    Does Reiki Therapy Decrease Pain During Chemotherapy and Radiation Treatment in Patients with Gynecological and Breast Cancer?

    OBJECTIVE: The objective of this selective EBM review is to determine whether or not Reiki therapy decreases pain during chemotherapy and radiation treatment in patients with gynecological and breast cancer. STUDY DESIGN: Review of two randomized control trials and one randomized prospective cross-over study published in 2003, 2004, and 2011. DATA SOURCES: One double blind randomized control trial, one 2-arm single blind randomized control trial and one randomized, prospective, 2-period crossover intervention study comparing Reiki therapy to placebo. Articles were found using PubMed and Medline. OUTCOMES MEASURED: Pain by BPI (brief pain inventory linear analog scale), SF-36 questionnaire, daily log of medication use, HTCQ linear scale, and Well-being analog scale. RESULTS: The study by Caitlin et al showed a slight increase in physical comfort in patients who received Reiki therapy verses those who received standard care during chemotherapy. However, sham Reiki therapy showed a statistically significant increase in physical comfort from an attentive presence of a designated nurse at the bedside. According to Cook et al, Reiki therapy showed a statistically significant decrease in a patient’s pain level verses a patient receiving standard care for radiation therapy. In addition, Reiki and Massage therapy was more effective at reducing pain in one 45-minute intervention (short-term relief) than a patient who received standard care shown in a study conducted by Post-white et al. CONCLUSIONS: Results from the three studies showed a decrease in pain when Reiki therapy is administered during radiation and chemotherapy. However, one study concluded there is no statistical difference in pain management when comparing Reiki therapy and sham Reiki. Another study discovered both Reiki and Massage therapy decreased pain when compared to standard care. In conclusion, there is no convincing evidence that Reiki therapy alone relieves pain, however, human touch with or without energy transfer showed positive benefits. Thus, further studies with adequate sample sizes, non-biased participants, and a greater variety of subjects from different treatment centers should be considered to determine if this alternative approach reduces pain

    Mulch-based no-tillage effects on weed community and management in an organic vegetable system

    Weeds can cooperate with the agroecosystem\u2019s functioning by providing ecosystem services. Effective weed management should mitigate negative weed\u2013crop interference, while maintaining a functional and balanced weed community. In a two-year trial, the in-line/roller crimper (RC) was used to terminate an agroecological service crop (ASC; here barley, Hordeum vulgare L.) before organic zucchini (Cucurbita pepo, L.) and compared with green manure (GM) ASC and tilled no-ASC with Mater-Bi mulch on the rows (No_ASC). Zucchini yield, soil N availability, weed density/cover, biomass, and community composition were assessed. Analysis of variance, exploratory statistical analysis, and non-parametric inferential approaches were run, respectively, on agronomic data, species-specific weed frequencies, and Shannon diversity. Zucchini yield was the highest in No_ASC, due to soil N immobilization under high C:N barley residues in GM and RC. Multivariate analysis discriminated RC from tilled systems, outlining a specific ensemble of weed species correlated to Shannon diversity. From zucchini fruit set, RC selectively favored Polygonum aviculare L. and Helminthotheca echioides (L.), reasonably because of their oligotrophy and creeping habit. Their dominance finally caused low RC weed control. Results highlight strong weed selective pressure by the mulch-based no-tillage.     7noopenThe integration of Agroecological Service Crops (ASCs) into agroecosystems can provide several ecological services, such as nutrient cycling and disease and weed management. A two-year experiment on an organic orchard was carried out to compare barley (B) and horse bean (HB) ASCs with a control without ASC (Cont) in combination with fertilizers. Their effects on soil fertility and weed- and soil-borne fungi communities were evaluated by direct measurements, visual estimation, and indicators computation. A Principal Component Analysis (PCA) was used to identify latent patterns and redundancy among variables, whereas a correlation analysis was used to discriminate the compared systems within the PCA matrix. The empirical results of this study put in evidence the correlation among soil, weed, and fungal variables. A slight contribution of fertilizers on the system’s variability was observed, whereas a clear effect of ASCs was highlighted. The systems differed in weed communities, with the lowest density associated to B and the highest to Cont. B showed the highest fungal diversity, with changes in community compared to HB. HB showed a contribution on soil fertility, being associated to organic matter increase and N availability, and evidencing mixed impacts on soil quality and ecosystem functioning.     The Sphingosine Kinase-Sphingosine-1-Phosphate Axis Is a Determinant of Mast Cell Function and Anaphylaxis

    SummarySphingosine-1-phosphate, a key mediator in immune cell trafficking, is elevated in the lungs of asthmatic patients and regulates pulmonary epithelium permeability. Stimulation of mast cells by allergens induces two mammalian sphingosine kinases (Sphk1 and Sphk2) to produce sphingosine-1-phosphate (S1P). Little is known about the individual role of these kinases in regulating immune cell function. Here we show that in mast cells, Sphk2 is required for production of S1P, for calcium influx, for activation of protein kinase C, and for cytokine production and degranulation. However, susceptibility to in vivo anaphylaxis is determined both by S1P within the mast cell compartment and by circulating S1P generated by Sphk1 predominantly from a non-mast cell source(s). Thus, sphingosine kinases are determinants of mast cell responsiveness, demonstrating a previously unrecognized relationship with anaphylaxis

    Lung segmentation and characterization in covid-19 patients for assessing pulmonary thromboembolism: An approach based on deep learning and radiomics

