    The practice of telemedicine and challenges to the regulatory authorities

    The practice of telemedicine is viewed as a possible solution to the human resources crisis in health care in South Africa and internationally. Reports on its successful implementation and combating of health-related problems are readily available from both developed and developing countries. Even though these reports indicate that telemedicine seems to have addressed the problem related to the shortage of health care personnel, it is still posing a challenge to regulatory authorities such as the Health Professions Council of South Africa. The regulatory authorities are there to ensure that quality health care service is delivered and that the patient will be protected from possible mismanagement by the health care practitioners involved. Misconduct can occur through improper clinical care or excessive billing. Policies and guidelines from both developing and developed countries were reviewed to highlight how telemedicine is regulated elsewhere. The focus was on the ethical implications of telemedicine practice. Telemedicine has proved itself to be a possible solution to the human resources crisis, especially in developing countries or where there are vast rural communities. Success of telemedicine has been reported in a number of developed and developing countries as a way of alleviating the human resources crisis and providing quality health care to needy communities

    Bagasse energy cogeneration potential in the South African sugar industry

    The paper explores the potential energy cogeneration effect of bagasse in the South African sugar industry. At the present moment very few sugar mills are exporting surplus electricity generated to the national grid. There is a need to take advantage of the abundant bagasse that is produced from the sugar industry through more efficient combustion processes to co-generate process steam and electricity for the supply to the grid. The industry also needs to take advantage of available technologies to efficiently generate electricity from bagasse using high pressure steam boilers. Using the current available technology the industry has a potential to produce about 3000 GWh per year and this is about 1.3% of the current generating capacity in South Africa. This translates to a generating capacity of 343 MW. Application of Integrated gasification combined cycle can result in 11000 GWh and this is about 5% of the current generating capacity at Eskom. In terms of MW this is equivalent to a generating capacity of 1255 MW. The other benefit derived from cogeneration is the1.619 kilo tonnes reduction in the use of coal which also translates to 2.84Mt of carbon dioxide. The cogeneration project will also improve the competitiveness of the sugar industry provided a viable price per KWh of electricity is agreed upon

    Prevalence of emotional, physical and sexual abuse of women in three South African provinces.

    INTRODUCTION: There is growing recognition in the ranks of the South African government that violence against women is a serious problem facing us all. Until now data on the epidemiology of violence against women in South Africa have been scanty. This report presents the findings of the first major community-based prevalence study. OBJECTIVES: To describe the prevalence of physical, sexual, financial, and emotional abuse of women. METHODS: A cross-sectional study conducted in the Eastern Cape (EC), Mpumalanga (MP) and the Northern Province (NP). The sample included one randomly selected woman aged 18-49 years living in each of 2,232 households. The sample was drawn using stratified, multistage, random methods, and 1,306 questionnaires were completed, giving a 90.3% response rate after adjusting for households without an eligible woman. RESULTS: The prevalences of ever having been physically abused by a current or ex-partner were 26.8% (EC), 28.4% (MP) and 19.1% (NP). The prevalences of abuse in the last year were 10.9% (EC), 11.9% (MP) and 4.5% (NP). The prevalences of rape were 4.5% (EC), 7.2% (MP) and 4.8% (NP). Considerable emotional and financial abuse was also reported, e.g. the prevalences of a partner having boasted about or brought home girlfriends in the previous year were 5.0% (EC), 10.4% (MP) and 7.0% (NP): The prevalences of physical abuse during a pregnancy were 9.1% (EC), 6.7% (MP) and 4.7% (NP). The proportions of abused women who were injured in the year before the survey were 34.5% (EC), 48.0% (MP) and 60.0% (NP). CONCLUSIONS: This study is the first large-scale, community-based prevalence study to be undertaken in South Africa

    Medical waste disposal at a hospital in Mpumalanga Province, South Africa: Implications for training of healthcare professionals

    Background. Healthcare professionals (HCPs) produce various types of waste in the course of rendering healthcare services. Each classification of waste must be disposed of according to the prescribed guidelines. Incorrect disposal of waste may pose a danger to employees, patients and the environment. HCPs must have adequate knowledge of the disposal of medical waste.Objectives. To determine the knowledge and practices of HCPs with regard to medical waste disposal at a hospital in Mpumalanga Province, South Africa.Methods. A quantitative cross-sectional research approach was used. The study respondents included nurses, medical doctors, dental health staff and allied health staff. Data were collected through self-administered questionnaires and analysed using IBM SPSS version 22.0.Results. A high proportion of HCPs did not have adequate knowledge regarding the disposal of medical waste, but nevertheless disposed of medical waste appropriately. While the knowledge and practices of HCPs with regard to medical waste disposal were not associated with age, gender or years of experience, there was an association between professional category and knowledge and practices.Conclusions. Disposal of medical waste is the responsibility of all HCPs. All categories of HCPs should receive regular training to improve their knowledge regarding disposal of medical waste and to minimise the risks associated with improper waste management. This will further increase compliance with the guidelines on disposal of medical waste

