No commercial use is permitted without permission.Soil salinity is one of the critical factors responsible for the ongoing land degradation in the irrigated lowlands of Central Asia, including in the lower reaches of the Amudarya Delta. This land degradation hinders sustainable development and presents a major challenge for the area's rural population, whose livelihood security depends on irrigated agriculture. The factors causing soil salinity are multifaceted and interlinked; recent studies and interventions confirm that no one action alone will deliver a sustainable solution. Recommendations for alleviating soil salinity should take into account the complex interactions and can be formulated only once the interlinked factors causing soil salinity are understood. In the past, little attention was paid to creeping land degradation, which has resulted from soil salinization and waterlogging across huge agricultural and even nonagricultural areas. This case study focuses on the vicious circle of soil salinization: agriculture's consumption of large amounts of water contributes to shallow groundwater, leading to recurring soil salinity, which in turn demands more water for leaching (flushing the salts out of the rooting zone). The situation is exacerbated when water is not available in sufficient amounts in time and in space. The seemingly stable present water flows in the major water source (the Amudarya River) since the major drought in 2000–01 is caused by increased glacier melting in upstream countries. This water supply in turn diverts attention from the strong need for improved irrigation and cropping practices. Efforts aimed at reducing the amounts of irrigation water use face the problem of the devilish vicious circle, which has not only technical but also financial and political dimensions.     Mapping Cropland Abandonment in the Aral Sea Basin with MODIS Time Series

    Cropland abandonment is globally widespread and has strong repercussions for regional food security and the environment. Statistics suggest that one of the hotspots of abandoned cropland is located in the drylands of the Aral Sea Basin (ASB), which covers parts of post-Soviet Central Asia, Afghanistan and Iran. To date, the exact spatial and temporal extents of abandoned cropland remain unclear, which hampers land-use planning. Abandoned land is a potentially valuable resource for alternative land uses. Here, we mapped the abandoned cropland in the drylands of the ASB with a time series of the Normalized Difference Vegetation Index (NDVI) from the Moderate Resolution Imaging Spectroradiometer (MODIS) from 2003–2016. To overcome the restricted ability of a single classifier to accurately map land-use classes across large areas and agro-environmental gradients, “stratum-specific” classifiers were calibrated and classification results were fused based on a locally weighted decision fusion approach. Next, the agro-ecological suitability of abandoned cropland areas was evaluated. The stratum-specific classification approach yielded an overall accuracy of 0.879, which was significantly more accurate ( p < 0.05) than a “global” classification without stratification, which had an accuracy of 0.811. In 2016, the classification results showed that 13% (1.15 Mha) of the observed irrigated cropland in the ASB was idle (abandoned). Cropland abandonment occurred mostly in the Amudarya and Syrdarya downstream regions and was associated with degraded land and areas prone to water stress. Despite the almost twofold population growth and increasing food demand in the ASB area from 1990 to 2016, abandoned cropland was also located in areas with high suitability for farming. The map of abandoned cropland areas provides a novel basis for assessing the causes leading to abandoned cropland in the ASB. This contributes to assessing the suitability of abandoned cropland for food or bioenergy production, carbon storage, or assessing the environmental trade-offs and social constraints of recultivation

    The use of indigenous knowledge has been seen by many as an alternative way of promoting development in poor rural communities in many parts of the world. By reviewing much of the recent work on indigenous knowledge, the paper suggests that a number of problems and tensions has resulted in indigenous knowledge not being as useful as hoped for or supposed. These include problems emanating from a focus on the (arte)factual; binary tensions between western science and indigenous knowledge systems; the problem of differentiation and power relations; the romanticization of indigenous knowledge; and the all too frequent decontextualization of indigenous knowledge

    Indigenous knowledges and development: a postcolonial caution

    As a result of the failure of formal top-down development, there has recently been increased interest in the possibilities of drawing upon the indigenous knowledges of those in the communities involved, in an attempt to produce more effective development strategies. The concept of indigenous knowledge calls for the inclusion of local voices and priorities, and promises empowerment through ownership of the process. However, there has been little critical examination of the ways in which indigenous knowledges have been included in the development process. Drawing upon postcolonial theory, this article suggests that indigenous knowledges are often drawn into development by both theorists and development institutions in a very limited way, failing to engage with other ways of perceiving development, and thus missing the possibility of devising more challenging alternatives

    A mass-loss rate determination for zeta Puppis from the quantitative analysis of X-ray emission line profiles

    We fit every emission line in the high-resolution Chandra grating spectrum of zeta Pup with an empirical line profile model that accounts for the effects of Doppler broadening and attenuation by the bulk wind. For each of sixteen lines or line complexes that can be reliably measured, we determine a best-fitting fiducial optical depth, tau_* = kappa*Mdot/4{pi}R_{\ast}v_{\infty}, and place confidence limits on this parameter. These sixteen lines include seven that have not previously been reported on in the literature. The extended wavelength range of these lines allows us to infer, for the first time, a clear increase in tau_* with line wavelength, as expected from the wavelength increase of bound-free absorption opacity. The small overall values of tau_*, reflected in the rather modest asymmetry in the line profiles, can moreover all be fit simultaneously by simply assuming a moderate mass-loss rate of 3.5 \pm 0.3 \times 10^{-6} Msun/yr, without any need to invoke porosity effects in the wind. The quoted uncertainty is statistical, but the largest source of uncertainty in the derived mass-loss rate is due to the uncertainty in the elemental abundances of zeta Pup, which affects the continuum opacity of the wind, and which we estimate to be a factor of two. Even so, the mass-loss rate we find is significantly below the most recent smooth-wind H-alpha mass-loss rate determinations for zeta Pup, but is in line with newer determinations that account for small-scale wind clumping.     Canadian Valuation of EQ-5D Health States: Preliminary Value Set and Considerations for Future Valuation Studies

