    Panniculitis in the Setting of Visceral Leishmaniasis

    Um homem de 38 anos com história pregressa de toxicofilia endovenosa, Síndroma da Imunodeficiência Adquirida (SIDA) e Leishmaniose Visceral em 2000, foi internado por febre, mioartralgias, cansaço fácil, hepatoesplenomegália e edema dos membros inferiores. Ao décimo dia de internamento surgiram nódulos subcutâneos das coxas e foi solicitada avaliação pela Dermatologia. O diagnóstico clínico e histológico das lesões foi compatível com Paniculite a Leishmania no contexto de recidiva de Leishmaniose Visceral. A Paniculite a Leishmania é um achado raramente encontrado no contexto de Leishmaniose Visceral, sendo mais frequente nos indivíduos coinfectados pelo VIH. Neste caso, o atingimento cutâneo permitiu o diagnóstico precoce e a confirmação histopatológica de recidiva de Leishmaniose

This article reviews the information that can be obtained in cases of nail dyschromia and especially in cases of melanonychia, in which the distinction between benign melanocytic activation or proliferation and malignancy is crucial. Dermatoscopic changes that accompany specific nail diseases are also reviewed, such as those observed with subungual hemorrhage, bacterial and fungal nail infections, psoriasis of the nail, lichen planus of the nail, and vascular abnormalities of the nail fold

    A 70-year-old male rural worker was referred to our clinic with widespread grey pigmentation of the skin and nails. The condition had been asymptomatic for its entire duration (5 years). He reported past intranasal application of 10% Silver Vitellinate. A skin biopsy was performed and histology corroborated the clinical diagnosis of Argyria. This case represents a currently rare dermatological curiosity. Although silver colloids and salts have been withdrawn and/or banned by some drug surveillance agencies, they continue to be freely sold and unregulated as food supplements and as ingredients in alternative medicines, thereby risking the emergence of new cases of silver poisoning

    Leiomiomatose Cutânea em Mãe e Filha

    A 34-year-old woman with no known medical history was evaluated for multiple painful brown nodules and papules on the anterior aspect of the trunk. She mentioned a history of similar cutaneous findings on her mother. Biopsies of three lesions revealed piloleiomyomata. Renal and adrenal ultrasound revealed an isolated simple cortical cyst, and pelvic and endovaginal ultrasound revealed two uterine myomata. The clinical diagnosis of hereditary leiomyomatosis and renal cell cancer was corroborated by the identification of a heterozygous variant on exon 5 of the fumarate hydratase gene (c.578C>T p.T193I). Identification of the tumor piloleiomyoma should alert the dermatologist to this rare genodermatosis, which is associated with an increased risk of renal cell tumors, demanding multidisciplinary follow-up, and personal and family counseling

    Cutaneous Leukemic Infiltration Following Varicella - a Case of Wolf's Isotopic Response

    Wolf's isotopic response designates the appearance of two subsequent unrelated dermatoses in the same anatomic location. We report the case of a 51-year-old man with a medical history of chronic lymphocytic leukemia without known extra-hematopoietic involvement. The patient developed a disseminated papulo-vesiculous eruption, diagnosed as varicella. Few days after recovering, an erythematous and violaceous papular dermatosis with histopathological examination compatible with leukemic infiltration appeared on the scars of previous herpetic lesions. Complete remission was obtained under systemic corticotherapy, without cutaneous recurrence or blastic transformation. Wolf's isotopic response is attributed to a localized immunologic imbalance following a certain stimulus. In this patient, herpetic infection acted as a local spur for inaugural cutaneous leukemic infiltration, with no impact on the prognosis for the underlying disease.info:eu-repo/semantics/publishedVersio

    An 83 year-old man was observed for an 8 year-long pruritic, erythematous plaque of the left axilla. He had been continuously medicated with topical antifungals and steroids with occasional symptomatic relief, but with disease progression. After a clinical and histological diagnosis of Extramammary Paget's Disease, a total excision with wide margins was performed. He remains disease-free 12 months after surgery.     Neoplasia Blástica de Células Dendríticas Plasmocitóides

    Blastic plasmacytoid dendritic cell neoplasm is a rare and aggressive hematodermic neoplasia with frequent cutaneous involvement and leukemic dissemination. We report the case of a 76-year-old man with a 2 month history of violaceous nodules and a tumor with stony consistency, located on the head, and mandibular, cervical and supraclavicular lymphadenopathies. Multiple thoracic and abdominal adenopathies were identified on computerized tomography. Flow cytometry analysis of the skin, lymph node and bone marrow biopsies demonstrated the presence of plasmocytoid dendritic cell neoplastic precursor cells (CD4+, CD45+, CD56+ and CD123+ phenotype). After initial clinical and laboratorial complete remission with chemotherapy, the patient died due to relapse of the disease associated with the appearance of a cervical mass with medullary compromise

    Why Are our Patients Claiming? Statistics from a Dermatology Clinic

    Introdução: A satisfação do utente constitui um indicador frequentemente usado na aferição da qualidade em saúde. Deste modo, é reconhecida a importância do utente como agente activo na melhoria dos serviços de saúde. Em Portugal, têm sido dados passos na caracterização das preocupações e necessidades do utilizador do Sistema Nacional de Saúde, pela análise oficial de reclamações e litígios. Objectivo: Analisar as reclamações dos utentes da consulta externa do serviço de dermatologia de um hospital português. Material e Métodos: Foram analisadas retrospectivamente todas as reclamações efectuadas pelos utentes no Livro Amarelo ou no Gabinete do Utente no período que decorreu entre os anos de 2000 e 2010, inclusive. Resultados: Identificaram-se 106 reclamações nos onze anos em estudo, com uma taxa de reclamações média de 0,4‰. A propensão para reclamar acentuou-se no tempo, predominando o reclamante do sexo feminino (60,4%). As reclamações ‘Administrativas ou referentes ao Sistema’ foram mais frequentes do que aquelas alusivas aos ‘Profissionais de Saúde’ (58,5% vs 41,5%). Nas primeiras, destacam-se as reclamações da tipologia ‘Leis ou Normas’ seguidas das relacionadas com ‘Procedimentos Administrativos’. As reclamações dirigidas aos ‘Profissionais de Saúde’ referem-se apenas a médicos ou actos médicos. A tipologia mais prevalente neste subgrupo foi a ‘Expectativa Frustrada’, seguindo-se as exposições por questões ‘Relacionais ou Comportamentais’. Conclusões: A taxa de reclamações no serviço de dermatologia em estudo é baixa comparativamente aos dados nacionais para a actividade hospitalar, porém com evolução crescente e em provável relação com a maior exigência e (des)conhecimento dos direitos pelo utente. Os resultados sublinham a pertinência das recentes melhorias organizacionais e a importância da relação médico-doente. A educação para a saúde poderá reflectir-se numa melhor gestão de expectativas e recursos
