    Broken discs: warp propagation in accretion discs

    We simulate the viscous evolution of an accretion disc around a spinning black hole. In general any such disc is misaligned, and warped by the Lense-Thirring effect. Unlike previous studies we use effective viscosities constrained to be consistent with the internal fluid dynamics of the disc. We find that nonlinear fluid effects, which reduce the effective viscosities in warped regions, can promote the breaking of the disc into two distinct planes. This occurs when the Shakura & Sunyaev dimensionless viscosity parameter alpha is ~ 45 degrees. The break can be a long-lived feature, propagating outwards in the disc on the usual alignment timescale, after which the disc is fully co- or counter-aligned with the hole. Such a break in the disc may be significant in systems where we know the inclination of the outer accretion disc to the line of sight, such as some X-ray binaries: the inner disc, and so any jets, may be noticeably misaligned with respect to the orbital plane.Comment: 8 pages, 9 figures.     Almost sure one-endedness of a random graph model of distributed ledgers

    Blockchain and other decentralized databases, known as distributed ledgers, are designed to store information online where all trusted network members can update the data with transparency. The dynamics of ledger's development can be mathematically represented by a directed acyclic graph (DAG). One essential property of a properly functioning shared ledger is that all network members holding a copy of the ledger agree on a sequence of information added to the ledger, which is referred to as consensus and is known to be related to a structural property of DAG called one-endedness. In this paper, we consider a model of distributed ledger with sequential stochastic arrivals that mimic attachment rules from the IOTA cryptocurrency. We first prove that the number of leaves in the random DAG is bounded by a constant infinitely often through the identification of a suitable martingale, and then prove that a sequence of specific events happens infinitely often. Combining those results we establish that, as time goes to infinity, the IOTA DAG is almost surely one-ended.Comment: 28 pages, updated information for author

    In this article we show that the Hurst parameter and a well-known wavelet estimator fail to distinguish on/o models with substantially dierent distributions of on-times. We identify the problem as arising when standard power spectrum techniques, which are well-dened for commonly considered processes, are applied to processes with extremely heavy tailed on-times. We provide an elementary mechanism whereby such extremely heavy-tailed on-time processes arise in a simple queuing model

    An Efficient, Green Chemical Synthesis of the Malaria\ud Drug, Piperaquine

    To provide a robust, efficient synthesis of the malaria drug piperaquine for potential use in resource-poor settings. We used in-process analytical technologies (IPAT; HPLC) and a program of experiments to develop a synthesis of piperaquine that avoids the presence of a toxic impurity in the API and is optimized for overall yield and operational simplicity. A green-chemical synthesis of piperaquine is described that proceeds in 92 – 93 % overall yield. The chemistry is robust and provides very pure piperaquine tetraphosphate salt (> 99.5 %). The overall process utilizes modest amounts (about 8 kg/kg) of 2-propanol and ethyl acetate as the only organic materials not incorporated into the API; roughly 60 % of this waste can be recycled into the production process. This process also completely avoids the formation of a toxic impurity commonly seen in piperaquine that is otherwise difficult to remove. An efficient synthesis of piperaquine is described that may be useful for application in resource-poor settings as a means of expanding access to and reducing the cost of ACTs

    Electronic health record-embedded individualized pain plans for emergency department treatment of vaso-occlusive episodes in adults with sickle cell disease: Protocol for a preimplementation and postimplementation study

    BACKGROUND: Individuals living with sickle cell disease often require aggressive treatment of pain associated with vaso-occlusive episodes in the emergency department. Frequently, pain relief is poor. The 2014 National Heart, Lung, and Blood Institute evidence-based guidelines recommended an individualized treatment and monitoring protocol to improve pain management of vaso-occlusive episodes. OBJECTIVE: This study will implement an electronic health record-embedded individualized pain plan with provider and patient access in the emergency departments of 8 US academic centers to improve pain treatment for adult patients with sickle cell disease. This study will assess the overall effects of electronic health record-embedded individualized pain plans on improving patient and provider outcomes associated with pain treatment in the emergency department setting and explore barriers and facilitators to the implementation process. METHODS: A preimplementation and postimplementation study is being conducted by all 8 sites that are members of the National Heart, Lung, and Blood Institute-funded Sickle Cell Disease Implementation Consortium. Adults with sickle cell disease aged 18 to 45 years who had a visit to a participating emergency department for vaso-occlusive episodes within 90 days prior to enrollment will be eligible for inclusion. Patients will be enrolled in the clinic or remotely. The target analytical sample size of this study is 160 patient participants (20 per site) who have had an emergency department visit for vaso-occlusive episode treatment at participating emergency departments during the study period. Each site is expected to enroll approximately 40 participants to reach the analytical sample size. The electronic health record-embedded individualized pain plans will be written by the patient\u27s sickle cell disease provider, and sites will work with the local informatics team to identify the best method to build the electronic health record-embedded individualized pain plan with patient and provider access. Each site will adopt required patient and provider implementation strategies and can choose to adopt optional strategies to improve the uptake and sustainability of the intervention. The study is informed by the Technology Acceptance Model 2 and the Reach, Effectiveness, Adoption, Implementation, and Maintenance framework. Provider and patient baseline survey, follow-up survey within 96 hours of an emergency department vaso-occlusive episode visit, and selected qualitative interviews within 2 weeks of an emergency department visit will be performed to assess the primary outcome, patient-perceived quality of emergency department pain treatment, and additional implementation and intervention outcomes. Electronic health record data will be used to analyze individualized pain plan adherence and additional secondary outcomes, such as hospital admission and readmission rates. RESULTS: The study is currently enrolling study participants. The active implementation period is 18 months. CONCLUSIONS: This study proposes a structured, framework-informed approach to implement electronic health record-embedded individualized pain plans with both patient and provider access in routine emergency department practice. The results of the study will inform the implementation of electronic health record-embedded individualized pain plans at a larger scale outside of Sickle Cell Disease Implementation Consortium centers.
