    A computational study of aortic insufficiency in patients supported with continuous flow left ventricular assist devices: Is it time for a paradigm shift in management?

    Background: De novo aortic insufficiency (AI) following continuous flow left ventricular assist device (CF-LVAD) implantation is a common complication. Traditional early management utilizes speed augmentation to overcome the regurgitant flow in an attempt to augment net forward flow, but this strategy increases the aortic transvalvular gradient which predisposes the patient to progressive aortic valve pathology and may have deleterious effects on aortic shear stress and right ventricular (RV) function. Materials and methods: We employed a closed-loop lumped-parameter mathematical model of the cardiovascular system including the four cardiac chambers with corresponding valves, pulmonary and systemic circulations, and the LVAD. The model is used to generate boundary conditions which are prescribed in blood flow simulations performed in a three-dimensional (3D) model of the ascending aorta, aortic arch, and thoracic descending aorta. Using the models, impact of various patient management strategies, including speed augmentation and pharmacological treatment on systemic and pulmonary (PA) vasculature, were investigated for four typical phenotypes of LVAD patients with varying degrees of RV to PA coupling and AI severity. Results: The introduction of mild/moderate or severe AI to the coupled RV and pulmonary artery at a speed of 5,500 RPM led to a reduction in net flow from 5.4 L/min (no AI) to 4.5 L/min (mild/moderate) to 2.1 L/min (severe). RV coupling ratio (Ees/Ea) decreased from 1.01 (no AI) to 0.96 (mild/moderate) to 0.76 (severe). Increasing LVAD speed to 6,400 RPM in the severe AI and coupled scenario, led to a 42% increase in net flow and a 16% increase in regurgitant flow (RF) with a nominal decrease of 1.6% in RV myocardial oxygen consumption (MVO2). Blood pressure control with the coupled RV with severe AI at 5,500 RPM led to an 81% increase in net flow with a 15% reduction of RF and an 8% reduction in RV MVO2. With an uncoupled RV, the introduction of mild/moderate or severe AI at a speed of 5,500 RPM led to a reduction in net flow from 5.0 L/min (no AI) to 4.0 L/min (mild/moderate) to 1.8 L/min (severe). Increasing the speed to 6,400 RPM with severe AI and an uncoupled RV increased net flow by 45%, RF by 15% and reduced RV MVO2 by 1.1%. For the uncoupled RV with severe AI, blood pressure control alone led to a 22% increase in net flow, 4.2% reduction in RF, and 3.9% reduction in RV MVO2; pulmonary vasodilation alone led to a 18% increase in net flow, 7% reduction in RF, and 26% reduction in RV MVO2; whereas, combined BP control and pulmonary vasodilation led to a 113% increase in net flow, 20% reduction in RF and 31% reduction in RV MVO2. Compared to speed augmentation, blood pressure control consistently resulted in a reduction in WSS throughout the proximal regions of the arterial system. Conclusion: Speed augmentation to overcome AI in patients supported by CF-LVAD appears to augment flow but also increases RF and WSS in the aorta, and reduces RV MVO2. Aggressive blood pressure control and pulmonary vasodilation, particularly in those patients with an uncoupled RV can improve net flow with more advantageous effects on the RV and AI RF

    Acute Kidney Injury in the Context of Inflammatory Bowel Disease - A Clinical Case

    Extraintestinal manifestations of inflammatory bowel disease are common and extendable to all organs. Kidney and lower genitourinary system occurs in 4-23% of cases. This may be dependent on inflammatory bowel disease activity, secondary to metabolic disorders, drugs or others. We present a case of a 68-year-old man with ulcerative colitis for 22 years admitted in our department for acute nephritic syndrome. Urinary microscopy suggested glomerular injury. A kidney biopsy was performed and was compatible with acute interstitial nephritis and IgA nephropathy. Toxicity of mesalazine and glomerulonephritis secondary to ulcerative colitis were assumed. The patient suspended mesalazine and started prednisolone with clinical improvement. Our purpose is to sensitize the importance of having a prompt and thorough evaluation of acute kidney injury in patients with inflammatory bowel disease.     Effects of alternative bedding substrates on lamb welfare, productive performance, and meat quality during the finishing phase of fattening

