    Radiculopathy as Delayed Presentations of Retained Spinal Bullet.

    Bullet injuries to the spine may cause injury to the anatomical structures with or without neurologic deterioration. Most bullet injuries are acute, resulting from direct injury. However, in rare cases, delayed injury may occur, resulting in claudication. We report a case of intradural bullet at the L3-4 level with radiculopathy in a 30-year-old male. After surgical removal, radicular and claudicating pain were improved significantly, and motor power of the right leg also improved.     Stereotactic guidance for navigated percutaneous sacroiliac joint fusion.

    Arthrodesis of the sacroiliac joint (SIJ) for surgical treatment of SIJ dysfunction has regained interest among spine specialists. Current techniques described in the literature most often utilize intraoperative fluoroscopy to aid in implant placement; however, image guidance for SIJ fusion may allow for minimally invasive percutaneous instrumentation with more precise implant placement. In the following cases, we performed percutaneous stereotactic navigated sacroiliac instrumentation using O-armĀ® multidimensional surgical imaging with StealthStationĀ® navigation (Medtronic, Inc. Minneapolis, MN). Patients were positioned prone and an image-guidance reference frame was placed contralateral to the surgical site. O-armĀ® integrated with StealthStationĀ® allowed immediate auto-registration. The skin incision was planned with an image-guidance probe. An image-guided awl, drill and tap were utilized to choose a starting point and trajectory. Threaded titanium cage(s) packed with autograft and/or allograft were then placed. O-armĀ® image-guidance allowed for implant placement in the SIJ with a small skin incision. However, we could not track the cage depth position with our current system, and in one patient, the SIJ cage had to be revised secondary to the anterior breach of sacrum

    Antiretroviral Genotypic Resistance Mutations in HIV-1 Infected Korean Patients with Virologic Failure

    Resistance assays are useful in guiding decisions for patients experiencing virologic failure (VF) during highly-active antiretroviral therapy (HAART). We investigated antiretroviral resistance mutations in 41 Korean human immunodeficiency virus type 1 (HIV-1) infected patients with VF and observed immunologic/virologic response 6 months after HAART regimen change. Mean HAART duration prior to resistance assay was 45.3Ā±27.5 months and commonly prescribed HAART regimens were zidovudine/lamivudine/nelfinavir (22.0%) and zidovudine/lamivudine/efavirenz (19.5%). Forty patients (97.6%) revealed intermediate to high-level resistance to equal or more than 2 antiretroviral drugs among prescribed HAART regimen. M184V/I mutation was observed in 36 patients (87.7%) followed by T215Y/F (41.5%) and M46I/L (34%). Six months after resistance assay and HAART regimen change, median CD4+ T cell count increased from 168 cells/ĀµL (interquartile range [IQR], 62-253) to 276 cells/ĀµL (IQR, 153-381) and log viral load decreased from 4.65 copies/mL (IQR, 4.18-5.00) to 1.91 copies/mL (IQR, 1.10-3.60) (P<0.001 for both values). The number of patients who accomplished viral load <400 copies/mL was 26 (63.4%) at 6 months follow-up. In conclusion, many Korean HIV-1 infected patients with VF are harboring strains with multiple resistance mutations and immunologic/virologic parameters are improved significantly after genotypic resistance assay and HAART regimen change

    ABSTRACT A spacer grid is one of the most important structural components in a LWR fuel assembly. The spacer grid, which supports nuclear fuel rods laterally and vertically with a friction grip, is an interconnected array of slotted grid straps welded at the intersections to form an egg-crate structure. Dimples and springs are stamped into each grid strap to support the fuel rods. The form of grid straps and spring form is known to be closely related with the crush strength of spacer grid assembly and the integrity of fuel rod support, respectively. Zircaloy is prevailing as the material of the spacer grid because of its low neutron absorption characteristic and its successful extensive in-reactor use. The primary considerations are to provide a Zircaloy spacer grid with crush strength sufficient to resist design basis loads especially due to seismic accidents, without significantly increasing pressure drop across the reactor core. Generally, the thickness and height of the Zircaloy grid strap have been the main design variables in order to meet the above considerations. Recently, it was reported that a dimple location is also a design variable that affects the crush strength of a spacer grid assembly. In this study, a new spacer grid form was developed in order to enhance the integrity of the fuel rod support and the crush strength of the spacer grid assembly by using a systematic optimization technique. Finite element analysis and crush strength tests on the developed new spacer grid form were carried out to check the performance enhancement compared to commercial spacer grids. The enhancement of fuel rod support was confirmed by comparisons of contact area, peak stresses, plastic deformation and etc. According to the results, it is estimated that the actual critical load enhancement of the spacer grid assembly is approximately up to 30 % and the actual contact area, when a fuel rod inserted into a spacer grid cell, is more than double for the developed new spacer grid form. And also, some design variables that effect the crush strength of a PWR spacer grid assembly were classified and their effects on the crush strength were investigated by a finite element analysis and a crush strength test
