Detrimental effect of cardiopulmonary bypass(CPB) on malignant disease
Patients with coronary artery disease associated with malignancy are a difficult group of patients to treat. The ideal approach to manage them is still controversial. Both problems can be manage by either a combined or staged operation. The use of CPB during revascularization of the myocardium among patients with malignant disease, may have an effect on dissimination of malignant cells. This was observed among two of our patients. We believe that the use of off-pump technique to revascularize the myocardium is a safe approach and can be performed either in combined or staged surgery to resect malignant disease
Ram pressure feeding super-massive black holes
When supermassive black holes at the center of galaxies accrete matter
(usually gas), they give rise to highly energetic phenomena named Active
Galactic Nuclei (AGN). A number of physical processes have been proposed to
account for the funneling of gas towards the galaxy centers to feed the AGN.
There are also several physical processes that can strip gas from a galaxy, and
one of them is ram pressure stripping in galaxy clusters due to the hot and
dense gas filling the space between galaxies. We report the discovery of a
strong connection between severe ram pressure stripping and the presence of AGN
activity. Searching in galaxy clusters at low redshift, we have selected the
most extreme examples of jellyfish galaxies, which are galaxies with long
tentacles of material extending for dozens of kpc beyond the galaxy disk. Using
the MUSE spectrograph on the ESO Very Large Telescope, we find that 6 out of
the 7 galaxies of this sample host a central AGN, and two of them also have
galactic-scale AGN ionization cones. The high incidence of AGN among the most
striking jellyfishes may be due to ram pressure causing gas to flow towards the
center and triggering the AGN activity, or to an enhancement of the stripping
caused by AGN energy injection, or both. Our analysis of the galaxy position
and velocity relative to the cluster strongly supports the first hypothesis,
and puts forward ram pressure as another, yet unforeseen, possible mechanism

In a recent article, Tonnesen et al suggest that a nicotine inhaler in smoking cessation could be implemented in general practitioner offices with high success rates and that it would be "acceptable" to patients. In addition, the program is described as "low intervention." Results from their study do not support such statements
Itch and skin rash from chocolate during fluoxetine and sertraline treatment: Case report
BACKGROUND: The skin contains a system for producing serotonin as well as serotonin receptors. Serotonin can also cause pruritus when injected into the skin. SSRI-drugs increase serotonin concentrations and are known to have pruritus and other dermal side effects. CASE PRESENTATION: A 46-year-old man consulted his doctor due to symptoms of depression. He did not suffer from any allergy but drinking red wine caused vasomotor rhinitis. Antidepressive treatment with fluoxetine 20 mg daily was initiated which was successful. After three weeks of treatment an itching rash appeared. An adverse drug reaction (ADR) induced by fluoxetine was suspected and fluoxetine treatment was discontinued. The symptoms disappeared with clemastine and betametasone treatment. Since the depressive symptoms returned sertraline medication was initiated. After approximately two weeks of sertraline treatment he noted an intense itching sensation in his scalp after eating a piece of chocolate cake. The itch spread to the arms, abdomen and legs and the patient treated himself with clemastine and the itch disappeared. He now realised that he had eaten a chocolate cake before this episode and remembered that before the first episode he had had a chocolate mousse dessert. He had never had any reaction from eating chocolate before and therefore reported this observation to his doctor. CONCLUSIONS: This case report suggests that there may be individuals that are very sensitive to increases in serotonin concentrations. Dermal side reactions to SSRI-drugs in these patients may be due to high activity in the serotonergic system at the dermal and epidermo-dermal junctional area rather than a hypersensitivity to the drug molecule itself
Chandra HETGS Multi-Phase Spectroscopy of the Young Magnetic O Star theta^1 Orionis C
We report on four Chandra grating observations of the oblique magnetic
rotator theta^1 Ori C (O5.5 V) covering a wide range of viewing angles with
respect to the star's 1060 G dipole magnetic field. We employ line-width and
centroid analyses to study the dynamics of the X-ray emitting plasma in the
circumstellar environment, as well as line-ratio diagnostics to constrain the
spatial location, and global spectral modeling to constrain the temperature
distribution and abundances of the very hot plasma. We investigate these
diagnostics as a function of viewing angle and analyze them in conjunction with
new MHD simulations of the magnetically channeled wind shock mechanism on
theta^1 Ori C. This model fits all the data surprisingly well, predicting the
temperature, luminosity, and occultation of the X-ray emitting plasma with