    The study analyzed the effect of using sawdust, cellulose, straw, and rice husk bedding on the welfare, performance and meat quality of lambs during the finishing phase of fattening. Ninety-six lambs (fed ad libitum with concentrate) were assigned to pens containing 1 of 4 bedding materials in groups of 12 for 14 days. Live weight and feed consumption were recorded to estimate daily gain and conversion index, and carcass and meat quality variables were measured. Stereotypies, exploratory interactions with bedding material, and agonistic and affiliative interactions between animals were recorded by continuous sampling techniques using video recording. The substrates affected behavior and stress response variables. There were more stereotypies and agonistic interactions at the beginning of the experiment in all treatments (P ≤ 0.05). The lambs fattened on cellulose bedding interacted more with the substrate throughout the finishing stage (P ≤ 0.05), where there were also more affiliative interactions (P ≤ 0.05). Stress levels were lower in lambs on straw, followed by cellulose. No significant differences in production or carcass and meat quality traits were observed between treatments. The results suggest that all substrates evaluated could be efficiently incorporated into the system as bedding material without affecting productivity or product quality

    Sustained safety and performance of the second-generation drug-eluting absorbable metal scaffold in patients with de novo coronary lesions: 12-month clinical results and angiographic findings of the BIOSOLVE-II first-in-man trial.

    Metal absorbable scaffolds constitute a conceptually attractive alternative to polymeric scaffolds. Promising 6-month outcomes of a second-generation drug-eluting absorbable metal scaffold (DREAMS 2G), consisting of an absorbable magnesium scaffold backbone, have been reported. We assessed the 12-month safety and performance of this novel device. The prospective, international, multi-centre, first-in-man BIOSOLVE-II trial enrolled 123 patients with up to two de novo lesions with a reference diameter between 2.2 and 3.7 mm. All patients were scheduled for angiographic follow-up at 6 months, and-if subjects consented-at 12 months. Dual antiplatelet therapy was recommended for 6 months. Quantitative coronary angiography (QCA) parameters remained stable from 6 to 12 months [paired data of 42 patients: in-segment late lumen loss 0.20 ± 0.21 mm vs. 0.25 ± 0.22 mm, P = 0.117, Δ 0.05 ± 0.21 mm (95% CI: -0.01;0.12); in-scaffold late lumen loss 0.37 ± 0.25 mm vs. 0.39 ± 0.27 mm, P = 0.446, Δ 0.03 ± 0.22 (95% CI: -0.04;0.10), respectively]. Intravascular ultrasound and optical coherence tomography findings corroborated the QCA results. Target lesion failure occurred in four patients (3.4%), consisting of one death of unknown cause, one target-vessel myocardial infarction, and two clinically driven target lesion revascularization. No additional event occurred beyond the 6-month follow-up. During the entire follow-up of 12 months, none of the patients experienced a definite or probable scaffold thrombosis. The novel drug-eluting metal absorbable scaffold DREAMS 2G showed a continuous favourable safety profile up to 12 months and stable angiographic parameters between 6 and 12 months. NCT01960504

    Nonlinear electrodynamics and CMB polarization

    Recently WMAP and BOOMERanG experiments have set stringent constraints on the polarization angle of photons propagating in an expanding universe: Δα=(2.4±1.9)\Delta \alpha = (-2.4 \pm 1.9)^\circ. The polarization of the Cosmic Microwave Background radiation (CMB) is reviewed in the context of nonlinear electrodynamics (NLED). We compute the polarization angle of photons propagating in a cosmological background with planar symmetry. For this purpose, we use the Pagels-Tomboulis (PT) Lagrangian density describing NLED, which has the form L(X/Λ4)δ1  XL\sim (X/\Lambda^4)^{\delta - 1}\; X , where X=1/4FαβFαβX=1/4 F_{\alpha\beta} F^{\alpha \beta}, and δ\delta the parameter featuring the non-Maxwellian character of the PT nonlinear description of the electromagnetic interaction. After looking at the polarization components in the plane orthogonal to the (xx)-direction of propagation of the CMB photons, the polarization angle is defined in terms of the eccentricity of the universe, a geometrical property whose evolution on cosmic time (from the last scattering surface to the present) is constrained by the strength of magnetic fields over extragalactic distances.Comment: 17 pages, 2 figures, minor changes, references adde

