    Assessment of ambient assisted living systems for patients with mild cognitive impairment

    According to the World Health Organization, about 50 million people worldwide suffer from dementia. Ten million new cases added every year. Mild Cognitive Impairment (MCI) affects more than 15% of the population aged 65. Technological solutions, such as smart home technology with ubiquitous computing devices, 24/7 telemedical observation and support can alleviate the growing problem and lower pressure on the healthcare system. This approach is also preferable for homecare patients in distant and rural areas. MCI patients are mostly home-based. Ambient Assisted Living (AAL) systems provide tools for automatic registration of vital signs and other medically and socially important information. AAL system for MCI patients is a logical answer to the problem. At the same time, many of the proposed AAL systems are proprietary, technically complicated and have a high price tag for implementation and service. Also, some proposed technical solutions not entirely reflect the opinion of healthcare stakeholders. The current study was proposed as a way to bridge the possible differences in the positions. An online anonymous questionnaire for healthcare professionals was created to prove or disprove the number of interconnected hypotheses about the necessity and feasibility of AAL system for MCI patients. The main focus was made on the hypotheses: "There is necessity of AAL systems for the healthcare" and "AAL systems are capable of providing assistance for patients with Mild Cognitive Impairment". The questionnaire was presented to more than three hundred potential respondents. Around a hundred and twenty agreed to fill it, and sixty completed the whole questionnaire. Results were analyzed to produce some directions guideline for future technical applications of AAL systems for MCI patients and future research. Descriptive statistics show support for the implementation of general AAL and variants for MCI patients. Comparative analysis of ordinal data for specific groups of respondents is done with help of non-parametric tests. Mann–Whitney–Wilcoxon test and Kruskal-Wallis test are applied. Table questions results are analyzed with chisquare for frequency tables. Group analysis demonstrated relative positive uniformity in of responses in the support of AAL of MCI patients.Segundo a Organização Mundial da Saúde, cerca de 50 milhões de pessoas em todo o mundo sofrem de demência. Dez milhões de novos casos adicionados a cada ano. O comprometimento cognitivo leve (MCI) afeta mais de 15% da população com 65 anos. Soluções tecnológicas, como tecnologia de casa inteligente com dispositivos de computação onipresentes, observação e suporte telemédico 24 horas por dia, 7 dias por semana, podem aliviar o problema crescente e diminuir a pressão sobre o sistema de saúde. Essa abordagem também é preferível para pacientes de cuidados domiciliares em áreas distantes e rurais. Os pacientes com CCL são, em sua maioria, domiciliares. Os sistemas Ambient Assisted Living (AAL) fornecem ferramentas para registro automático de sinais vitais e outras informações médicas e socialmente importantes. O sistema AAL para pacientes com MCI é uma resposta lógica para o problema. Ao mesmo tempo, muitos dos sistemas AAL propostos são proprietários, tecnicamente complicados e têm um alto preço para implementação e serviço. Além disso, algumas soluções técnicas propostas não refletem inteiramente a opinião das partes interessadas na área da saúde. O presente estudo foi proposto como forma de colmatar as possíveis diferenças nas posições. Um questionário anônimo online para profissionais de saúde foi criado para comprovar ou refutar o número de hipóteses interligadas sobre a necessidade e viabilidade do sistema AAL para pacientes com CCL. O foco principal foi feito nas hipóteses: "Há necessidade de sistemas de AAL para a saúde" e "Os sistemas de AAL são capazes de prestar assistência a pacientes com Comprometimento Cognitivo Leve". O questionário foi apresentado a mais de trezentos respondentes potenciais. Cerca de cento e vinte concordaram em preenchê-lo e sessenta preencheram todo o questionário. Os resultados foram analisados para produzir algumas diretrizes para futuras aplicações técnicas de sistemas AAL para pacientes com MCI e pesquisas futuras. Estatísticas descritivas mostram suporte para a implementação de AAL geral e variantes para pacientes com CCL. A análise comparativa de dados ordinais para grupos específicos de respondentes é feita com a ajuda de testes não paramétricos. Aplicam-se os testes de Mann-Whitney-Wilcoxon e Kruskal-Wallis. Os resultados das questões da tabela são analisados com qui-quadrado para tabelas de frequência. A análise do grupo demonstrou relativa uniformidade positiva nas respostas no suporte de AAL de pacientes com CCL.Selon l'Organisation mondiale de la santé, environ 50 millions de personnes dans le monde souffrent de démence. Les solutions technologiques, telles que la technologie de la maison intelligente avec des appareils informatiques omniprésents, l'observation et le soutien télémédicaux 24 heures sur 24, 7 jours sur 7, peuvent atténuer le problème croissant et réduire la pression sur le système de santé. Cette approche est également préférable pour les patients en soins à domicile dans les régions éloignées et rurales. Les patients MCI sont pour la plupart à domicile. Les systèmes Ambient Assisted Living (AAL) fournissent des outils pour l'enregistrement automatique des signes vitaux et d'autres informations importantes sur le plan médical et social. Le système AAL pour les patients MCI est une réponse logique au problème. Dans le même temps, bon nombre des systèmes AAL proposés sont propriétaires, techniquement compliqués et ont un prix élevé pour la mise en oeuvre et le service. De plus, certaines solutions techniques proposées ne reflètent pas entièrement l'opinion des acteurs de santé. L'étude actuelle a été proposée comme un moyen de combler les différences possible dans les positions. Un questionnaire anonyme en ligne destiné aux professionnels de la santé a été créé pour prouver ou réfuter le nombre d'hypothèses interconnectées sur la nécessité et la faisabilité du système AAL pour les patients MCI. L'accent a été mis principalement sur les hypothèses: "Il existe une nécessité de systèmes AAL pour les soins de santé" et "Les systèmes AAL sont capables de fournir une assistance aux patients atteints de troubles cognitifs légers". Le questionnaire a été présenté à plus de trois cents répondants potentiels. Environ cent vingt ont accepté de le remplir, et soixante ont rempli tout le questionnaire. Les résultats ont été analysés pour produire des lignes directrices pour les futures applications techniques des systèmes AAL pour les patients MCI et l'avenir de la recherche. Les statistiques descriptives montrent un soutien à la mise en oeuvre de l'AAL général et des variantes pour les patients MCI. L'analyse comparative des données ordinales pour des groupes spécifiques de répondants est effectuée à l'aide de tests non paramétriques. Le test de Mann-Whitney-Wilcoxon et le test de Kruskal-Wallis sont appliqués. Les résultats des questions de tableau sont analysés avec le chi carré pour les tableaux de fréquence.     Genetic epidemiology of BRCA1- and BRCA2-associated cancer across Latin America

