősebb változásokat, mindemellett eltérő krisztallin profil változást tapasztaltam a vad típusú PVR és TG2 knock-out PVR csoportokban. Munkám során sikerrel alkalmaztam a kétdimenziós elektroforézist és a PVR egérmodellt a diszpáz által indukált PVR-re jellemző fehérje változások és a TG2 hiányában bekövetkező változások tanulmányozására. Eredményeim jó egyezést mutatnak az irodalmi adatokkal, de további funkcionális vizsgálatok szükségesek a megfigyelt jelenség mögött húzódó molekuláris magyarázat felderítésére.
Munkám második felében az volt a célom, hogy megvizsgáljam a fehérje keresztkötéseket, illetve azt, hogy hogyan változik a fehérje keresztkötés-profil a neutrofil extracelluláris csapda (NET) képződés során 5-(biotinamido)-pentil-amin és spermin kezelések hatására. A NET fehérjéket elektroforézis és folyadék kromatográffal-kapcsolt tandem tömegspektrometriával tanulmányoztam 4000 QTRAP tömegspektrométeren. Az MS/MS adatok alapján a keletkezett keresztkötések helyét és típusát StavroX keresztkötött fehérje vizsgáló szoftver segítségével azonosítottam. A fehérje keresztkötés mintázatokban változásokat mutattam ki a kezelések hatására. Adataim alapján megállapítható, hogy a keresztkötött fehérje hálózat kialakításáért két folyamat felelős: a TGázok és az mieloperoxidáz (MPO) által katalizált folyamatok, amelyek poliamin klóraminok keresztkötését hozzák létre. Adataink azt mutatták, hogy az MPO és a TGázok együttes jelenléte szükséges a NET megfelelő stabilizációjához.Transglutaminases (TGases) are widely investigated Ca2+-dependent enzymes with diverse functions. They play a role in numerous essential physiological and pathological processes via catalyzing the isopeptide bond formation between lysine and glutamine residues leading to the formation of ɛ-(γ-glutamyl)-lysine bonds.
In the first study our aim was to identify proteins associated with proliferative vitreoretinopathy (PVR) in the vitreous bodies of mice using two dimensional electrophoresis and mass spectrometry-based protein identification. We could demonstrate proteins related to PVR in the vitreous bodies of mice both in the presence and lack of TG2. Highest changes in the amount of different forms of crystallins upon dispase treatment were detected. In addition a different crystallin profile change was observed in the wild-type PVR group compared to TG2 knock-out PVR group. The utilization of PVR mice model along with two-dimension gel electrophoresis to analyze the protein changes upon dispase induced PVR formation and the effect of the lack of TG2 was successful. The results are in accordance with data reported in the literature, but further functional studies are needed to elucidate the molecular basis of this phenomenon.
In the second study our aim was to investigate the protein cross-links and how the protein crosslink-profile changes during neutrophil extracellular trap (NET) formation upon BPNH2 and SPM treatments. NET proteins were analyzed by electrophoresis and HPLC-coupled tandem mass spectrometry on a 4000 QTRAP mass spectrometer. Based on MS/MS data, the site and the type of cross-links formed were identified using StavroX protein cross-link examination software. We could demonstrate the changes in the protein cross-linking patterns upon the applied treatments and according to our data for the cross-linked protein network formation two processes are responsible: both the TGase and the myeloperoxidase (MPO)-catalyzed polyamine chloramine mediated crosslinks are important. Our data showed that the presence of MPO and TGases together is required for proper NET stabilization.d

Association of PPAR Alpha Intron 7 G/C, PPAR Gamma 2 Pro12Ala, and C161T Polymorphisms with Serum Fetuin-A Concentrations
BACKGROUND: Both peroxisome activator proteins (PPARs) and fetuin-A play a role in lipid and glucose metabolism. AIMS: We investigated whether PPARalpha intron 7 G2468/C and PPARgamma2 Pro12Ala and PPARgamma exon 6 C161T polymorphisms are associated with serum fetuin-A concentrations. PATIENTS AND METHODS: The PPARalpha intron 7 G/C polymorphism was studied in cohort 1 (79 reference individuals, 165 postinfarction patients). The two PPARgamma polymorphisms were investigated in cohort 2 (162 reference individuals, 165 postinfarction patients). Fetuin-A levels and PPAR polymorphisms were determined by radial immunodiffusion and polymerase chain reaction-restriction fragment length polymorphism techniques. RESULTS: The C allele variant of PPARalpha intron 7 G2467C was associated with higher fetuin-A levels (p = 0.018). Postinfarction status (p = 0.001), PPARalpha intron 7 GG/GC/CC genotypes (p = 0.032), and the C allele (p = 0.021) were the strongest determinants of fetuin-A concentration in a multiple regression model. Higher fetuin-A levels were associated with the Pro variant of PPARgamma2 (p = 0.047). Postinfarction status (p = 0.041) and BMI (p < 0.001) but not PPARgamma2 Pro were the strongest determinants of fetuin-A concentrations. PPARgamma exon 6 C161T genotypes were not associated with fetuin-A levels. CONCLUSIONS: Fetuin-A was determined mainly by the PPARalpha intron 7C allele and postinfarction status in cohort 1 and the BMI and postinfarction in cohort 2. A diabetes és a depresszió együttes előfordulása, lehetséges kétirányú kapcsolata és szűrésük jelentősége
