    Prospective association between late-life physical activity and hospital care utilisation: a 7-year nationwide follow-up study

    BACKGROUND: It is still equivocal whether there is a potential role of late-life physical activity in ameliorating the challenges of increasing healthcare expenditure due to the consequence of global population ageing. OBJECTIVE: this study aimed to examine the prospective association between physical activity and subsequent hospital care utilisation in older adults and to explore the optimal dose of physical activity required to reduce hospital care utilisation. DESIGN: this was a prospective cohort study based on the data from the Taiwan 2005 National Health Interview Survey, which were linked to the 2005-12 claims data from the National Health Insurance system. PARTICIPANTS: 1,760 older adults aged 65 or more. METHODS: the frequency, duration and intensity for physical activity were assessed, and total physical activity energy expenditure was estimated. The average annualised hospital care utilisation for the period 2006 through 2012, including number of hospitalisations, number of days in hospital and the costs of hospitalisation, were calculated. RESULTS: older adults engaging in at least moderate volume of physical activity (≥1,000 kcal/week) experienced fewer subsequent hospital admissions and fewer days in hospital than did sedentary individuals, after adjusting for covariates. Trends for reduced hospitalisation costs were also found. These associations persisted in sensitivity analyses, including tests of reverse causation. CONCLUSION: this study has provided evidence that older adults who are at least moderately active may minimise utilisation of hospital care services. The findings highlight the importance of maintaining a physically active lifestyle in later life

    SECaps: A Sequence Enhanced Capsule Model for Charge Prediction

    Automatic charge prediction aims to predict appropriate final charges according to the fact descriptions for a given criminal case. Automatic charge prediction plays a critical role in assisting judges and lawyers to improve the efficiency of legal decisions, and thus has received much attention. Nevertheless, most existing works on automatic charge prediction perform adequately on high-frequency charges but are not yet capable of predicting few-shot charges with limited cases. In this paper, we propose a Sequence Enhanced Capsule model, dubbed as SECaps model, to relieve this problem. Specifically, following the work of capsule networks, we propose the seq-caps layer, which considers sequence information and spatial information of legal texts simultaneously. Then we design a attention residual unit, which provides auxiliary information for charge prediction. In addition, our SECaps model introduces focal loss, which relieves the problem of imbalanced charges. Comparing the state-of-the-art methods, our SECaps model obtains 4.5% and 6.4% absolutely considerable improvements under Macro F1 in Criminal-S and Criminal-L respectively. The experimental results consistently demonstrate the superiorities and competitiveness of our proposed model.Comment: 13 pages, 3figures, 5 table

    Physical activity, smoking, and the incidence of clinically diagnosed insomnia

    OBJECTIVE: This study was designed to examine the independent and combined associations of physical activity and smoking on the incidence of doctor-diagnosed insomnia using a nationally representative sample over seven years, taking into account other relevant covariates. METHODS: Participants aged 18 years or older in the 2005 Taiwan National Health Interview Survey (NHIS) with links to National Health Insurance (NHI) claim data between 2005 and 2012 and without diagnosed insomnia before 2005, were selected into this study (n = 12,728). Participants were classified as having insomnia with International Classification of Diseases, Ninth Revision (ICD-9) CM codes 307.41, 307.42, or 780.52. Self-reported smoking status and frequency, duration, and types of leisure-time and non−leisure-time physical activities were collected. Metabolic equivalent (MET) intensity levels for each activity were assigned, and weekly energy expenditure of each activity was calculated and summed. RESULTS: Inactive participants had a higher risk of incident insomnia [hazard ratio (HR) = 1.22, 95% confidence interval (CI) = 1.06–1.42, p = 0.007] than the active group, and ever-smokers were more likely to have incident insomnia than never smokers (HR = 1.45, 95% CI = 1.20–1.76, p < 0.001). Compared with the nonsmoker/active group, the ever-smoker/inactive group had a higher risk of incident insomnia (HR = 1.78, 95% CI = 1.41–2.25, p < 0.001). Sensitivity analyses excluding individuals diagnosed with other sleep disorders or mental disorders yielded similar results, with the ever-smoker/inactive group having the highest risk of insomnia. CONCLUSIONS: Inactive adults and smokers are at higher risk for incident insomnia, highlighting the importance of a healthy lifestyle and pointing to strategies such as encouraging smoking cessation and physical activity to avoid insomnia among adults

    Creation and suppression of point defects through a kick-out substitution process of Fe in InP

    Indium antisite defect In P-related photoluminescence has been observed in Fe-diffused semi-insulating (SI) InP. Compared to annealed undoped or Fe-predoped SI InP, there are fewer defects in SI InP obtained by long-duration, high-temperature Fe diffusion. The suppression of the formation of point defects in Fe-diffused SI InP can be explained in terms of the complete occupation by Fe at indium vacancy. The In P defect is enhanced by the indium interstitial that is caused by the kick out of In and the substitution at the indium site of Fe in the diffusion process. Through these Fe-diffusion results, the nature of the defects in annealed undoped SI InP is better understood. © 2002 American Institute of Physics.published_or_final_versio

