    Impact of Diabetes on Postinfarction Heart Failure and Left Ventricular Remodeling

    Diabetes mellitus, the metabolic syndrome, and the underlying insulin resistance are increasingly associated with diastolic dysfunction and reduced stress tolerance. The poor prognosis associated with heart failure in patients with diabetes after myocardial infarction is likely attributable to many factors, important among which is the metabolic impact from insulin resistance and hyperglycemia on the regulation of microvascular perfusion and energy generation in the cardiac myocyte. This review summarizes epidemiologic, pathophysiologic, diagnostic, and therapeutic data related to diabetes and heart failure in acute myocardial infarction and discusses novel perceptions and strategies that hold promise for the future and deserve further investigation

    The role of partial knowledge in statistical word learning

    A critical question about the nature of human learning is whether it is an all-or-none or a gradual, accumulative process. Associative and statistical theories of word learning rely critically on the later assumption: that the process of learning a word’s meaning unfolds over time. That is, learning the correct referent for a word involves the accumulation of partial knowledge across multiple instances. Some theories also make an even stronger claim: Partial knowledge of one word–object mapping can speed up the acquisition of other word–object mappings. We present three experiments that test and verify these claims by exposing learners to two consecutive blocks of cross-situational learning, in which half of the words and objects in the second block were those that participants failed to learn in Block 1. In line with an accumulative account, Re-exposure to these mis-mapped items accelerated the acquisition of both previously experienced mappings and wholly new word–object mappings. But how does partial knowledge of some words speed the acquisition of others? We consider two hypotheses. First, partial knowledge of a word could reduce the amount of information required for it to reach threshold, and the supra-threshold mapping could subsequently aid in the acquisition of new mappings. Alternatively, partial knowledge of a word’s meaning could be useful for disambiguating the meanings of other words even before the threshold of learning is reached. We construct and compare computational models embodying each of these hypotheses and show that the latter provides a better explanation of the empirical data

    Structure-mapping processes enable infants' learning across domains including language

    Humans have an astounding ability to acquire new information. Like many other animals, we can learn by association and by perceptual generalization. However, unlike most other species, we also acquire new information by means of relational generalization and transfer. In this chapter, we explore the origins of a uniquely developed human capacity-our ability to learn relational abstractions through analogical comparison. We focus on whether and how infants can use analogical comparison to derive relational abstractions from examples. We frame our work in terms of structure-mapping theory, which has been fruitfully applied to analogical processing in children and adults. We find that young infants show two key signatures of structure mapping: first, relational abstraction is fostered by comparing alignable examples, and second, relational abstraction is hampered by the presence of highly salient objects. The studies we review make it clear that structure-mapping processes are evident in the first months of life, prior to much influence of language and culture. This finding suggests that infants are born with analogical processing mechanisms that allow them to learn relations through comparing examples

To this effect, this paper briefly reviews psychosomatics within the framework of holistic theory and care, which happens to be one of the key concepts of person-centered medicine (PCM). Next, the paper considers the basic notions of person-centered medicine and the way this has evolved historically from ancient civilizations through modern medicine. The conceptualization of PCM is further approached from a systematic study organized by the International College of Person Centered Medicine. After that, the paper focuses on person-centered psychiatry in general and then more specifically on person-centered diagnosis both as a theoretical model and as a practical guide
