Junior doctorsâ communication with hospital pharmacists about prescribing: findings from a qualitative interview study
Objectives To explore factors affecting communication between Foundation Year (FY) 1 doctors and hospital pharmacists about prescribing from the junior doctorsâ perspective.
Methods Trained interviewers (n=4) conducted semistructured interviews with FY1 doctors who were purposively sampled from three hospitals in England. FY1 doctors were asked about their experiences of communication with hospital pharmacists about their prescribing; instances where they disagreed with or did not implement a hospital pharmacistâs recommendation; and their preferences for communicating with hospital pharmacists about prescribing. Interviews were audiorecorded, transcribed verbatim and analysed thematically.
Results A total of 27 FY1 doctors were interviewed. Findings were categorised into four main themes: (1) nature and context of communication; (2) FY1 doctorsâ perceptions of communication with hospital pharmacists; (3) factors influencing FY1 doctorsâ decision whether to act on pharmacistsâ prescribing recommendations; and (4) suggestions to improve communication with pharmacists. FY1 doctors and hospital pharmacists generally communicated well. FY1 doctors appreciated and frequently acted on pharmacistsâ advice yet there was deference to senior medical staff when advice differed. Joint ward rounds, pharmacist-led teaching sessions and a standardised approach to communication were all suggested as ways to improve communication and may increase the likelihood of pharmacistsâ recommendations being acted on.
Conclusions FY1 doctors and hospital pharmacists communicated frequently about medication prescribing. Issues occurred when there were differences in professional judgement between senior medical staff and pharmacists but these were usually resolved satisfactorily for the FY1 doctor. Further interventions to improve communication and safe prescribing could involve a multidisciplinary and systems approach

Linking the effects of helminth infection, diet and the gut microbiota with human whole-blood signatures
Helminth infection and dietary intake can affect the intestinal microbiota, as well as the immune system. Here we analyzed the relationship between fecal microbiota and blood profiles of indigenous Malaysians, referred to locally as Orang Asli, in comparison to urban participants from the capital city of Malaysia, Kuala Lumpur. We found that helminth infections had a larger effect on gut microbial composition than did dietary intake or blood profiles. Trichuris trichiura infection intensity also had the strongest association with blood transcriptional profiles. By characterizing paired longitudinal samples collected before and after deworming treatment, we determined that changes in serum zinc and iron levels among the Orang Asli were driven by changes in helminth infection status, independent of dietary metal intake. Serum zinc and iron levels were associated with changes in the abundance of several microbial taxa. Hence, there is considerable interplay between helminths, micronutrients and the microbiota on the regulation of immune responses in humans
First Dark Matter Results from the XENON100 Experiment
The XENON100 experiment, in operation at the Laboratori Nazionali del Gran
Sasso in Italy, is designed to search for dark matter WIMPs scattering off 62
kg of liquid xenon in an ultra-low background dual-phase time projection
chamber. In this letter, we present first dark matter results from the analysis
of 11.17 live days of non-blind data, acquired in October and November 2009. In
the selected fiducial target of 40 kg, and within the pre-defined signal
region, we observe no events and hence exclude spin-independent WIMP-nucleon
elastic scattering cross-sections above 3.4 x 10^-44 cm^2 for 55 GeV/c^2 WIMPs
at 90% confidence level. Below 20 GeV/c^2, this result constrains the
interpretation of the CoGeNT and DAMA signals as being due to spin-independent,
elastic, light mass WIMP interactions.Comment: 5 pages, 5 figures. Matches published versio
Quantitative uptake of colloidal particles by cell cultures