Absztrakt:
A cukorbetegség és a depresszió világszerte népbetegség. Mindkettő kialakulása
multifaktoriális, amelyben szerepet játszhatnak külső, környezeti tényezők,
valamint belső, a szervezetben játszódó folyamatok, többek között különböző
gének nem megfelelő működése. Bár a diabetes és a depresszió komorbiditása jól
ismert, a két betegség hátterében zajló folyamatok, illetve ezen folyamatok
egymásra hatása nem minden részletet illetően tisztázott. A szerzők összefoglaló
közleményének célja bemutatni a 2-es típusú cukorbetegség és a depresszió
közötti kétirányú kapcsolat hátterében álló ismert és még csak feltételezett
mechanizmusokat, illetve felhívni a figyelmet a két betegség egyidejű szűrésének
és kezelésének fontosságára, hiszen együttes fennállásukkor nőhet a morbiditás
és a mortalitás. A cukorbetegek depresszióra irányuló szűrésével és a
depressziós betegek diabetes irányú kivizsgálásával csökkenthető lehet a
komorbiditásból származó komplikációk és szövődmények, illetve a kezelési
nehézségek előfordulása. A két betegség ellátásában fontos szerepe van a
családorvosnak, aki a korai diagnózissal és kezeléssel javíthatja a betegek
életminőségét, lassíthatja a betegségek progresszióját és a szövődmények

Abstract:
Diabetes and depression are considered global epidemics. Both have multifactorial
aetiologies, including external, environmental factors and internal factors in
connection with physiological processes as well as genetic variants triggering
disease onset. Although the co-occurrence of diabetes and depression is well
described, the mechanisms underlying these diseases and their interactions are
still not entirely revealed. The authors aimed to present known and potential
explanations of the co-occurrence of these diseases and to highlight the
importance of their timely diagnosis and effective treatment, as their
co-occurrence may increase morbidity and mortality. Screening for depression
among diabetes patients and for diabetes among patients with depression may
decrease the incidence of complications and consequences of this comorbidity and
may foster more effective treatment. General practitioners play a key role in
the care of both diseases by providing timely diagnosis and adequate treatment
potentially leading to a better quality of life, slower disease progression and

Serum fetuin-A, tumor necrosis factor alpha and C-reactive protein concentrations in patients with hereditary angioedema with C1-inhibitor deficiency
Abstract Background and aims Hereditary angioedema with C1-inhibitor deficiency (C1-INH-HAE) is characterized by localized, non-pitting, and transient swelling of submucosal or subcutaneous region. Human fetuin-A is a multifunctional glycoprotein that belongs to the proteinase inhibitor cystatin superfamily and has structural similarities to the high molecular weight kininogen. Fetuin-A is also known a negative acute phase reactant with anti-inflammatory characteristics. In this study we aimed to determine serum fetuin-A, C-reactive protein (CRP) and tumor necrosis factor alpha (TNFα) concentrations in patients with C1-INH-HAE during symptom-free period and during attacks and compare them to those of healthy controls. Further we analyzed possible relationship among these parameters as well as D-dimer levels which was known as marker of HAE attacks. Patients and methods Serum samples of 25 C1-INH-HAE patients (8 men, 17 women, age: 33.1 ± 6.9 years, mean ± SD) were compared to 25 healthy controls (15 men, 10 women, age: 32.5 ± 7.8 years). Serum fetuin-A and TNFα concentrations were determined by ELISA, CRP and D-dimer by turbidimetry. Results Compared to healthy controls patients with C1-INH-HAE in the symptom-free period had significantly decreased serum fetuin-A 258 μg/ml (224–285) vs. 293 μg/ml (263–329), (median (25–75% percentiles, p = 0.035) and TNFα 2.53 ng/ml (1.70–2.83) vs. 3.47 ng/ml (2.92–4.18, p = 0.0008) concentrations. During HAE attacks fetuin-A levels increased from 258 (224–285) μg/ml to 287 (261–317) μg/ml (p = 0.021). TNFα and CRP levels did not change significantly. We found no significant correlation among fetuin-A CRP, TNFα and D-dimer levels in any of these three groups. Conclusions Patients with C1-INH-HAE have decreased serum fetuin-A concentrations during the symptom-free period. Given the anti-inflammatory properties of fetuin-A, the increase of its levels may contribute to the counter-regulation of edema formation during C1-INH-HAE attacks
Human Fetuin-A Rs4918 Polymorphism and its Association with Obesity in Healthy Persons and in Patients with Myocardial Infarction in Two Hungarian Cohorts.
