Interdisciplinary three-step strategy to treat aortic stenosis and coronary artery disease in a patient with end-stage chronic obstructive pulmonary disease
Background: Valvular aortic stenosis is a common disease in the elderly, often in multimorbid patients. It is often associated with coronary artery disease and peripheral artery disease. In this situation, the risk of conventional open-heart surgery is too high, and other treatment strategies have to be evaluated.
Case report: A 79-year-old female patient with severe aortic stenosis, coronary artery disease and end-stage chronic obstructive pulmonary disease suffering from dyspnea at rest and permanently dependent on oxygen was treated in three steps. Firstly, her pulmonary infection was treated with antibiotics for 7 days. Then, the left anterior descending artery was stented (bare-metal stent). In the same session, valvuloplasty of the aortic valve was performed. She was sent to rehabilitation to improve her pulmonary condition and took clopidogrel for 4 weeks. Finally, she underwent transapical aortic valve replacement. She was released to rehabilitation on postoperative day 12.
Conclusion: A combination of modern interventional and minimally invasive surgical techniques to treat aortic stenosis and coronary heart disease can be a viable option for multimorbid patients with extremely high risk in conventional open-heart surgery
Perspectives in Microvascular Fluid Handling: Does the Distribution of Coagulation Factors in Human Myocardium Comply with Plasma Extravasation in Venular Coronary Segments?
Background: Heterogeneity of vascular permeability has been suggested for the coronary system. Whereas arteriolar and capillary segments are tight, plasma proteins pass readily into the interstitial space at venular sites. Fittingly, lymphatic fluid is able to coagulate. However, heart tissue contains high concentrations of tissue factor, presumably enabling bleeding to be stopped immediately in this vital organ. The distribution of pro- and anti-coagulatively active factors in human heart tissue has now been determined in relation to the types of microvessels. Methods and Results: Samples of healthy explanted hearts and dilated cardiomyopathic hearts were immunohistochemically stained. Albumin was found throughout the interstitial space. Tissue factor was packed tightly around arterioles and capillaries, whereas the tissue surrounding venules and small veins was practically free of this starter of coagulation. Thrombomodulin was present at the luminal surface of all vessel segments and especially at venular endothelial cell junctions. Its product, the anticoagulant protein C, appeared only at discrete extravascular sites, mainly next to capillaries. These distribution patterns were basically identical in the healthy and diseased hearts, suggesting a general principle. Conclusions: Venular extravasation of plasma proteins probably would not bring prothrombin into intimate contact with tissue factor, avoiding interstitial coagulation in the absence of injury. Generation of activated protein C via thrombomodulin is favored in the vicinity of venular gaps, should thrombin occur inside coronary vessels. This regionalization of distribution supports the proposed physiological heterogeneity of the vascular barrier and complies with the passage of plasma proteins into the lymphatic system of the heart. Ten-Year Results of a Randomized Trial Comparing Tacrolimus Versus Cyclosporine A in Combination With Mycophenolate Mofetil After Heart Transplantation
Background. Long-term results of prospective randomized trials comparing triple immunosuppressive strategies combining tacrolimus (TAC) or cyclosporine A (CsA) with mycophenolate mofetil (MMF) and steroids after heart transplantation (HTX) are rarely published. Therefore, we collected long-term follow-up data of an intervention cohort 10 years after randomization. Methods. Ten-year follow-up data of 60 patients included in a prospective, randomized trial between 1998 and 2000 were analyzed as intention-to-treat (TAC-MMF n=30; CsA-MMF n=30). Baseline characteristics were well balanced. Cardiac allograft vasculopathy (CAV) was graduated in accordance with the new ISHLT classification. Results. Survival at 1, 5, and 10 years was 96.7%, 80.0%, and 66.7% for TAC-MMF and 90.0%, 83.3%, and 80.0% for CsA-MMF (P=ns). Freedom from acute rejection (AR) was significantly higher in TAC-MMF versus CsA-MMF (65.5% vs. 21.7%, log-rank 8.3, P=0.004). Freedom from ISHLT >= CAV(1) after 5 and 10 years was in TAC-MMF 64.0% and 45.8%, and in CsA-MMF 36.0% (log-rank 3.0, P=0.085) and 8.0% (log-rank 9.0, P=0.003). No difference in long-term results for freedom from coronary angioplasty or stenting, renal dysfunction, diabetes mellitus, CMV infection, or malignancy was detected. Conclusion. Cross-over effects because of treatment switch may result in impairment of significance between the groups. The long-term analysis resulted in a significant difference in manifestation of CAV between the groups after 10 years. Less rejection in the TAC-group might have contributed to the lower incidence of CAV. Superior freedom from AR and CAV in the TAC-MMF group did not result in better long-term survival
Decoupled carbonate chemistry controls on the incorporation of boron into Orbulina universa
In order to fully constrain paleo-carbonate systems, proxies for two out of seven parameters, plus temperature and salinity, are required. The boron isotopic composition (ÎŽ11B) of planktonic foraminifera shells is a powerful tool for reconstructing changes in past surface ocean pH. As B(OH)4â is substituted into the biogenic calcite lattice in place of CO32â, and both borate and carbonate ions are more abundant at higher pH, it was suggested early on that BâŻââŻCa ratios in biogenic calcite may serve as a proxy for [CO32â]. Although several recent studies have shown that a direct connection of BâŻââŻCa to carbonate system parameters may be masked by other environmental factors in the field, there is ample evidence for a mechanistic relationship between BâŻââŻCa and carbonate system parameters. Here, we focus on investigating the primary relationship to develop a mechanistic understanding of boron uptake.
