Situaciones y respuestas de ansiedad escolar más temidas en el alumnado chileno de educación secundaria
From the interactive-three-dimensional approach, school anxiety can be explained as a cognitive, psychophysiological or behavioral response, expressed in high pressure school situations such as school failure, school violence or academic and social evaluation. The purpose of this study was to analyze the most feared responses and school situations in a sample of Chilean Secondary Education students, according to gender. The sample was comprised of 4156 adolescents, 1950 boys and 2206 girls (46.9% and 53.1%, respectively) with ages between 13 and 18 years (M = 15.31, SD = 1.42). To evaluate the School Anxiety Inventory (IAES), which measures anxiety responses (cognitive, psychophysiological and behavioral), situations that generate anxiety (school failure and punishment, aggression, school and social evaluation) and a total score. The data were analyzed following a cross-sectional design and applying t tests to establish the differences and tests d to estimate the magnitude of said differences. The results indicate that girls present higher levels of school anxiety than boys in all the factors studied and in the total score, with higher scores in cognitive responses, especially in the item "I think I should have worked more", as well as also in situations related to school failure, mainly in the "if I repeat course" item. These results support those obtained in previous research and allow improving the understanding of situations that limit the normal academic, personal and social development of Chilean students.Desde el enfoque interactivo-tridimensional, la ansiedad escolar puede explicarse como una respuesta de tipo cognitiva, psicofisiológica o conductual, expresada ante situaciones escolares de gran presión como el fracaso escolar, la violencia escolar o la evaluación académica y social. El propósito de este estudio fue analizar las respuestas y situaciones escolares más temidas en una muestra de estudiantes chilenos de Educación Secundaria, en función del género. La muestra estuvo comprendida por 4156 adolescentes, 1950 chicos y 2206 chicas (46.9% y 53.1%, respectivamente) con edades entre los 13 y los 18 años (M = 15.31; DE = 1.42). Para evaluar se utilizó el Inventario de Ansiedad Escolar (IAES), que mide respuestas de ansiedad (cognitiva, psicofisiológica y conductual), situaciones que generan ansiedad (fracaso y castigo escolar, agresión, evaluación escolar y social) y una puntuación total. Los datos fueron analizados siguiendo un diseño de corte transversal y aplicando pruebas t para establecer las diferencias y pruebas d para estimar la magnitud de dichas diferencias. Los resultados indican que las chicas presentan mayores niveles de ansiedad escolar que los chicos en todos los factores estudiados y en la puntuación total, siendo superiores las puntuaciones en las respuestas cognitivas, especialmente en el ítem “creo que debería haber trabajado más”, así como también en las situaciones relacionadas con el fracaso escolar, principalmente, en el ítem “si repito curso”. Estos resultados apoyan los obtenidos en investigaciones previas y permiten mejorar la comprensión de las situaciones que limitan el normal desarrollo académico, personal y social del alumnado chileno

Doctor-patient care relationship in genetic cardiomyopathies: An exploratory study on clinical consultations
BackgroundThe present study aims to explore the setting of consultation and communication between physicians and patients affected by genetic cardiomyopathies, investigating how the two parts of the therapeutic relationship participate and share information.Methods and results45 adult patients affected by various cardiomyopathies took part in a prospective case study while attending consultations at a cardiologic outpatient clinic constituting an Italian referral centre for cardiomyopathies. A researcher observed the consultations, which were audio-recorded and transcribed. Transcripts were coded and an analysis of setting, type of communication implemented and participation of doctors and patients in terms of word-count and type of questions/answers was carried out. Overall word-count was significantly higher for physicians than for patients (t(44) = 9,506; pConclusionsFindings emphasize patients' need for adequate time and space to share their subjective illness experience with the physician, within an approach informed by the insights and recommendations of Narrative Medicine. These findings are instrumental to improving the specific clinical setting for individuals with genetic cardiomyopathies
The role of the genetic counsellor: a systematic review of research evidence
