Liver abscess following ingestion of a foreign object
A 31 year old, previously healthy female presented to A&E on 24/07/2012 with a 2 day history of colicky epigastric pain and spiking fever; however on investigation no pathology was found except for an ovarian cyst of 4cm. It was concluded that it was unlikely that the cyst was causing pain and fever and the patient was discharged. She was given proton pump inhibitors for 2 weeks and pain improved, however on stopping therapy, the pain became much more severe, with radiation to the back. She presented to A&E again on 05/08/2012. A more detailed history elicited the fact that she had ingested half a toothpick by mistake 3 weeks previously. Imaging showed the formation of an abscess between the stomach and liver; which needed drainage.peer-reviewe
Discitis
Discitis is an inflammatory condition of the intervertebral disc or disc space. It is an uncommon condition which occurs mostly
in young children or as a post-operative complication, such as following an epidural. It is extremely rare in the elderly as disc size
and sponginess decreases with age and consequently the risk of infection decreases. In this case, discitis occurred spontaneously
in an elderly patient and was complicated by a psoas abscess. The patient also suffers from acute renal failure, chronic heart failure,
left sided pneumonia and shingles (Herpes Zoster) in dermatome S3. A trapped spinal nerve occurring at the intervertebral
foramen is a common complication in fact, the subject did suffer from sciatica for the past 30 years.peer-reviewe
Right Neural Substrates of Language and Music Processing Left Out: Activation Likelihood Estimation (ALE) and Meta-Analytic Connectivity Modelling (MACM)
Introduction: Language and music processing have been investigated in neuro-based research for over a century. However, consensus of independent and shared neural substrates among the domains remains elusive due to varying neuroimaging methodologies. Identifying functional connectivity in language and music processing via neuroimaging meta-analytic methods provides neuroscientific knowledge of higher cognitive domains and normative models may guide treatment development in communication disorders based on principles of neural plasticity.
Methods: Using BrainMap software and tools, the present coordinate-based meta-analysis analyzed 65 fMRI studies investigating language and music processing in healthy adult subjects. We conducted activation likelihood estimates (ALE) in language processing, music processing, and language + music (Omnibus) processing. Omnibus ALE clusters were used to elucidate functional connectivity by use of meta-analytic connectivity modelling (MACM). Paradigm Class and Behavioral Domain analyses were completed for the ten identified nodes to aid functional MACM interpretation.
Results: The Omnibus ALE revealed ten peak activation clusters (bilateral inferior frontal gyri, left medial frontal gyrus, right superior temporal gyrus, left transverse temporal gyrus, bilateral claustrum, left superior parietal lobule, right precentral gyrus, and right anterior culmen). MACM demonstrates an interconnected network consisting of unidirectional and bidirectional connectivity. Subsequent analyses demonstrated nodal involvement across 44 BrainMap paradigms and 32 BrainMap domains.
Discussion: These findings demonstrate functional connectivity among Omnibus areas of activation in language and music processing. We analyze ALE and MACM outcomes by comparing them to previously observed roles in cognitive processing and functional network connectivity. Finally, we discuss the importance of translational neuroimaging and need for normative models guiding intervention

In high income countries, jumping and diving into water are a small but persistent cause of death and serious injury especially among male youth and young adults. Although water entries maintain a high media profile, little is known about what entry competencies and underlying water safety knowledge youth bring to this practice. Undergraduates enrolled in aquatics (N= 76) completed a survey before attempting 7 entry jumping and diving tasks. While safety attitudes and self-reported behaviours were generally good, considerable variation in practical entry competence was evident. Most completed a deep-water compact jump (87%) and PFD jump (88%) with ease. Many completed a crouch dive (57%) and standing dive (53%) into deep water with ease, but only 33% completed a standing dive from a block/bulkhead (<1m height) with ease. Ways of addressing weaknesses in knowledge, attitudes, and behaviours are discussed and recommendations made to enhance the teaching of safe water entry. © 2021 Human Kinetics Publishers Inc.. Acute interstitial nephritis with acetaminophen and alcohol intoxication
Drug-induced acute interstitial nephritis (AIN) represents a growing cause of renal failure in current medical practice. While antimicrobials and non-steroidal anti-inflammatory drugs are typically associated with drug-induced AIN, few reports have been made on the involvement of other analgesics. We report our experience in managing a 17-year-old female with AIN and subsequent renal injury following an acetaminophen overdose in conjunction with acute alcohol intoxication. It is well established that acetaminophen metabolism, particularly at high doses, produces reactive metabolites that may induce renal and hepatic toxicity. It is also plausible however, that such reactive species could instead alter renal peptide immunogenicity, thereby inducing AIN. In the following report, we review a possible mechanism for the acetaminophen-induced AIN observed in our patient and also discuss the potential involvement of acute alcohol ingestion in disease onset. The objective of our report is to increase awareness of healthcare professionals to the potential involvement of these commonly used agents in AIN pathogenesis
