    Design considerations for a wearable monitor to measure finger posture

    BACKGROUND: Objective measures of hand function as individuals participate in home and community activities are needed in order to better plan and evaluate rehabilitation treatments. Traditional measures collected in the clinical setting are often not reflective of actual functional performance. Recent advances in technology, however, enable the development of a lightweight, comfortable data collection monitor to measure hand kinematics. METHODS: This paper presents the design analysis of a wearable sensor glove with a specific focus on the sensors selected to measure bend. The most important requirement for the glove is easy donning and removal for individuals with significantly reduced range of motion in the hands and fingers. Additional requirements include comfort and durability, cost effectiveness, and measurement repeatability. These requirements eliminate existing measurement gloves from consideration. Glove construction is introduced, and the sensor selection and glove evaluation process are presented. RESULTS: Evaluation of commercial bend sensors shows that although most are not appropriate for repeatable measurements of finger flexion, one has been successfully identified. A case study for sensor glove repeatability using the final glove configuration and sensors does show a high degree of repeatability in both the gripped and flat hand positions (average coefficient of variability = 2.96% and 0.10%, respectively). CONCLUSION: Measuring functional outcomes in a portable manner can provide a wealth of information important to clinicians for the evaluation and treatment of movement disorders in the hand and fingers.     Accurate Dielectric Response of Solids: Combining the Bethe-Salpeter Equation with the Random Phase Approximation

    The Bethe-Salpeter equation (BSE) can provide an accurate description of low-energy optical spectra of insulating crystals - even when excitonic effects are important. However, due to high computational costs it is only possible to include a few bands in the BSE Hamiltonian. As a consequence, the dielectric screening given by the real part of the dielectric function can be significantly underestimated by the BSE. Here we show that universally accurate optical response functions can be obtained by combining a four-point BSE-like equation for the irreducible polarisability with a two-point Dyson equation which includes the higher-lying transitions within the random phase approximation (RPA). The new method is referred to as BSE+. It has a computational cost comparable to the BSE but a much faster convergence with respect to the size of the electron-hole basis. We use the method to calculate refractive indices and electron energy loss spectra for a test set of semiconductors and insulators. In all cases the BSE+ yields excellent agreement with experimental data across a wide frequency range and outperforms both BSE and RPA.Comment: 6 pages, 4 figure

    High Mass Triple Systems: The Classical Cepheid Y Car

    We have obtained an HST STIS ultraviolet high dispersion Echelle mode spectrum the binary companion of the double mode classical Cepheid Y Car. The velocity measured for the hot companion from this spectrum is very different from reasonable predictions for binary motion, implying that the companion is itself a short period binary. The measured velocity changed by 7 km/ s during the 4 days between two segments of the observation confirming this interpretation. We summarize "binary" Cepheids which are in fact members of triple system and find at least 44% are triples. The summary of information on Cepheids with orbits makes it likely that the fraction is under-estimated.Comment: accepted by A

    Polaris B, an optical companion of Polaris (alpha UMi) system: atmospheric parameters, chemical composition, distance and mass

    We present an analysis of high-resolution spectroscopic observations of Polaris B, the optical companion of the Polaris Ab system. The star has a radial velocity V_r of -16.6km/s to -18.9km/s, and a projected rotational velocity vsini=110 km/s. The derived atmospheric parameters are: Teff=6900K; logg=4.3; V_t=2.5km/s. Polaris B has elemental abundances generally similar to those of the Cepheid Polaris A (Usenko et al. 2005a), although carbon, sodium and magnesium are close to the solar values. At a spectral type of F3V Polaris B has a luminosity of 3.868L_sun, an absolute magnitude of +3.30mag, and a distance of 109.5pc. The mass of the star is estimated to be 1.39M_sun, close to a mass of 1.38+/-0.61M_sun for the recently-resolved orbital periods companion Polaris Ab observed by Evans et al.     Survivors of Chronic Stroke Experience Continued Impairment of Dexterity But Not Strength in the Nonparetic Upper Limb

    Objective To investigate the performance of the less affected upper limb in people with stroke compared with normative values. To examine less affected upper limb function in those whose prestroke dominant limb became paretic and those whose prestroke nondominant limb became paretic. Design Cohort study of survivors of chronic stroke (7.2±6.7y post incident). Setting The study was performed at a freestanding academic rehabilitation hospital. Participants Survivors of chronic stroke (N=40) with severe hand impairment (Chedoke-McMaster Stroke Assessment rating of 2-3 on Stage of Hand) participated in the study. In 20 participants the prestroke dominant hand (DH) was tested (nondominant hand [NH] affected by stroke), and in 20 participants the prestroke NH was tested (DH affected by stroke). Interventions Not applicable. Main Outcome Measure Jebsen-Taylor Hand Function Test. Data from survivors of stroke were compared with normative age- and sex-matched data from neurologically intact individuals. Results When combined, DH and NH groups performed significantly worse on fine motor tasks with their nonparetic hand relative to normative data (PP\u3e.140). Conclusions Survivors of stroke with severe impairment of the paretic limb continue to present significant upper extremity impairment in their nominally nonparetic limb even years after stroke. This phenomenon was observed regardless of whether the DH or NH hand was primarily affected. Because this group of survivors of stroke is especially dependent on the nonparetic limb for performing functional tasks, our results suggest that the nonparetic upper limb should be targeted for rehabilitation

