    Health transformation project and defensive medicine practice among neurosurgeons in Turkey.

    The term "Defensive" medicine was coined in the early 1970's and has been an important topic of scientific investigation and professional debate ever since.The aim of this study was to investigate the characteristics of defensive medicine, its reasons, and the extent to which it is practiced in the Turkish health care system. This is the first national survey to study the practice of defensive medicine among neurosurgeons in Turkey.The present cross-sectional study on defensive medicine assessed neurosurgeons registered at the Turkish Neurosurgical Society, who are actively working in various centers and hospitals within the Turkish health care system. A 40-question survey was adapted from existing measures described in the literature and was completed by a total of 404 neurosurgeons, representing 36.7% of the neurosurgeons registered at the Turkish Neurosurgical Society.Seventy-two percent of the participants in the current study reported practicing defensive medicine. This practice was mainly reported among inexperienced neurosurgeons (74.4%). Most were younger than 40 years of age (75.2%), working in state hospitals/universities (72.7%), and living in the Marmara region (38%). Respondents reported engaging in defensive medicine by avoiding high-risk surgery (62.6%), ordering additional imaging studies (60.9%) and laboratory tests (33.7%), and referring patients to consultants (31.2%). Most participants consider every patient as a potential threat in terms of a medical lawsuit (68.3%) and do not believe the courts can distinguish malpractice from complications (89.6%).Concerns and perceptions about medical liability lead neurosurgeons to practice defensive medicine. By avoiding high-risk surgery, ordering unnecessary diagnostic tests, and referring the patients to consultants, neurosurgeons try to minimize the risk of malpractice and protect themselves from legal risks, resulting in higher healthcare expenditure and longer treatment periods

    Kemaliye (Erzincan) Ecology Based Nature Education Project in Participants' Eyes

    The goal of Kemaliye (Erzincan) Ecology-Based Nature Education Project is to give education people whose job is or is going to be training broad masses of the society about the observing components of nature with their senses, exploring living and unliving entities in their habitats and reflecting their theoretical knowledge on nature to their practices. The participations of the research were, research assistants of universities, teachers, Ph.D. and M.A. students, under graduate students, those working at related institutions is considered appropriate. Participants were attend the research for 9 days during the project program as two different groups (N:60). 19 ecology based nature education activities were involved in project. Three open ended questions were asked to participants to get their suggestion and criticism about the changes in their lives because of the project and how they will spread their gained knowledge and experience. Data obtained from their answers are analyzed by using qualitative research method with inductive analysis. At the end of the project, themes which were related to the sharing the knowledge and increasing awareness (f : 16) and reflecting professional environment and life (f: 29) are the result of the study. It is thought that both education period and participants' experience are beneficial to gain a different point of view and contributions to other science education researches in this Ecology-Based Nature Education Project which supported by TUBITAK.     Nebivolol compared with metoprolol for erectile function in males undergoing coronary artery bypass graft

    Objective: The aim of this study was to evaluate erectile function in males undergoing coronary artery bypass graft (CABG) while on two different adrenoceptor beta-blocker regimens, namely nebivolol and metoprolol. We hypothesize that the negative effects of cardiopulmonary bypass on erectile function may be possibly attenuated by preferring a vasodilating selective beta(1)-blocker, nebivolol, to metoprolol as an anti-ischemic and antiarrhythmic agent in males undergoing CABG
