    A Case Report of Case Report Pursuit by Medical Student

    Medical students often seek case reports as vehicles for academic writing opportunities, conference presentation avenues, and residency/fellowship application highlights. Here we review a case where, due to unfortunate circumstances, a student made a unique diagnosis central to proper patient clinical care, wished to write up the case subsequently, but was ultimately excluded from the final work stemming from the patient case. We review the pitfalls that occurred in the process of pursuing publication of an interesting case, the educational value of pursuing case reports for students, the necessity for strong mentorship in this process, and general principles that medical students can follow regarding case report creation to avoid being burned

    How Do Analyst Recommendations Respond to Major News?

    We examine how analysts respond to public information when setting stock recommendations. We model the determinants of analysts’ recommendation changes following large stock price movements. We find evidence of an asymmetry following large positive and negative returns. Following large stock price increases, analysts are equally likely to upgrade or downgrade. Following large stock price declines, analysts are more likely to downgrade. This asymmetry exists after accounting for investment banking relationships and herding behavior. This result suggests recommendation changes are “sticky” in one direction, with analysts reluctant to downgrade. Moreover, this result implies that analysts’ optimistic bias may vary through time

    Predictive Behavior of a Computational Foot/Ankle Model through Artificial Neural Networks

    Computational models are useful tools to study the biomechanics of human joints. Their predictive performance is heavily dependent on bony anatomy and soft tissue properties. Imaging data provides anatomical requirements while approximate tissue properties are implemented from literature data, when available. We sought to improve the predictive capability of a computational foot/ankle model by optimizing its ligament stiffness inputs using feedforward and radial basis function neural networks. While the former demonstrated better performance than the latter per mean square error, both networks provided reasonable stiffness predictions for implementation into the computational model

    Bone - Prosthesis Junction for Active Tendon Implants: A Biomechanical Comparison of Two Fixation Techniques

    End-to-end repair of damaged tendon is not possible in many instances of injury. Staged flexor tendon reconstruction has been proven to be a valuable method of addressing complex flexor tendon injuries when the patient presents late, has failed primary repair, or the injury mandates it (1). Silicone implants have been used for two-stage flexor tendon reconstructions with reports of long-term use as active implants (2,3). Implants have been reported functional approximately one year post-surgery with some still in use up to 25 years after reconstruction (4). This suggests the potential of the implant serving as a permanent prosthesis. The biomechanical strength of two proximal junction methods using a commercially available active tendon implant has been studied with a reported mean ultimate failure load of 220N (5). Biomechanical investigation of distal fixation has yet to be studied, but combined with the proximal results could lead to changes in two-staged flexor tendon reconstruction protocols allowing for earlier active range of motion, delayed Stage II surgeries, and perhaps permanent implantation.     Houses of stories: the whale rider at the American Museum of Natural History

    In April 2013, fifteen members of the Māori tribal arts group Toi Hauiti travelled to New York to reconnect with their carved wooden ancestor figure, Paikea, at the American Museum of Natural History (AMNH). They gave educational presentations  to school groups, museum staff and members of the public about Paikea and the whare kōrero , or house of stories, which Paikea had adorned as a gable figure.Through a discussion anchored in the importance of taonga (ancestral treasures), this paper describes embodied forms of knowledge used by Paikea’s descendants to know him in his absence, and introduce him to diverse audiences. Its foci are: museum education in multicultural contexts; learning by doing through the use of interactive activities; and community outreach and museum education. In addition, it discusses the challenges to protocols and opportunities for learning offered to AMNH staff through this engagement, and examines the impact it had son Toi Hauiti members themselves

    Environmental and public health policy continues to evolve in response to new and complex social, economic and environmental drivers. Globalization and centralization of commerce, evolving patterns of land use (e.g., urbanization, deforestation), and technological advances in such areas as manufacturing and development of genetically modified foods have created new and complex classes of stressors and risks (e.g., climate change, emergent and opportunist disease, sprawl, genomic change). In recognition of these changes, environmental risk assessment and its use are changing from stressor-endpoint specific assessments used in command and control types of decisions to an integrated approach for application in communitybased decisions. As a result, the process of risk assessment and supporting risk analyses are evolving to characterize the human-environment relationship. Integrating risk paradigms combine the process of risk estimation for humans, biota, and natural resources into one assessment to improve the information used in environmental decisions (Suter et al. 2003b). A benefit to this approach includes a broader, system-wide evaluation that considers the interacting effects of stressors on humans and the environment, as well the interactions between these entities. To improve our understanding of the linkages within complex systems, risk assessors will need to rely on a suite of techniques for conducting rigorous analyses characterizing the exposure and effects relationships between stressors and biological receptors. Many of the analytical techniques routinely employed are narrowly focused and unable to address the complexities of an integrated assessment. In this paper, we describe an approach to integrated risk assessment, and discuss qualitative community modeling and Probabilistic Relational Modeling techniques that address these limitations and evaluate their potential for use in an integrated risk assessment of cyanobacteria

    Trends and characteristics of accidental and intentional codeine overdose deaths in Australia

    Examines trends in codeine-related mortality rates in Australia, and the clinical and toxicological characteristics of codeine-related deaths. Abstract Objectives: To examine trends in codeine-related mortality rates in Australia, and the clinical and toxicological characteristics of codeine-related deaths. Design and setting: Analysis of prospectively collected data from the National Coronial Information System on deaths where codeine toxicity was determined to be an underlying or contributory cause of death. The study period was 2000–2013. Main outcome measures: Population-adjusted numbers (per million persons) of (1) codeine-related deaths, classified by intent (accidental or intentional); and (2) heroin- and Schedule 8 opioid-related deaths (as a comparator). Results: The overall rate of codeine-related deaths increased from 3.5 per million in 2000 to 8.7 per million in 2009. Deaths attributed to accidental overdoses were more common (48.8%) than intentional deaths (34.7%), and their proportion increased during the study period. High rates of prior comorbid mental health (53.6%), substance use (36.1%) and chronic pain (35.8%) problems were recorded for these deaths. For every two Schedule 8 opioid-related deaths in 2009, there was one codeine-related death. Most codeine-related deaths (83.7%) were the result of multiple drug toxicity. Conclusions: Codeine-related deaths (with and without other drug toxicity) are increasing as the consumption of codeine-based products increases. Educational messages are needed to better inform the public about the potential harms of chronic codeine use, especially in the context of polypharmacy