Differentiating between the effects of pH and [CO32â] is problematic, as they co-vary closely in natural systems, so the major control on boron incorporation remains unclear. To deconvolve the effects of pH and [CO32â] and to investigate their impact on the BâŻââŻCa ratio and ÎŽ11B, we conducted culture experiments with the planktonic foraminifer Orbulina universa in manipulated culture media: constant pH (8.05), but changing [CO32â] (238, 286 and 534âŻÂ”molâŻkgâ1âŻCO32â) and at constant [CO32â] (276âŻÂ±âŻ19.5âŻÂ”molâŻkgâ1) and varying pH (7.7, 7.9 and 8.05). Measurements of the isotopic composition of boron and the BâŻââŻCa ratio were performed simultaneously using a femtosecond laser ablation system coupled to a MC-ICP-MS (multiple-collector inductively coupled plasma mass spectrometer). Our results show that, as expected, ÎŽ11B is controlled by pH but it is also modulated by [CO32â]. On the other hand, the BâŻââŻCa ratio is driven by [HCO3â], independently of pH. This suggests that BâŻââŻCa ratios in foraminiferal calcite can possibly be used as a second, independent, proxy for complete paleo-carbonate system reconstructions. This is discussed in light of recent literature demonstrating that the primary relationship between BâŻââŻCa and [HCO3â] can be obscured by other environmental parameters
oxLDL Downregulates the Dendritic Cell Homing Factors CCR7 and CCL21
Introduction. Dendritic cells (DCs) and oxLDL play an important role in the atherosclerotic process with DCs accumulating in the plaques during plaque progression. Our aim was to investigate the role of oxLDL in the modulation of the DC homing-receptor CCR7 and endothelial-ligand CCL21. Methods and Results. The expression of the DC homing-receptor CCR7 and its endothelial-ligand CCL21 was examined on atherosclerotic carotic plaques of 47 patients via qRT-PCR and immunofluorescence. In vitro, we studied the expression of CCR7 on DCs and CCL21 on human microvascular endothelial cells (HMECs) in response to oxLDL. CCL21- and CCR7-mRNA levels were significantly downregulated in atherosclerotic plaques versus non-atherosclerotic controls [90% for CCL21 and 81% for CCR7 (P < 0.01)]. In vitro, oxLDL reduced CCR7 mRNA levels on DCs by 30% and protein levels by 46%. Furthermore, mRNA expression of CCL21 was significantly reduced by 50% (P < 0.05) and protein expression by 24% in HMECs by oxLDL (P < 0.05). Conclusions. The accumulation of DCs in atherosclerotic plaques appears to be related to a downregulation of chemokines and their ligands, which are known to regulate DC migration. oxLDL induces an in vitro downregulation of CCR7 and CCL21, which may play a role in the reduction of DC migration from the plaques

Preventing Cardiac Allograft Vasculopathy: Long-Term Beneficial Effects of Mycophenolate Mofetil
Abstract Heart transplantation is a well established therapy for end-stage heart failure. Long-term results are limited by malignancy and cardiac allograft vasculopathy. The causes for cardiac allograft vasculopathy and the predictors for its onset and progression are multifactorial and determined by both immunological and non-immunological risk factors. The detection of cardiac allograft vasculopathy is difficult owing to the denervation of the allograft and the diffuse disease progression, especially in medium-sized and smaller arteries, in a diffuse and concentric fashion. The treatment options are limited and often do not provide long-term success so that retransplantation remains the only solution in some cases. Therefore, the emphasis has to be placed on cardiac allograft vasculopathy prevention. A variety of strategies for endothelial protection exist, beginning with matching and virtual crossmatching prior to transplantation. Further approaches involve preservation solutions and additives as well as transportation modalities and the prevention of ischemic injury during reperfusion. After transplantation, the interplay of recipient nonimmune cardiovascular risk factors and the effects of cellular and antibody mediated rejection both injuring the allograft endothelium have to be considered, leading to an optimized medical prevention of cardiac allograft vasculopathy. Established medical approaches for cardiac allograft vasculopathy prevention involve statins, vasodilators, infection prophylaxis, proliferation signal inhibitors, mycophenolate mofetil, and endothelial protective agents. This review will look at the pathophysiology of cardiac allograft vasculopathy, its diagnosis, and current and future concepts for cardiac allograft vasculopathy prevention
