    ACL injury prevention, more effective with a different way of motor learning?

    What happens to the transference of learning proper jump-landing technique in isolation when an individual is expected to perform at a competitive level yet tries to maintain proper jump-landing technique? This is the key question for researchers, physical therapists, athletic trainers and coaches involved in ACL injury prevention in athletes. The need for ACL injury prevention is clear, however, in spite of these ongoing initiatives and reported early successes, ACL injury rates and the associated gender disparity have not diminished. One problem could be the difficulties with the measurements of injury rates and the difficulties with the implementation of thorough large scale injury prevention programs. A second issue could be the transition from conscious awareness during training sessions on technique in the laboratory to unexpected and automatic movements during a training or game involves complicated motor control adaptations. The purpose of this paper is to highlight the issue of motor learning in relation to ACL injury prevention and to post suggestions for future research. ACL injury prevention programs addressing explicit rules regarding desired landing positions by emphasizing proper alignment of the hip, knee, and ankle are reported in the literature. This may very well be a sensible way, but the use of explicit strategies may be less suitable for the acquisition of the control of complex motor skills (Maxwell et al.     Introduction: It has been proposed that individual genetic variation contributes to the course of severe infections and sepsis. Recent studies of single nucleotide polymorphisms (SNPs) within the endotoxin receptor and its signaling system showed an association with the risk of disease development. This study aims to examine the response associated with genetic variations of TLR4, the receptor for bacterial LPS, and a central intracellular signal transducer (TIRAP/Mal) on cytokine release and for susceptibility and course of severe hospital acquired infections in distinct patient populations. Methods: Three intensive care units in tertiary care university hospitals in Greece and Germany participated. 375 and 415 postoperative patients and 159 patients with ventilator associated pneumonia (VAP) were included. TLR4 and TIRAP/Mal polymorphisms in 375 general surgical patients were associated with risk of infection, clinical course and outcome. In two prospective studies, 415 patients following cardiac surgery and 159 patients with newly diagnosed VAP predominantly caused by Gram-negative bacteria were studied for cytokine levels in-vivo and after ex-vivo monocyte stimulation and clinical course. Results: Patients simultaneously carrying polymorphisms in TIRAP/Mal and TLR4 and patients homozygous for the TIRAP/Mal SNP had a significantly higher risk of severe infections after surgery (odds ratio (OR) 5.5; confidence interval (CI): 1.34 - 22.64; P = 0.02 and OR: 7.3; CI: 1.89 - 28.50; P < 0.01 respectively). Additionally we found significantly lower circulating cytokine levels in double-mutant individuals with ventilator associated pneumonia and reduced cytokine production in an ex-vivo monocyte stimulation assay, but this difference was not apparent in TIRAP/Mal-homozygous patients. In cardiac surgery patients without infection, the cytokine release profiles were not changed when comparing different genotypes. Conclusions: Carriers of mutations in sequential components of the TLR signaling system may have an increased risk for severe infections. Patients with this genotype showed a decrease in cytokine release when infected which was not apparent in patients with sterile inflammation following cardiac surgery

    Early prognostic factors for disability in multiple sclerosis, a European multicenter study

    The effects of initial clinical variables on short-term prognosis are analyzed in a cross-sectional study of 574 multiple sclerosis patients from 7 centers in 5 European countries. Patients with a primary progressive course had a 2.3 higher mean disability score (EDSS) than the primary remittent group after a mean duration of disease of 6.6 years. High age at onset was associated with a primary progressive course, and was also related to increased risk of a rapid shift to a secondary progressive course. Among the remittent patients without a secondary progressive course a high age at onset was significantly correlated to a higher disability score. In the whole remittent group the presence of pyramidal and cerebellar symptoms at onset predicted both a high disability score and a rapid shift to a secondary progression, while the effect was reverse for sensory and visual symptoms. No difference between the sexes was found

    A subarcsecond near-infrared view of massive galaxies at z &gt; 1 with Gemini Multiconjugate Adaptive Optics

    We present images taken using the Gemini South Adaptive Optics Imager (GSAOI) with the Gemini Multiconjugate Adaptive Optics System (GeMS) in three 2 arcmin2^2 fields in the Spitzer Extragalactic Representative Volume Survey. These GeMS/GSAOI observations are among the first 0.1\approx 0.1^{''} resolution data in the near-infrared spanning extragalactic fields exceeding 1.51.5^{\prime} in size. We use these data to estimate galaxy sizes, obtaining results similar to those from studies with the Hubble Space Telescope, though we find a higher fraction of compact star forming galaxies at z>2. To disentangle the star-forming galaxies from active galactic nuclei (AGN), we use multiwavelength data from surveys in the optical and infrared, including far-infrared data from Herschel, as well as new radio continuum data from the Australia Telescope Compact Array and Very Large Array. We identify ultraluminous infrared galaxies (ULIRGs) at z13z \sim 1-3, which consist of a combination of pure starburst galaxies and Active Galactic Nuclei (AGN)/starburst composites. The ULIRGs show signs of recent merger activity, such as highly disturbed morphologies and include a rare candidate triple AGN. We find that AGN tend to reside in hosts with smaller scale sizes than purely star-forming galaxies of similar infrared luminosity. Our observations demonstrate the potential for MCAO to complement the deeper galaxy surveys to be made with the James Webb Space Telescope