    Background The EQ-5D is a preference based instrument which provides a description of a respondent's health status, and an empirically derived value for that health state often from a representative sample of the general population. It is commonly used to derive Quality Adjusted Life Year calculations (QALY) in economic evaluations. However, values for health states have been found to differ between countries. The objective of this study was to develop a set of values for the EQ-5D health states for use in Canada. Methods Values for 48 different EQ-5D health states were elicited using the Time Trade Off (TTO) via a web survey in English. A random effect model was fitted to the data to estimate values for all 243 health states of the EQ-5D. Various model specifications were explored. Comparisons with EQ-5D values from the UK and US were made. Sensitivity analysis explored different transformations of values worse than dead, and exclusion criteria of subjects. Results The final model was estimated from the values of 1145 subjects with socio-demographics broadly representative of Canadian general population with the exception of Quebec. This yielded a good fit with observed TTO values, with an overall R2 of 0.403 and a mean absolute error of 0.044. Conclusion A preference-weight algorithm for Canadian studies that include the EQ-5D is developed. The primary limitations regarded the representativeness of the final sample, given the language used (English only), the method of recruitment, and the difficulty in the task. Insights into potential issues for conducting valuation studies in countries as large and diverse as Canada are gained

    Long term costs and effects of reducing the number of twin pregnancies in IVF by single embryo transfer: the TwinSing study

    Contains fulltext : 87274.pdf (publisher's version ) (Open Access)BACKGROUND: Pregnancies induced by in vitro fertilisation (IVF) often result in twin gestations, which are associated with both maternal and perinatal complications. An effective way to reduce the number of IVF twin pregnancies is to decrease the number of embryos transferred from two to one. The interpretation of current studies is limited because they used live birth as outcome measure and because they applied limited time horizons. So far, research on long-term outcomes of IVF twins and singletons is scarce and inconclusive. The objective of this study is to investigate the short (1-year) and long-term (5 and 18-year) costs and health outcomes of IVF singleton and twin children and to consider these in estimating the cost-effectiveness of single embryo transfer compared with double embryo transfer, from a societal and a healthcare perspective. METHODS/DESIGN: A multi-centre cohort study will be performed, in which IVF singletons and IVF twin children born between 2003 and 2005 of whom parents received IVF treatment in one of the five participating Dutch IVF centres, will be compared. Data collection will focus on children at risk of health problems and children in whom health problems actually occurred. First year of life data will be collected in approximately 1,278 children (619 singletons and 659 twin children). Data up to the fifth year of life will be collected in approximately 488 children (200 singletons and 288 twin children). Outcome measures are health status, health-related quality of life and costs. Data will be obtained from hospital information systems, a parent questionnaire and existing registries. Furthermore, a prognostic model will be developed that reflects the short and long-term costs and health outcomes of IVF singleton and twin children. This model will be linked to a Markov model of the short-term cost-effectiveness of single embryo transfer strategies versus double embryo transfer strategies to enable the calculation of the long-term cost-effectiveness. DISCUSSION: This is, to our knowledge, the first study that investigates the long-term costs and health outcomes of IVF singleton and twin children and the long-term cost-effectiveness of single embryo transfer strategies versus double embryo transfer strategies

    A study of the diagnostic accuracy of the PHQ-9 in primary care elderly

    <p>Abstract</p> <p>Background</p> <p>The diagnostic accuracy of the Patient Health Questionnaire-9 (PHQ-9) for assessment of depression in elderly persons in primary care settings in the United States has not been previously addressed. Thus, the purpose of this study was to evaluate the test performance of the PHQ-9 for detecting major and minor depression in elderly patients in primary care.</p> <p>Methods</p> <p>A prospective study of diagnostic accuracy was conducted in two primary care, university-based clinics in the Pacific Northwest of the United States. Seventy-one patients aged 65 years or older participated; all completed the PHQ-9 and the 15-item Geriatric Depression Scale (GDS) and underwent the Structured Clinical Interview for Depression (SCID). Sensitivity, specificity, area under the receiver operating characteristic (ROC) curve, and likelihood ratios (LRs) were calculated for the PHQ-9, the PHQ-2, and the 15-item GDS for major depression alone and the combination of major plus minor depression.</p> <p>Results</p> <p>Two thirds of participants were female, with a mean age of 78 and two chronic health conditions. Twelve percent met SCID criteria for major depression and 13% minor depression. The PHQ-9 had an area under the curve (AUC) of 0.87 (95% confidence interval [CI], 0.74-1.00) for major depression, while the PHQ-2 and the 15-item GDS each had an AUC of 0.81 (95% CI for PHQ-2, 0.64-0.98, and for 15-item GDS, 0.70-0.91; <it>P </it>= 0.551). For major and minor depression combined, the AUC for the PHQ-9 was 0.85 (95% CI, 0.73-0.96), for the PHQ-2, 0.80 (95% CI, 0.68-0.93), and for the 15-item GDS, 0.71 (95% CI, 0.55-0.87; <it>P </it>= 0.187).</p> <p>Conclusions</p> <p>Based on AUC values, the PHQ-9 performs comparably to the PHQ-2 and the 15-item GDS in identifying depression among primary care elderly.</p