    The prevalence and contribution of BRCA1/2 (BRCA) pathogenic variants (PVs) to the cancer burden in Latin America are not well understood. This study aims to address this disparity. BRCA analyses were performed on prospectively enrolled Latin American Clinical Cancer Genomics Community Research Network participants via a combination of methods: a Hispanic Mutation Panel (HISPANEL) on MassARRAY; semiconductor sequencing; and copy number variant (CNV) detection. BRCA PV probability was calculated using BRCAPRO. Among 1,627 participants (95.2% with cancer), we detected 236 (14.5%) BRCA PVs; 160 BRCA1 (31% CNVs); 76 BRCA2 PV frequency varied by country: 26% Brazil, 9% Colombia, 13% Peru, and 17% Mexico. Recurrent PVs (seen ≥3 times), some region-specific, represented 42.8% (101/236) of PVs. There was no ClinVar entry for 14% (17/125) of unique PVs, and 57% (111/196) of unique VUS. The area under the ROC curve for BRCAPRO was 0.76. In summary, we implemented a low-cost BRCA testing strategy and documented a significant burden of non-ClinVar reported BRCA PVs among Latin Americans. There are recurrent, population-specific PVs and CNVs, and we note that the BRCAPRO mutation probability model performs adequately. This study helps address the gap in our understanding of BRCA-associated cancer in Latin America

    SIRT1 promotes the central adaptive response to diet restriction through activation of the dorsomedial and lateral nuclei of the hypothalamus

    Diet restriction retards aging and extends life span by triggering adaptive mechanisms that alter behavioral, physiological, and biochemical responses in mammals. Little is known about the molecular pathways evoking the corresponding central response. One factor that mediates the effects of diet restriction is the mammalian nicotinamide adenine dinucleotide (NAD)-dependent deacetylase SIRT1. Here we demonstrate that diet restriction significantly increases SIRT1 protein levels and induces neural activation in the dorsomedial and lateral hypothalamic nuclei. Increasing SIRT1 in the brain of transgenic (BRASTO) mice enhances neural activity specifically in these hypothalamic nuclei, maintains a higher range of body temperature, and promotes physical activity in response to different diet-restricting paradigms. These responses are all abrogated in Sirt1-deficient mice. SIRT1 up-regulates expression of the orexin type 2 receptor specifically in these hypothalamic nuclei in response to diet-restricting conditions, augmenting response to ghrelin, a gut hormone whose levels increase in these conditions. Our results suggest that in the hypothalamus, SIRT1 functions as a key mediator of the central response to low nutritional availability, providing insight into the role of the hypothalamus in the regulation of metabolism and aging in mammals