Hot gas in Mach cones around Virgo Cluster spiral galaxies
The detailed comparison between observations and simulations of ram pressure
stripped spiral galaxies in the Virgo cluster has led to a three dimensional
view of the galaxy orbits within the hot intracluster medium. The 3D velocities
and Mach numbers derived from simulations can be used to derive simple Mach
cone geometries for Virgo spiral galaxies. We search for indications of hot gas
within Mach cones in X-ray observations of selected Virgo Cluster spiral
galaxies (NGC 4569, NGC 4388, and NGC 4501). We find extraplanar diffuse X-ray
emission in all galaxies. Based on the 3D velocity vectors from dynamical
modelling a simple Mach cone is fitted to the triangular shape of NGC 4569's
diffuse X-ray emission. Assuming that all extraplanar diffuse X-ray emission
has to be located inside the Mach cone, we also fit Mach cones to NGC 4388's
and NGC 4501's extraplanar X-ray emission. For NGC 4569 it is hard to reconcile
the derived Mach cone opening angle with a Mach number based on the sound speed
alone. Instead, a Mach number involving the Alfv\'enic speed seems to be more
appropriate, yielding a magnetic field strength of -6 G for a
intracluster medium density of cm. Whereas the
temperature of the hot component of NGC 4569's X-ray halo (0.5 keV) is at the
high end but typical for a galactic outflow, the temperature of the hot gas
tails of NGC 4388 and NGC 4501 are significantly hotter (0.7-0.9 keV). In NGC
4569 we find direct evidence for a Mach cone which is filled with hot gas from
a galactic superwind. We suggest that the high gas temperatures in the X-ray
tails of NGC 4388 and NGC 4501 are due to the mixing of the stripped ISM into

The GALFA-HI Compact Cloud Catalog
We present a catalog of 1964 isolated, compact neutral hydrogen clouds from
the Galactic Arecibo L-Band Feed Array Survey Data Release One (GALFA-HI DR1).
The clouds were identified by a custom machine-vision algorithm utilizing
Difference of Gaussian kernels to search for clouds smaller than 20'. The
clouds have velocities typically between |VLSR| = 20-400 km/s, linewidths of
2.5-35 km/s, and column densities ranging from 1 - 35 x 10^18 cm^-2. The
distances to the clouds in this catalog may cover several orders of magnitude,
so the masses may range from less than a Solar mass for clouds within the
Galactic disc, to greater than 10^4 Solar Masses for HVCs at the tip of the
Magellanic Stream. To search for trends, we separate the catalog into five
populations based on position, velocity, and linewidth: high velocity clouds
(HVCs); galaxy candidates; cold low velocity clouds (LVCs); warm, low
positive-velocity clouds in the third Galactic Quadrant; and the remaining warm
LVCs. The observed HVCs are found to be associated with previously-identified
HVC complexes. We do not observe a large population of isolated clouds at high
velocities as some models predict. We see evidence for distinct histories at
low velocities in detecting populations of clouds corotating with the Galactic

LoCuSS: Luminous infrared galaxies in the merging cluster Abell 1758 at z=0.28
We present the first galaxy evolution results from the Local Cluster
Substructure Survey (LoCuSS), a multi-wavelength survey of 100 X-ray selected
galaxy clusters at 0.15<z<0.30. LoCuSS combines far-UV through far-IR
observations of cluster galaxies with gravitational lensing analysis and X-ray
data to investigate the interplay between the hierarchical assembly of clusters
and the evolution of cluster galaxies. Here we present new panoramic
Spitzer/MIPS 24micron observations of the merging cluster Abell 1758 at z=0.279
spanning 6.5x6.5Mpc and reaching a 90% completeness limit of 400uJy. We
estimate a global cluster SFR of 910\pm320 M_sun/yr within 3 Mpc of the cluster
centre, originating from 42 galaxies with L_IR > 5x10^10 L_sun. The obscured
activity in A1758 is therefore comparable with that in Cl 0024+1654, the most
active cluster previously studied at 24um. The obscured galaxies faithfully
trace the cluster potential as revealed by the weak-lensing mass map of the
cluster, including numerous mass peaks at R~2-3Mpc that are likely associated
with infalling galaxy groups and filamentary structures. However the core
(R<500kpc) of A1758N is 2x more active in the IR than that of A1758S, likely
reflecting differences in the recent dynamical history of the two clusters. The
24micron results from A1758 therefore suggest that dust-obscured cluster
galaxies are common in merging clusters and suggests that obscured activity in
clusters is triggered by both the details of cluster-cluster mergers and
processes that operate at larger radii including those within in-falling
groups. Our ongoing far-UV through far-IR observations of a large sample of
clusters should allow us to disentangle the different physical processes
