    Elastic foundation effects on arch dams

    Earthquake response of an arch dam should be calculated under ground motion effects. This study presents three-dimensional linear earthquake response of an arch dam. Thereby, we considered different ground motion effects and also foundation conditions in the finite element analyses. For this purpose, the Type 3 double curvature arch dam was selected for application. All numerical analyses are carried out by SAP2000 program for empty reservoir cases. In the scope of this study, linear modal time-history analyses are performed using three dimensional finite element model of the arch dam and arch dam-foundation interaction systems. According to numerical analyses, maximum horizontal displacements and maximum normal stresses are presented by dam height in the largest section. These results are evaluated for rigid and various elastic foundation conditions. Furthermore, near-fault and far-field ground motion effects on the selected arch dam are taken into account by different accelerograms obtained from the Loma Prieta earthquake at various distances

    Global respiratory syncytial virus–related infant community deaths

    Background Respiratory syncytial virus (RSV) is a leading cause of pediatric death, with >99% of mortality occurring in low- and lower middle-income countries. At least half of RSV-related deaths are estimated to occur in the community, but clinical characteristics of this group of children remain poorly characterized. Methods The RSV Global Online Mortality Database (RSV GOLD), a global registry of under-5 children who have died with RSV-related illness, describes clinical characteristics of children dying of RSV through global data sharing. RSV GOLD acts as a collaborative platform for global deaths, including community mortality studies described in this supplement. We aimed to compare the age distribution of infant deaths <6 months occurring in the community with in-hospital. Results We studied 829 RSV-related deaths <1 year of age from 38 developing countries, including 166 community deaths from 12 countries. There were 629 deaths that occurred <6 months, of which 156 (25%) occurred in the community. Among infants who died before 6 months of age, median age at death in the community (1.5 months; IQR: 0.8−3.3) was lower than in-hospital (2.4 months; IQR: 1.5−4.0; P < .0001). The proportion of neonatal deaths was higher in the community (29%, 46/156) than in-hospital (12%, 57/473, P < 0.0001). Conclusions We observed that children in the community die at a younger age. We expect that maternal vaccination or immunoprophylaxis against RSV will have a larger impact on RSV-related mortality in the community than in-hospital. This case series of RSV-related community deaths, made possible through global data sharing, allowed us to assess the potential impact of future RSV vaccines

    Dilated cardiomyopathy secondary to rickets-related hypocalcaemia: eight case reports and a review of the literature

    Introduction: Dilated cardiomyopathy is usually idiopathic and may arise secondary to infections or metabolic or genetic causes. Another rare cause is hypocalcaemia. Owing to the fact that calcium plays an essential role in excitation and contraction of myocardial muscle, myocardial contractility may decline in patients with hypocalcaemia. Materials and Methods: Patients with symptoms of congestive heart failure and rickets-related hypocalcaemia were assessed clinically and by echocardiography in a paediatric cardiology clinic. Echocardiography was performed for all patients. Rickets was diagnosed according to the clinical, laboratory, and radiologic findings. Maternal lifestyle and living conditions were investigated, and the maternal 25-OH vitamin D-3 blood level was measured. Results: We evaluated eight patients who developed heart failure as a result of severe hypocalcaemia associated with rickets between August, 1999 and June, 2012. The age distribution of the patients was 3-12 months. Laboratory results were consistent with advanced-stage rickets. Severe hypocalcaemia was detected in all patients. The maternal 25-OH vitamin D-3 levels were low. Echocardiography revealed increased pre-treatment left ventricle end-systolic and end-diastolic diameters for age and reduced ejection fraction and fractional shortening. After clinical improvement, the patients were discharged. Conclusions: Severe hypocalcaemia associated with rickets must always be kept in mind among the causes of dilated cardiomyopathy and impaired cardiac function in infants. If diagnosed and treated in time, dilated cardiomyopathy and severe heart failure related to rickets respond well

    Seismotectonic database of Turkey

    Turkey is located in one of the most seismically active regions in the world. Characterizing seismic source zones in this region requires evaluation and integration of geological, geophysical, seismological and geodetical data. This first seismotectonic database for Turkey presented herein was prepared, under the framework of the National Earthquake Strategy and Action Plan-2023. The geographic information system (GIS)-based database includes maps of active faults, catalogues of instrumental and historical earthquakes, moment tensor solutions and data on crustal thickness. On the basis of these data, 18 major seismotectonic zones were delineated for Turkey and the surrounding region. The compilation and storage of the seismotectonic data sets in a digital GIS will allow analyses and systematic updates as new data accrete over time