    The Effects of Repeated Dyspnea Exposure on Response Inhibition

    In order to treat dyspnea (=breathlessness) successfully, response inhibition (RI) as a major form of self-regulation is a premise. This is supported by research showing that self-regulation is associated with beneficial behavioral changes supporting treatment success in patients. Recent research showed that dyspnea has an impairing effect on RI, but the effects of repeated dyspnea exposure on RI remain unknown. Therefore, the present study tested the effects of repeated resistive load-induced dyspnea on RI over a 5-day period. Healthy volunteers (n = 34) performed the standard version of the Stroop task during baseline and dyspnea conditions on the first and fifth testing day and underwent an additional dyspnea exposure phase on each testing day. Variables of interest to investigate RI were reaction time, accuracy as well as the event-related potentials late positive complex (LPC) and N400 in the electroencephalogram. Reduced accuracy for incongruent compared to congruent stimuli during the dyspnea condition on the first testing day were found (p < 0.001). This was paralleled by a reduced LPC and an increased N400 for incongruent stimuli during the induction of dyspnea (p < 0.05). After undergoing dyspnea exposure, habituation of dyspnea intensity was evident. Importantly, on the fifth testing day, no differences between baseline, and dyspnea conditions were found for behavioral and electrophysiological measures of RI. These findings demonstrate that the impairing effect of dyspnea on RI disappeared after repeated dyspnea exposure in healthy participants. Translated to a clinical sample, it might cautiously be suggested that dyspnea exposure such as dyspnea perceived during physical exercise could reduce the impairing effect of dyspnea on RI which might have the potential to help increase self-regulation abilities and subsequent treatment efforts in dyspneic patients

Основные результаты: Рекомендации сформированы в таблицы для распространения между определенными целевыми группами. Данные материалы считаются удобными в использовании и не требуют глубокого профессионального погружения в вопросы феномена делистинга.     Detection of dimethylamine in the low pptv range using nitrate chemical ionization atmospheric pressure interface time-of-flight (CI-APi-TOF) mass spectrometry

    Amines are potentially important for atmospheric new particle formation, but their concentrations are usually low with typical mixing ratios in the pptv range or even smaller. Therefore, the demand for highly sensitive gas-phase amine measurements has emerged in the last several years. Nitrate chemical ionization mass spectrometry (CIMS) is routinely used for the measurement of gas-phase sulfuric acid in the sub-pptv range. Furthermore, extremely low volatile organic compounds (ELVOCs) can be detected with a nitrate CIMS. In this study we demonstrate that a nitrate CIMS can also be used for the sensitive measurement of dimethylamine (DMA, (CH3)(2)NH) using the NO3-center dot(HNO3)(1-2)center dot(DMA) cluster ion signal. Calibration measurements were made at the CLOUD chamber during two different measurement campaigns. Good linearity between 0 and similar to 120 pptv of DMA as well as a sub-pptv detection limit of 0.7 pptv for a 10 min integration time are demonstrated at 278 K and 38% RH.Peer reviewe

    Excitatory-inhibitory balance within EEG microstates and resting-state fMRI networks: assessed via simultaneous trimodal PET-MR-EEG imaging

    The symbiosis of neuronal activities and glucose energy metabolism is reflected in the generation of functional magnetic resonance imaging (fMRI) and electroencephalography (EEG) signals. However, their association with the balance between neuronal excitation and inhibition (E/I-B), which is closely related to the activities of glutamate and γ-aminobutyric acid (GABA) and the receptor availability (RA) of GABAA and mGluR5, remains unexplored. This research investigates these associations during the resting state (RS) condition using simultaneously recorded PET/MR/EEG (trimodal) data. The trimodal data were acquired from three studies using different radio-tracers such as, [11C]ABP688 (ABP) (N = 9), [11C]Flumazenil (FMZ) (N = 10) and 2-[18F]fluoro-2-deoxy-D-glucose (FDG) (N = 10) targeted to study the mGluR5, GABAA receptors and glucose metabolism respectively. Glucose metabolism and neuroreceptor binding availability (non-displaceable binding potential (BPND)) of GABAA and mGluR5 were found to be significantly higher and closely linked within core resting-state networks (RSNs). The neuronal generators of EEG microstates and the fMRI measures were most tightly associated with the BPND of GABAA relative to mGluR5 BPND and the glucose metabolism, emphasising a predominance of inhibitory processes within in the core RSNs at rest. Changes in the neuroreceptors leading to an altered coupling with glucose metabolism may render the RSNs vulnerable to psychiatric conditions. The paradigm employed here will likely help identify the precise neurobiological mechanisms behind these alterations in fMRI functional connectivity and EEG oscillations, potentially benefitting individualised healthcare treatment measures

    mGluR5 receptor availability is associated with lower levels of negative symptoms and better cognition in male patients with chronic schizophrenia

    Consistent findings postulate disturbed glutamatergic function (more specifically a hypofunction of the ionotropic NMDA receptors) as an important pathophysiologic mechanism in schizophrenia. However, the role of the metabotropic glutamatergic receptors type 5 (mGluR5) in this disease remains unclear. In this study, we investigated their significance (using [11 C]ABP688) for psychopathology and cognition in male patients with chronic schizophrenia and healthy controls. In the patient group, lower mGluR5 binding potential (BPND ) values in the left temporal cortex and caudate were associated with higher general symptom levels (negative and depressive symptoms), lower levels of global functioning and worse cognitive performance. At the same time, in both groups, mGluR5 BPND were significantly lower in smokers (F[27,1] = 15.500; p = .001), but without significant differences between the groups. Our findings provide support for the concept that the impaired function of mGluR5 underlies the symptoms of schizophrenia. They further supply a new perspective on the complex relationship between tobacco addiction and schizophrenia by identifying glutamatergic neurotransmission-in particularly mGluR5-as a possible connection to a shared vulnerability.