    Improving interface bonding of double-skinned CFST columns

    It has been demonstrated that high-strength concrete (HSC) is able to improve the strength-to-weight ratio of reinforced concrete columns and maximise the usable areas of tall buildings. However, closely spaced transverse reinforcement needs to be installed to provide stronger confinement for averting brittle failure of HSC. To resolve the problem, double-skinned concrete-filled-steel-tubular (CFST) columns have been advocated, which eliminates the steel congestion problem for better concrete placing and reduces the concrete arching action thus providing a more uniform confining pressure. Despite these advantages, a major shortcoming of double-skinned CFST columns is that imperfect interface bonding occurs in the elastic stage that reduces elastic strength and stiffness. Thus, the authors proposed to adopt external confinement to restrict the lateral dilation of the outer tube of double-skinned CFST columns. To verify the effectiveness of the proposed external rings, a total of 20 double-skinned normal- and high-strength CFST columns were tested. From the test results, it was observed that the stiffness, axial load-carrying capacity and ductility of ringconfined double-skinned CFST columns were significantly higher than the unconfined columns

    Quasi two-dimensional dye-sensitized In2O3 phototransistors for ultrahigh responsivity and photosensitivity photodetector applications

    We report the development of dye-sensitized thin-film phototransistors consisting of an ultrathin layer (&lt;10 nm) of indium oxide (In2O3) the surface of which is functionalized with a self-assembled monolayer of the light absorbing organic dye D102. The resulting transistors exhibit a preferential color photoresponse centered in the wavelength region of ∼500 nm with a maximum photosensitivity of ∼106 and a responsivity value of up to 2 × 103 A/W. The high photoresponse is attributed to internal signal gain and more precisely to charge carriers generated upon photoexcitation of the D102 dye which lead to the generation of free electrons in the semiconducting layer and to the high photoresponse measured. Due to the small amount of absorption of visible photons, the hybrid In2O3/D102 bilayer channel appears transparent with an average optical transmission of &gt;92% in the wavelength range 400-700 nm. Importantly, the phototransistors are processed from solution-phase at temperatures below 200 °C hence making the technology compatible with inexpensive and temperature sensitive flexible substrate materials such as plastic

    On the selection and design of proteins and peptide derivatives for the production of photoluminescent, red-emitting gold quantum clusters

    Novel pathways of the synthesis of photoluminescent gold quantum clusters (AuQCs) using biomolecules as reactants provide biocompatible products for biological imaging techniques. In order to rationalize the rules for the preparation of red-emitting AuQCs in aqueous phase using proteins or peptides, the role of different organic structural units was investigated. Three systems were studied: proteins, peptides, and amino acid mixtures, respectively. We have found that cysteine and tyrosine are indispensable residues. The SH/S-S ratio in a single molecule is not a critical factor in the synthesis, but on the other hand, the stoichiometry of cysteine residues and the gold precursor is crucial. These observations indicate the importance of proper chemical behavior of all species in a wide size range extending from the atomic distances (in the AuI-S semi ring) to nanometer distances covering the larger sizes of proteins assuring the hierarchical structure of the whole self-assembled system

    Polynomial Growth Harmonic Functions on Finitely Generated Abelian Groups

    In the present paper, we develop geometric analytic techniques on Cayley graphs of finitely generated abelian groups to study the polynomial growth harmonic functions. We develop a geometric analytic proof of the classical Heilbronn theorem and the recent Nayar theorem on polynomial growth harmonic functions on lattices \mathds{Z}^n that does not use a representation formula for harmonic functions. We also calculate the precise dimension of the space of polynomial growth harmonic functions on finitely generated abelian groups. While the Cayley graph not only depends on the abelian group, but also on the choice of a generating set, we find that this dimension depends only on the group itself.Comment: 15 pages, to appear in Ann.     Self-rated health in middle-aged and elderly Chinese : distribution, determinants and associations with cardio-metabolic risk factors

    Background: Self-rated health (SRH) has been demonstrated to be an accurate reflection of a person's health and a valid predictor of incident mortality and chronic morbidity. We aimed to evaluate the distribution and factors associated with SRH and its association with biomarkers of cardio-metabolic diseases among middle-aged and elderly Chinese. Methods: Survey of 1,458 men and 1,831 women aged 50 to 70 years, conducted in one urban and two rural areas of Beijing and Shanghai in 2005. SRH status was measured and categorized as good (very good and good) vs. not good (fair, poor and very poor). Determinants of SRH and associations with biomarkers of cardio-metabolic diseases were evaluated using logistic regression. Results: Thirty two percent of participants reported good SRH. Males and rural residents tended to report good SRH. After adjusting for potential confounders, residence, physical activity, employment status, sleep quality and presence of diabetes, cardiovascular disease, and depression were the main determinants of SRH. Those free from cardiovascular disease (OR 3.68; 95%CI 2.39; 5.66), rural residents (OR 1.89; 95% CI 1.47; 2.43), non-depressed participants (OR 2.50; 95% CI 1.67; 3.73) and those with good sleep quality (OR 2.95; 95% CI 2.22; 3.91) had almost twice or over the chance of reporting good SRH compared to their counterparts. There were significant associations -and trend- between SRH and levels of inflammatory markers, insulin levels and insulin resistance. Conclusion: Only one third of middle-aged and elderly Chinese assessed their health status as good or very good. Although further longitudinal studies are required to confirm our findings, interventions targeting social inequalities, lifestyle patterns might not only contribute to prevent chronic morbidity but as well to improve populations' perceived health
