Micro-Scale, Meso-Scale, Macro-Scale, and Temporal Scale: Comparing the Relative Importance for Robbery Risk in New York City
We compare the relative importance of four dimensions for explaining the micro location of robberies: 1) the micro spatial scale of street segments; 2) the meso spatial scale surrounding the street segment; 3) the temporal pattern, and 4) the macro-scale of the surrounding 2.5 miles. This study uses crime, business, and land use data from New York City and aggregates it to street segments and hours of the day. Although the measures capturing the micro-scale of the street segment explained the largest amount of unique variance, the measures capturing temporal scale across hours of the day (and weekdays) explained the next largest amount of unique variance. The measures of the characteristics in the 2.5 miles macro scale explained the next largest amount of unique variance, and combined with the measures at the meso-scale explained nearly as much of the variance as the street segment measures
Racially diverse neighborhoods in diverse areas are linked to lower crime rates
In new research, Young-An Kim and James C. Wo look at the implications of increasing racial and ethnic diversity in Americaβs communities on crime. Using evidence from Southern California, they tested the effects of racial diversity on neighborhood crime rates, finding that there were fewer violent and property crimes in neighborhood blocks which were more diverse, and especially those in more diverse areas. They attribute this to the greater opportunities created by diverse communities to overcome bias and create social ties and trust across different groups
Easy Diagnosis of Asthma: Computer-Assisted, Symptom-Based Diagnosis
Diagnosis of asthma is often challenging in primary-care physicians due to lack of tools measuring airway obstruction and variability. Symptom-based diagnosis of asthma utilizing objective diagnostic parameters and appropriate software would be useful in clinical practice. A total of 302 adult patients with respiratory symptoms responded to a questionnaire regarding asthma symptoms and provoking factors. Questions were asked and recorded by physicians into a computer program. A definite diagnosis of asthma was made based on a positive response to methacholine bronchial provocation or bronchodilator response (BDR) testing. Multivariate logistic regression analysis was used to evaluate the significance of questionnaire responses in terms of discriminating asthmatics. Asthmatic patients showed higher total symptom scores than non-asthmatics (mean 5.93 vs. 4.93; p<0.01). Multivariate logistic regression analysis identified that response to questions concerning the following significantly discriminated asthmatics; wheezing with dyspnea, which is aggravated at night, and by exercise, cold air, and upper respiratory infection. Moreover, the presence of these symptoms was found to agree significantly with definite diagnosis of asthma (by kappa statistics). Receiver-operating characteristic curve analysis revealed that the diagnostic accuracy of symptom-based diagnosis was high with an area under the curve of 0.647Β±0.033. Using a computer-assisted symptom-based diagnosis program, it is possible to increase the accuracy of diagnosing asthma in general practice, when the facilities required to evaluate airway hyperresponsiveness or BDR are unavailable
The impact of hyperhidrosis on patients' daily life and quality of life : A qualitative investigation
Background: An understanding of the daily life impacts of hyperhidrosis and how patients deal with them, based on qualitative research, is lacking. This study investigated the impact of hyperhidrosis on the daily life of patients using a mix of qualitative research methods. Methods: Participants were recruited through hyperhidrosis patient support groups such as the Hyperhidrosis Support Group UK. Data were collected using focus groups, interviews and online surveys. A grounded theory approach was used in the analysis of data transcripts. Data were collected from 71 participants, out of an initial 100 individuals recruited. Results: Seventeen major themes capturing the impacts of hyperhidrosis were identified; these covered all areas of life including daily life, psychological well-being, social life, professional /school life, dealing with hyperhidrosis, unmet health care needs and physical impact. Conclusions: Psychosocial impacts are central to the overall impacts of hyperhidrosis, cutting across and underlying the limitations experienced in other areas of life.Peer reviewe
Narrow-band imaging versus white light for the detection of proximal colon serrated lesions: a randomized, controlled trial
Background
The value of narrow-band imaging (NBI) for detecting serrated lesions is unknown.
Objective
To assess NBI for the detection of proximal colon serrated lesions.
Design
Randomized, controlled trial.
Setting
Two academic hospital outpatient units.
Patients
Eight hundred outpatients 50 years of age and older with intact colons undergoing routine screening, surveillance, or diagnostic examinations.
Interventions
Randomization to colon inspection in NBI versus white-light colonoscopy.
Main Outcome Measurements
The number of serrated lesions (sessile serrated polyps plus hyperplastic polyps) proximal to the sigmoid colon.
Results
The mean inspection times for the whole colon and proximal colon were the same for the NBI and white-light groups. There were 204 proximal colon lesions in the NBI group and 158 in the white light group (P = .085). Detection of conventional adenomas was comparable in the 2 groups.
Limitations
Lack of blinding, endoscopic estimation of polyp location.
Conclusion
NBI may increase the detection of proximal colon serrated lesions, but the result in this trial did not reach significance. Additional study of this issue is warranted. (Clinical trial registration number: NCT01572428.
Variation in genes encoding eosinophil granule proteins in atopic dermatitis patients from Germany
<p>Abstract</p> <p>Background</p> <p>Atopic dermatitis (AD) is believed to result from complex interactions between genetic and environmental factors. A main feature of AD as well as other allergic disorders is serum and tissue eosinophilia. Human eosinophils contain high amounts of cationic granule proteins, including eosinophil cationic protein (ECP), eosinophil-derived neurotoxin (EDN), eosinophil peroxidase (EPO) and major basic protein (MBP). Recently, variation in genes encoding eosinophil granule proteins has been suggested to play a role in the pathogenesis of allergic disorders. We therefore genotyped selected single nucleotide polymorphisms within the <it>ECP, EDN, EPO </it>and <it>MBP </it>genes in a cohort of 361 German AD patients and 325 healthy controls.</p> <p>Results</p> <p>Genotype and allele frequencies did not differ between patients and controls for all polymorphisms investigated in this study. Haplotype analysis did not reveal any additional information.</p> <p>Conclusion</p> <p>We did not find evidence to support an influence of variation in genes encoding eosinophil granule proteins for AD pathogenesis in this German cohort.</p
A systematic review of the evidence for single stage and two stage revision of infected knee replacement
BACKGROUND:
Periprosthetic infection about the knee is a devastating complication that may affect between 1% and 5% of knee replacement. With over 79 000 knee replacements being implanted each year in the UK, periprosthetic infection (PJI) is set to become an important burden of disease and cost to the healthcare economy. One of the important controversies in treatment of PJI is whether a single stage revision operation is superior to a two-stage procedure. This study sought to systematically evaluate the published evidence to determine which technique had lowest reinfection rates.
METHODS:
A systematic review of the literature was undertaken using the MEDLINE and EMBASE databases with the aim to identify existing studies that present the outcomes of each surgical technique. Reinfection rate was the primary outcome measure. Studies of specific subsets of patients such as resistant organisms were excluded.
RESULTS:
63 studies were identified that met the inclusion criteria. The majority of which (58) were reports of two-stage revision. Reinfection rated varied between 0% and 41% in two-stage studies, and 0% and 11% in single stage studies. No clinical trials were identified and the majority of studies were observational studies.
CONCLUSIONS:
Evidence for both one-stage and two-stage revision is largely of low quality. The evidence basis for two-stage revision is significantly larger, and further work into direct comparison between the two techniques should be undertaken as a priority