BACKGROUND Human fetuin A (AHSG) has been associated with the development of obesity, insulin resistance, type 2 diabetes mellitus, and atherosclerosis. Observations on the role of AHSG rs4918 single-nucleotide polymorphism are contradictory. We investigated the association between variants of rs4918 and parameters of obesity, lipid status, tumor necrosis factor-alpha (TNFalpha), adipokines (adiponectin, resistin, leptin), and insulin resistance in healthy persons and in patients with previous myocardial infarction. MATERIAL AND METHODS This was a cross-sectional study comprising cohort 1 (81 healthy individuals) and cohort 2 (157 patients with previous myocardial infarction). We used the allele-specific KASP genotyping assay to detect rs4918 polymorphism. RESULTS In cohort 1, G-nucleotide carriers had significantly lower serum TNFalpha, adiponectin, and higher leptin concentrations than in non-G carriers. These differences, however, were not observed in cohort 2. In cohort 2, G-carriers had lower BMI and waist circumferences than in non-G carriers. The G allele was more frequent among lean than obese patients (RR=1.067, 95%CI=1.053-2.651, p=0.015). An association between BMI and rs4918 polymorphism was observed among patients without diabetes (CC/CG/GG genotypes: p=0.003, G vs. non-G allele: p=0.008) but not in diabetics. In addition, a strong linearity between BMI and the CC/CG/GG genotypes (association value: 4.416, p=0.036) and the frequency of the G allele (7.420, p=0.006) could be identified. In cohort 2, non-obese, non-diabetic G-carriers still had lower BMI and waist circumferences than in non-G carriers. CONCLUSIONS The rs4918 minor variant is associated with lower TNFalpha and adiponectin, higher leptin levels in healthy persons, and more favorable anthropomorphic parameters of obesity in cohort 2
Hivatással és hálapénzzel kapcsolatos vélekedések a magyarországi családorvosok és családorvos rezidensek körében | Opinions of Hungarian family physicians and residents on vocation and informal payment
Absztrakt
Bevezetés: A XXI. században a családorvosi hivatás változása
figyelhető meg. Az elvándorlás, a gyógyítók elöregedése és a hálapénz kérdése
meghatározóak az egészségügyi szektor humánerőforrás-krízise szempontjából.
Célkitűzés: A szerzők célul tűzték ki a hazai családorvosok
és családorvos rezidensek hivatásról és a hálapénzről való vélekedéseinek
feltárását. Módszer: Feltáró jellegű kvantitatív vizsgálat
családorvosok (n = 363) és családorvos rezidensek (n = 180) körében. Jelen
elemzés középpontjában a jövedelemre vonatkozó kérdések, a hálapénzzel és
hivatással kapcsolatos vélemények álltak. Eredmények: A
válaszadók hivatásválasztásában és hivatástudatában az altruizmus, a szolgálat
és a felelősségvállalás a legmeghatározóbb tényezők. A paraszolvencia
tekintetében a rezidensminta szignifikánsan elutasítóbb (19,7% vs. 38,3%;
p<0,001). A rezidensek kisebb összeget tartanak elfogadhatónak (14,3% vs.
8,9%; p<0,034), valamint nagyobb arányban mondják azt, hogy a befolyt
jövedelmet praxisfejlesztésre fordítanák (1,4% vs. 9,4%; p<0,023).
Következtetések: A családorvosok és a családorvos
rezidensek hivatással kapcsolatos legfontosabb attitűdjei közel azonosak, de a
hálapénz kérdéséről a két generáció másként vélekedik. Orv. Hetil., 2016,

Abstract
Introduction: The changing of the family medicine can be
observed in the New Millennium. Migration, the aging of the healers and informal
payment are crucial to the human resource crisis of the health sector.
Aim: The aim of this study was to investigate the family
physicians’ and residents’ opinions about the vocation and informal payment.
Method: Exploratory, quantitative study was carried out
among family physicians (n = 363) and family physician residents (n = 180). The
central questions of the study were the vocation, the income and the informal
payment. Results: The most decisive factors of the carrier
choice were altruism, service and responsibility. Residents were significantly
rejective (19.7% vs. 38.3%, p<0.001) about informal payment. They would
accept smaller amounts of informal payment (14.3% vs. 8.9%, p<0.034), and
would spend it on praxis development (1.4% vs.9.4% p<0.023).
Conclusions: The attitudes of family physicians and
residents are the same in case of the vocation, but on the issue of informal
payment, the two generations have different opinions. Orv. Hetil., 2016,