    The impact of smart-warehousing on a local foodservice equipment company’s external customers

    Abstract: This paper highlights the impact that smartwarehousing has on customer satisfaction and loyalty in the foodservice industry. The study follows an exploratory approach which uses quantitative means to analyze the information collected. The high products demand in the foodservice makes the industry complex to handle as the need to satisfy the customer is a priority, thus the reason behind this study which can benefit both the concerned industry and academia. 90 respondents (60 customers and 30 warehouse employees) participated in the study where a questionnaire was given to them to collect critical information about the study. The retrieved data were captured and analyzed using Microsoft Excel was used to help determine the sample size as well as random numbers to define the sample. SPSS 25 was also used to conduct the correlation analysis and assess the relationship between the warehouse activities and the customers’ credit notes (which are linked to their satisfaction and loyalty). The study revealed that there is a positive relationship between the use of smart-warehousing and the satisfaction and loyalty of customers

    Preferences and skills of Indian public sector teachers

    With a sample of 700 future public sector primary teachers in India, a Discrete Choice Experiment is used to measure job preferences, particularly regarding location. General skills are also tested. Urban origin teachers and women are more averse to remote locations than rural origin teachers and men respectively. Women would require a 26-73 percent increase in salary for moving to a remote location. The results suggest that existing caste and gender quotas can be detrimental for hiring skilled teachers willing to work in remote locations. The most preferred location is home, which supports decentralised hiring, although this could compromise skills

    Developing a tool to measure health worker motivation in district hospitals in Kenya

    BACKGROUND: We wanted to try to account for worker motivation as a key factor that might affect the success of an intervention to improve implementation of health worker practices in eight district hospitals in Kenya. In the absence of available tools, we therefore aimed to develop a tool that could enable a rapid measurement of motivation at baseline and at subsequent points during the 18-month intervention study. METHODS: After a literature review, a self-administered questionnaire was developed to assess the outcomes and determinants of motivation of Kenyan government hospital staff. The initial questionnaire included 23 questions (from seven underlying constructs) related to motivational outcomes that were then used to construct a simpler tool to measure motivation. Parallel qualitative work was undertaken to assess the relevance of the questions chosen and the face validity of the tool. RESULTS: Six hundred eighty-four health workers completed the questionnaires at baseline. Reliability analysis and factor analysis were used to produce the simplified motivational index, which consisted of 10 equally-weighted items from three underlying factors. Scores on the 10-item index were closely correlated with scores for the 23-item index, indicating that in future rapid assessments might be based on the 10 questions alone. The 10-item motivation index was also able to identify statistically significant differences in mean health worker motivation scores between the study hospitals (p<0.001). The parallel qualitative work in general supported these conclusions and contributed to our understanding of the three identified components of motivation. CONCLUSION: The 10-item score developed may be useful to monitor changes in motivation over time within our study or be used for more extensive rapid assessments of health worker motivation in Kenya

    Hands washing, glove use, and avoiding recontamination before aseptic procedures at birth: A multicenter time-and-motion study conducted in Zanzibar.

    BACKGROUND: Our primary objective was to assess hand hygiene (HH) compliance before aseptic procedures among birth attendants in the 10 highest-volume facilities in Zanzibar. We also examined the extent to which recontamination contributes to poor HH. Recording exact recontamination occurrences is not possible using the existing World Health Organization HH audit tool. METHODS: In this time-and-motion study, 3 trained coders used WOMBATv2 software to record the hand actions of all birth attendants present in the study sites. The percentage compliance and 95% confidence intervals (CIs) for individual behaviors (hand washing/rubbing, avoiding recontamination and glove use) and for behavioral sequences during labor and delivery were calculated. RESULTS: We observed 104 birth attendants and 781 HH opportunities before aseptic procedures. Compliance with hand rubbing/washing was 24.6% (95% CI, 21.6-27.8). Only 9.6% (95% CI, 7.6-11.9) of birth attendants also donned gloves and avoided recontamination. Half of the time when rubbing/washing or glove donning was performed, hands were recontaminated prior to the aseptic procedure. CONCLUSIONS: In this study, HH compliance by birth attendants before aseptic procedures was poor. To our knowledge, this is the first study in a low- to middle-income country to show the large contribution to poor HH compliance from hand and glove recontamination before the procedure. Recontamination is an important driver of infection risk from poor HH. It should be understood for the purposes of improvement and therefore included in HH monitoring and interventions