    Rotating Black Branes in the presence of nonlinear electromagnetic field

    In this paper, we consider a class of gravity whose action represents itself as a sum of the usual Einstein-Hilbert action with cosmological constant and an U(1)U(1) gauge field for which the action is given by a power of the Maxwell invariant. We present a class of the rotating black branes with Ricci flat horizon and show that the presented solutions may be interpreted as black brane solutions with two event horizons, extreme black hole and naked singularity provided the parameters of the solutions are chosen suitably. We investigate the properties of the solutions and find that for the special values of the nonlinear parameter, the solutions are not asymptotically anti-deSitter. At last, we obtain the conserved quantities of the rotating black branes and find that the nonlinear source effects on the electric field, the behavior of spacetime, type of singularity and other quantities.Comment: 7 pages, 5 figures, to appear in EPJ

    Transmural Remission Improves Clinical Outcomes Up to 5 years in Crohn's Disease

    Introduction: Evidence supporting transmural remission (TR) as a long-term treatment target in Crohn's disease (CD) is still unavailable. Less stringent but more reachable targets such as isolated endoscopic (IER) or radiologic remission (IRR) may also be acceptable options in the long-term. Methods: Multicenter retrospective study including 404 CD patients evaluated by magnetic resonance enterography and colonoscopy. Five-year rates of hospitalization, surgery, use of steroids, and treatment escalation were compared between patients with TR, IER, IRR, and no remission (NR). Results: 20.8% of CD patients presented TR, 23.3% IER, 13.6% IRR and 42.3% NR. TR was associated with lower risk of hospitalization (odds-ratio [OR] 0.244 [0.111-0.538], p < 0.001), surgery (OR 0.132 [0.030-0.585], p = 0.008), steroid use (OR 0.283 [0.159-0.505], p < 0.001), and treatment escalation (OR 0.088 [0.044-0.176], p < 0.001) compared to no NR. IRR resulted in lower risk of hospitalization (OR 0.333 [0.143-0.777], p = 0.011) and treatment escalation (OR 0.260 [0.125-0.540], p < 0.001), while IER reduced the risk of steroid use (OR 0.442 [0.262-0.745], p = 0.002) and treatment escalation (OR 0.490 [0.259-0.925], p = 0.028) compared to NR. Conclusions: TR improved clinical outcomes over 5 years of follow-up in CD patients. Distinct but significant benefits were seen with IER and IRR.     Observation of the Bs0J/ψϕϕB_s^0 \rightarrow J/\psi \phi \phi decay

    The Bs0J/ψϕϕB_s^0 \rightarrow J/\psi \phi \phi decay is observed in pppp collision data corresponding to an integrated luminosity of 3 fb1^{-1} recorded by the LHCb detector at centre-of-mass energies of 7 TeV and 8 TeV. This is the first observation of this decay channel, with a statistical significance of 15 standard deviations. The mass of the Bs0B_s^0 meson is measured to be 5367.08±0.38±0.155367.08\,\pm \,0.38\,\pm\, 0.15 MeV/c2^2. The branching fraction ratio B(Bs0J/ψϕϕ)/B(Bs0J/ψϕ)\mathcal{B}(B_s^0 \rightarrow J/\psi \phi \phi)/\mathcal{B}(B_s^0 \rightarrow J/\psi \phi) is measured to be 0.0115\,\pm\, 0.0012\, ^{+0.0005}_{-0.0009}. In both cases, the first uncertainty is statistical and the second is systematic. No evidence for non-resonant Bs0J/ψϕK+KB_s^0 \rightarrow J/\psi \phi K^+ K^- or Bs0J/ψK+KK+KB_s^0 \rightarrow J/\psi K^+ K^- K^+ K^- decays is found.Comment: All figures and tables, along with any supplementary material and additional information, are available at https://lhcbproject.web.cern.ch/lhcbproject/Publications/LHCbProjectPublic/LHCb-PAPER-2015-033.htm