    The COVID-19 pandemic is inevitably changing the world in a dramatic way, and the role of computed tomography (CT) scans can be pivotal for the prognosis of COVID-19 patients. Since the start of the pandemic, great care has been given to the relationship between interstitial pneumonia caused by the infection and the onset of thromboembolic phenomena. In this preliminary study, we collected n = 20 CT scans from the Polyclinic of Bari, all from patients positive with COVID-19, nine of which developed pulmonary thromboembolism (PTE). For eight CT scans, we obtained masks of the lesions caused by the infection, annotated by expert radiologists; whereas for the other four CT scans, we obtained masks of the lungs (including both healthy parenchyma and lesions). We developed a deep learning-based segmentation model that utilizes convolutional neural networks (CNNs) in order to accurately segment the lung and lesions. By considering the images from publicly available datasets, we also realized a training set composed of 32 CT scans and a validation set of 10 CT scans. The results obtained from the segmentation task are promising, allowing to reach a Dice coefficient higher than 97%, posing the basis for analysis concerning the assessment of PTE onset. We characterized the segmented region in order to individuate radiomic features that can be useful for the prognosis of PTE. Out of 919 extracted radiomic features, we found that 109 present different distributions according to the Mann–Whitney U test with corrected p-values less than 0.01.     Photon-mediated long range coupling of two Andreev level qubits

    In a superconducting weak link, the supercurrent is carried by Andreev bound states (ABSs) formed by the phase-coherent reflection of electrons and their time-reversed partners. A single, highly transmissive ABS can serve as an ideal, compact two-level system, due to a potentially large energy difference to the next ABS. While the coherent manipulation of such Andreev levels qubits (ALQs) has been demonstrated, a long-range coupling between two ALQs, necessary for advanced qubit architectures, has not been achieved, yet. Here, we demonstrate a coherent remote coupling between two ALQs, mediated by a microwave photon in a novel superconducting microwave cavity coupler. The latter hosts two modes with different coupling rates to an external port. This allows us to perform fast readout of each qubit using the strongly coupled mode, while the weakly coupled mode is utilized to mediate the coupling between the qubits. When both qubits are tuned into resonance with the latter mode, we find excitation spectra with avoided-crossings, in very good agreement with the Tavis-Cummings model. Based on this model, we identify highly entangled two-qubit states for which the entanglement is mediated over a distance of six millimeters.     Massive transcriptome sequencing of human spinal cord tissues provides new insights into motor neuron degeneration in als

    ALS is a devastating and debilitating human disease characterized by the progressive death of upper and lower motor neurons. Although much effort has been made to elucidate molecular determinants underlying the onset and progression of the disorder, the causes of ALS remain largely unknown. In the present work, we have deeply sequenced whole transcriptome from spinal cord ventral horns of post-mortem ALS human donors affected by the sporadic form of the disease (which comprises ∼90% of the cases but which is less investigated than the inherited form of the disease). We observe 1160 deregulated genes including 18 miRNAs and show that down regulated genes are mainly of neuronal derivation while up regulated genes have glial origin and tend to be involved in neuroinflammation or cell death. Remarkably, we find strong deregulation of SNAP25 and STX1B at both mRNA and protein levels suggesting impaired synaptic function through SNAP25 reduction as a possible cause of calcium elevation and glutamate excitotoxicity. We also note aberrant alternative splicing but not disrupted RNA editing

    Profile, Healthcare Resource Consumption and Related Costs in ANCA-Associated Vasculitis Patients: A Real-World Analysis in Italy

    Introduction: Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are rare autoimmune diseases triggering inflammation of small vessels. This real-world analysis was focused on the most common AAV forms, granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA), to describe patients’ demographic and clinical characteristics, therapeutic management, disease progression, and the related economic burden. Methods: A retrospective analysis was conducted on administrative databases of a representative sample of Italian healthcare entities, covering approximately 12 million residents. Between January 2010 and December 2020, adult GPA patients were identified by payment waiver code or hospitalization discharge diagnosis, and MPA patients by payment waiver code with or without hospitalization discharge diagnosis. Clinical outcomes were evaluated through AAV-related hospitalizations, renal failure onset, and mortality. Economic analysis included healthcare resource utilization deriving from drugs, hospitalizations, and outpatient specialist services. The related mean direct costs year/patient were also calculated in patients stratified by presence/absence of glucocorticoid therapy and type of inclusion criterion (hospitalization/payment waiver code). Results: Overall, 859 AAV patients were divided into GPA (n = 713; 83%) and MPA (n = 146; 17%) cohorts. Outcome indicators highlighted a clinically worse phenotype associated with GPA compared to MPA. Cost analysis during follow-up showed tendentially increased expenditures in glucocorticoid-treated patients versus untreated (overall AAV: €8728 vs. €7911; GPA: €9292 vs. €9143; MPA: €5967 vs. €2390), mainly driven by drugs (AAV: €2404 vs. €874; GPA: €2510 vs. €878; MPA: €1881 vs. €854) and hospitalizations. Conclusion: Among AAV forms, GPA resulted in a worse clinical picture, higher mortality, and increased costs. This is the first real-world pharmaco-economic analysis on AAV patients stratified by glucocorticoid use on disease management expenditures. In both GPA and MPA patients, glucocorticoid treatment resulted in higher healthcare costs, mostly attributable to medications, and then hospitalizations, confirming the clinical complexity and economic burden for management of patients with autoimmune diseases under chronic immunosuppression