The use of nanotechnologies involving nano- and microparticles has increased tremendously in the recent past. There are various beneficial characteristics that make particles attractive for a wide range of technologies. However, colloidal particles on the other hand can potentially be harmful for humans and environment. Today, complete understanding of the interaction of colloidal particles with biological systems still remains a challenge. Indeed, their uptake, effects, and final cell cycle including their life span fate and degradation in biological systems are not fully understood. This is mainly due to the complexity of multiple parameters which need to be taken in consideration to perform the nanosafety research. Therefore, we will provide an overview of the common denominators and ideas to achieve universal metrics to assess their safety. The review discusses aspects including how biological media could change the physicochemical properties of colloids, how colloids are endocytosed by cells, how to distinguish between internalized versus membrane-attached colloids, possible correlation of cellular uptake of colloids with their physicochemical properties, and how the colloidal stability of colloids may vary upon cell internalization. In conclusion three main statements are given. First, in typically exposure scenarios only part of the colloids associated with cells are internalized while a significant part remain outside cells attached to their membrane. For quantitative uptake studies false positive counts in the form of only adherent but not internalized colloids have to be avoided. pH sensitive fluorophores attached to the colloids, which can discriminate between acidic endosomal/lysosomal and neutral extracellular environment around colloids offer a possible solution. Second, the metrics selected for uptake studies is of utmost importance. Counting the internalized colloids by number or by volume may lead to significantly different results. Third, colloids may change their physicochemical properties along their life cycle, and appropriate characterization is required during the different stages.This work was supported by the European Commission (grant FutureNanoNeeds) grant agreement no. 604602 to WJP. Search for Neutrinoless Double-Beta Decay of Te with CUORE-0
We report the results of a search for neutrinoless double-beta decay in a
9.8~kgyr exposure of Te using a bolometric detector array,
CUORE-0. The characteristic detector energy resolution and background level in
the region of interest are FWHM and ~counts/(keVkgyr), respectively. The
median 90%~C.L. lower-limit sensitivity of the experiment is and surpasses the sensitivity of previous searches. We find
no evidence for neutrinoless double-beta decay of Te and place a
Bayesian lower bound on the decay half-life, ~ at 90%~C.L. Combining CUORE-0 data with the 19.75~kgyr
exposure of Te from the Cuoricino experiment we obtain at 90%~C.L.~(Bayesian), the most stringent
limit to date on this half-life. Using a range of nuclear matrix element
estimates we interpret this as a limit on the effective Majorana neutrino mass,

Material screening and selection for XENON100
Results of the extensive radioactivity screening campaign to identify
materials for the construction of XENON100 are reported. This Dark Matter
search experiment is operated underground at Laboratori Nazionali del Gran
Sasso (LNGS), Italy. Several ultra sensitive High Purity Germanium detectors
(HPGe) have been used for gamma ray spectrometry. Mass spectrometry has been
applied for a few low mass plastic samples. Detailed tables with the
radioactive contaminations of all screened samples are presented, together with
the implications for XENON100.Comment: 8 pages, 1 figur
CUORE-0 results and prospects for the CUORE experiment
With 741 kg of TeO2 crystals and an excellent energy resolution of 5 keV
(0.2%) at the region of interest, the CUORE (Cryogenic Underground Observatory
for Rare Events) experiment aims at searching for neutrinoless double beta
decay of 130Te with unprecedented sensitivity. Expected to start data taking in
2015, CUORE is currently in an advanced construction phase at LNGS. CUORE
projected neutrinoless double beta decay half-life sensitivity is 1.6E26 y at 1
sigma (9.5E25 y at the 90% confidence level), in five years of live time,
corresponding to an upper limit on the effective Majorana mass in the range
40-100 meV (50-130 meV). Further background rejection with auxiliary bolometric
detectors could improve CUORE sensitivity and competitiveness of bolometric
detectors towards a full analysis of the inverted neutrino mass hierarchy.
CUORE-0 was built to test and demonstrate the performance of the upcoming CUORE
experiment. It consists of a single CUORE tower (52 TeO2 bolometers of 750 g
each, arranged in a 13 floor structure) constructed strictly following CUORE
recipes both for materials and assembly procedures. An experiment its own,
CUORE-0 is expected to reach a sensitivity to the neutrinoless double beta
decay half-life of 130Te around 3E24 y in one year of live time. We present an
update of the data, corresponding to an exposure of 18.1 kg y. An analysis of
the background indicates that the CUORE performance goal is satisfied while the