In Europe, genetic counsellors are employed in specialist genetic centres or other specialist units. According to the European Board of Medical Genetics, the genetic counsellor must fulfil a range of roles, including provision of information and facilitation of psychosocial adjustment of the client to their genetic status and situation. To evaluate the extent to which genetic counsellors fulfil their prescribed roles, we conducted a systematic review of the published relevant scientific evidence. We searched five relevant electronic databases (Medline, CINAHL, SocIndex, AMED and PsychInfo) using relevant search terms and handsearched four subject-specific journals for research-based papers published in English between 1 January 2000 and 30 June 2013. Of 419 potential papers identified initially, seven satisfied the inclusion criteria for the review. Themes derived from the thematic analysis of the data were: (i) rationale for genetic counsellors to provide care, (ii) appropriate roles and responsibilities and (iii) the types of conditions included in the genetic counsellor caseload. The findings of this systematic review indicate that where genetic counsellors are utilised in specialist genetic settings, they undertake a significant workload associated with direct patient care and this appears to be acceptable to patients. With the burden on genetic services, there is an argument for the increased use of genetic counsellors in countries where they are under-utilised. In addition, roles undertaken by genetic counsellors in specialist genetic settings could be adapted to integrate genetic counsellors into multi-disciplinary teams in other specialisms
Polymorphisms in genes involved in estrogen and progesterone metabolism and mammographic density changes in women randomized to postmenopausal hormone therapy: results from a pilot study
INTRODUCTION: Mammographic density is a strong independent risk factor for breast cancer, and can be modified by hormonal exposures. Identifying genetic variants that determine increases in mammographic density in hormone users may be important in understanding hormonal carcinogenesis of the breast. METHODS: We obtained mammograms and DNA from 232 postmenopausal women aged 45 to 75 years who had participated in one of two randomized, double-blind clinical trials with estrogen therapy (104 women, taking 1 mg/day of micronized 17β-estradiol, E2), combined estrogen and progestin therapy (34 women, taking 17β-estradiol and 5 mg/day of medroxyprogesterone acetate for 12 days/month) or matching placebos (94 women). Mammographic percentage density (MPD) was measured on baseline and 12-month mammograms with a validated computer-assisted method. We evaluated polymorphisms in genes involved in estrogen metabolism (catechol-O-methyltransferase (COMT (Val158Met)), cytochrome P450 1B1 (CYP1B1 (Val432Leu)), UDP-glucuronosyltransferase 1A1 (UGT1A1 (<7/≥ 7 TA repeats))) and progesterone metabolism (aldo-keto reductase 1C4 (AKR1C4 (Leu311Val))) with changes in MPD. RESULTS: The adjusted mean change in MPD was +4.6% in the estrogen therapy arm and +7.2% in the combined estrogen and progestin therapy arm, compared with +0.02% in the placebo arm (P = 0.0001). None of the genetic variants predicted mammographic density changes in women using estrogen therapy. Both the AKR1C4 and the CYP1B1 polymorphisms predicted mammographic density change in the combined estrogen and progestin therapy group (P < 0.05). In particular, the eight women carrying one or two low-activity AKR1C4 Val alleles showed a significantly greater increase in MPD (16.7% and 29.3%) than women homozygous for the Leu allele (4.0%). CONCLUSION: Although based on small numbers, these findings suggest that the magnitude of the increase in mammographic density in women using combined estrogen and progestin therapy may be greater in those with genetically determined lower activity of enzymes that metabolize estrogen and progesterone
The evolution of the North Atlantic Oscillation for the last 700 years inferred from D/H isotopes in the sedimentary record of Lake Azul (Azores archipelago, Portugal).
TheD plant leaf wax variations provide insights on precipitation and evaporation evolution through time. This proxy has been used to reconstruct the temporal evolution of the North Atlantic Oscillation (NAO) climate mode since this mode rules most of the climate variability in the central North Atlantic area. A total lipid extraction preparation and the correspondent analyses in the IRMS have been done for 100 samples from the uppermost 1.5 m of the sedimentary infill of Lake Azul (Azores archipelago, Portugal). According to the chronological model, established by 210Pb profile and 4 AMS 14C dates, this record contains the environmental history of the last 730 years. The reconstructed precipitation variations obtained from D/H isotope values, suggest that this area has suffered significant changes in its distribution and intensity rainfall patterns through time. The end of the Medieval Climate Anomaly (MCA, 1100- 1300 AD) is characterized by a progressive enrichmentof D/H isotope values which meant decreasing arid conditions. These rainfalls’ increase might be interpreted by a shift from positive to negative dominance of the NAO. The Little Ice Age (LIA, 1300 - 1850 AD) was characterized by two humid periods (1300- 1550 AD and 1650 - 1850 AD) separated by a relatively dry period. These precipitation oscillations are clearly visible by marked changes in the D/H isotope values. The LIA was followed by the persistence of the positive NAO mode, exhibited by the depletion of the D/H isotope signal, which indicated an overall decrease of the precipitation in the central North Atlantic area. Surprisingly, the D/H of the last 100 years, characterized by the present global warming and a persistent positive NAO mode, display large fluctuations most possibly linked to an enhancement of the storminess which is in concordance with the data fluctuations observed in the instrumental record for the last 80 years in the archipelago. This climatic evolution is in accordance with other NAO records
of the North Atlantic region (Trouet et al., 2012) highlighting the validity of the D/H isotopes as precipitation proxy.Peer Reviewe