    Polaris the Cepheid returns: 4.5 years of monitoring from ground and space

    We present the analysis of 4.5 years of nearly continuous observations of the classical Cepheid Polaris, which comprise the most precise data available for this star. We have made spectroscopic measurements from ground and photometric measurements from the WIRE star tracker and the SMEI instrument on the Coriolis satellite. Measurements of the amplitude of the dominant oscillation (P = 4 days), that go back more than a century, show a decrease from 120 mmag to 30 mmag (V magnitude) around the turn of the millennium. It has been speculated that the reason for the decrease in amplitude is the evolution of Polaris towards the edge of the instability strip. However, our new data reveal an increase in the amplitude by about 30% from 2003-2006. It now appears that the amplitude change is cyclic rather than monotonic, and most likely the result of a pulsation phenomenon. In addition, previous radial velocity campaigns have claimed the detection of long-period variation in Polaris (P > 40 days). Our radial velocity data are more precise than previous datasets, and we find no evidence for additional variation for periods in the range 3-50 days with an upper limit of 100 m/s. However, in the WIRE data we find evidence of variation on time-scales of 2-6 days, which we interpret as being due to granulation.Comment: Accepted by ApJ. 8 pages. Quality of figures 1+3+7 degrade

    Hubble Space Telescope WFPC-2 Imaging of Cassiopeia A

    The young SNR Cassiopeia A was imaged with WFPC-2 through four filters selected to capture the complete velocity range of the remnant's main shell in several important emission lines. Primary lines detected were [O III] 4959,5007, [N II] 6583, [S II] 6716,6731 + [O II] 7319,7330 + [O I] 6300,6364, and [S III] 9069,9532. About 3/4th of the remnant's main shell was imaged in all four filters. Considerable detail is observed in the reverse-shocked ejecta with typical knot scale lengths of 0.2"-0.4" (1 - 2 x 10^16 cm). Both bright and faint emission features appear highly clumped. Large differences in [S III] and [O III] line intensities indicating chemical abundance differences are also seen, particularly in knots located along the bright northern limb and near the base of the northeast jet. A line of curved overlapping filament in the remnant's northwestern rim appears to mark the location of the remnant's reverse shock front in this region. Finger-like ejecta structures elsewhere suggest cases where the reverse shock front is encountering the remnant's clumped ejecta. Narrow-band [N II] images of the remnant's circumstellar knots ("QSFs") reveal them to be 0.1"-0.6" thick knots and filaments, often with diffuse edges facing away from the center of expansion. Three color composite images of the whole remnant and certain sections along with individual filter enlargements of selected regions of the bright optical shell are presented and discussed.Comment: 26 pages, 12 figures Accepted to the Astronomical Journa

    Do health education initiatives assist socioeconomically disadvantaged populations? : a systematic review and meta-analyses

    Background: Health education interventions are considered critical for the prevention and management of conditions of public health concern. Although the burden of these conditions is often greatest in socio-economically disadvantaged populations, the effectiveness of interventions that target these groups is unknown. We aimed to identify and synthesize evidence of the effectiveness of health-related educational interventions in adult disadvantaged populations. Methods: We pre-registered the study on Open Science Framework https://osf.io/ek5yg/. We searched Medline, Embase, Emcare, and the Cochrane Register from inception to 5/04/2022 to identify studies evaluating the effectiveness of health-related educational interventions delivered to adults in socio-economically disadvantaged populations. Our primary outcome was health related behaviour and our secondary outcome was a relevant biomarker. Two reviewers screened studies, extracted data and evaluated risk of bias. Our synthesis strategy involved random-effects meta-analyses and vote-counting. Results: We identified 8618 unique records, 96 met our criteria for inclusion – involving more than 57,000 participants from 22 countries. All studies had high or unclear risk of bias. For our primary outcome of behaviour, meta-analyses found a standardised mean effect of education on physical activity of 0.05 (95% confidence interval (CI) = -0.09–0.19), (5 studies, n = 1330) and on cancer screening of 0.29 (95% CI = 0.05–0.52), (5 studies, n = 2388). Considerable statistical heterogeneity was present. Sixty-seven of 81 studies with behavioural outcomes had point estimates favouring the intervention (83% (95% CI = 73%-90%), p < 0.001); 21 of 28 studies with biomarker outcomes showed benefit (75% (95%CI = 56%-88%), p = 0.002). When effectiveness was determined based on conclusions in the included studies, 47% of interventions were effective on behavioural outcomes, and 27% on biomarkers. Conclusions: Evidence does not demonstrate consistent, positive impacts of educational interventions on health behaviours or biomarkers in socio-economically disadvantaged populations. Continued investment in targeted approaches, coinciding with development of greater understanding of factors determining successful implementation and evaluation, are important to reduce inequalities in health

    Vortex Structure Around a Magnetic Dot in Planar Superconductors

    The problem of the giant vortex state around a magnetic dot which is embedded in a superconducting film is investigated. The full non-linear, self-consistent Ginzburg-Landau equations are solved numerically in order to calculate the free energy, the order parameter of the host superconductor, the internal magnetic field due to the supercurrents, the corresponding current density, the magnetization probed in the vicinity of the dot, and the normal electron density as a function of the various parameters of the system. We find that, as we increase the magnetic moment of the dot, higher flux quanta vortex states become energetically more favorable, as they can better compete with the external magnetic field via the Meissner effect. In addition to that, they progressively become closer to each other in energy with direct experimental consequences, i.e. physical quantities like magnetization may fluctuate when measured, for example, as a function of a uniform external magnetic field.Comment: text 21 pages (REVTEX), 8 figures available upon reques
