    Constrained Statistical Modelling of Knee Flexion from Multi-Pose Magnetic Resonance Imaging

    © 1982-2012 IEEE.Reconstruction of the anterior cruciate ligament (ACL) through arthroscopy is one of the most common procedures in orthopaedics. It requires accurate alignment and drilling of the tibial and femoral tunnels through which the ligament graft is attached. Although commercial computer-Assisted navigation systems exist to guide the placement of these tunnels, most of them are limited to a fixed pose without due consideration of dynamic factors involved in different knee flexion angles. This paper presents a new model for intraoperative guidance of arthroscopic ACL reconstruction with reduced error particularly in the ligament attachment area. The method uses 3D preoperative data at different flexion angles to build a subject-specific statistical model of knee pose. To circumvent the problem of limited training samples and ensure physically meaningful pose instantiation, homogeneous transformations between different poses and local-deformation finite element modelling are used to enlarge the training set. Subsequently, an anatomical geodesic flexion analysis is performed to extract the subject-specific flexion characteristics. The advantages of the method were also tested by detailed comparison to standard Principal Component Analysis (PCA), nonlinear PCA without training set enlargement, and other state-of-The-Art articulated joint modelling methods.     Length frequency, length -weight relationship and gonad development status of silver pomfret, Pampus argenteus, in Khouzestan (Iran) and Kuwait coastal waters, Persian Gulf

    Data used in this publication is a part of a comprehensive study was carried out jointly between South Aquaculture Research Center (SIARC) and Kuwait Institute for Scientific Research (KISR) on silver pomfret (Pampus argenteus) in 2003 to 2005. The study was aimed to collect efficient data from two regions from biological and population parameters of this economically important fish to properly explain its stock situation and ultimately leads to improve a better management program for conservation of stock and yield sustainability. Monthly data collection on length and biology, started since May 2003 and ended by December 2005. Shrimp trawl and gill nets were used for sample collection from the predicted areas in both sea regions. FL of the fish was within the range of 12-34cm, with the dominant length of 18-25cm in Kuwaiti waters. In Khouzestan waters fork length was ranged 14-22cm for the years of 2003 and 2004 but 14-26cm for 2005. Strong relationship between length and weight in both countries with a scanty difference was observed for this species (R2 =0.989 for Khouzestn, R^2=0.947 for Kuwait). The findings of present study are suggesting that silver pomfret is following an allometric growth pattern in studied area. Spawning of silver pomfert begin in July and continue till October in Kuwaiti waters but in Khouzestan waters the heights spawning of this fish started in end of May till October. Length at first maturity of this species in the studied area of Kuwait was found to be 19.6cm FL based on Spearman-Karber method but 23.3cm (FL) based on the logistic model.     Scope actuation system for articulated laparoscopes

    Background: An articulated laparoscope comprises a rigid shaft with an articulated distal end to change the viewing direction. The articulation provides improved navigation of the operating field in confined spaces. Furthermore, incorporation of an actuation system tends to enhance the control of an articulated laparoscope. Methods: A preliminary prototype of a scope actuation system to maneuver an off-the-shelf articulated laparoscope (EndoCAMaleon by Karl Storz, Germany) was developed. A user study was conducted to evaluate this prototype for the surgical paradigm of video-assisted thoracic surgery. In the study, the subjects maneuvered an articulated scope under two modes of operation: (a) actuated mode where an operating surgeon maneuvers the scope using the developed prototype and (b) manual mode where a surgical assistant directly maneuvers the scope. The actuated mode was further assessed for multiple configurations based on the orientation of the articulated scope at the incision. Results: The data show the actuated mode scored better than the manual mode on all the measured performance parameters including (a) total duration to visualize a marked region, (a) duration for which scope focus shifts outside a predefined visualization region, and (c) number of times for which scope focus shifts outside a predefined visualization region. Among the different configurations tested using the actuated mode, no significant difference was observed. Conclusions: The proposed articulated scope actuation system facilitates better navigation of an operative field as compared to a human assistant. Secondly, irrespective of the orientation in which an articulated scope’s shaft is inserted through an incision, the proposed actuation system can navigate and visualize the operative field

    A generic scope actuation system for flexible endoscopes

    Background: A scope actuation system assists a surgeon in steering a scope for navigating an operative field during an interventional or diagnostic procedure. Each system is tailored for a specific surgical procedure. The development of a generic scope actuation system could assist various laparoscopic and endoscopic procedures. This has the potential to reduce the deployment and maintenance costs for a hospital, making it more accessible for clinical usage. Methods: A modular actuation system (for maneuvering rigid laparoscopes) was adapted to enable incorporation of flexible endoscopes. The design simplifies the installation and disassembly processes. User studies were conducted to assess the ability of the system to focus onto a diagnostic area, and to navigate during a simulated esophagogastroduodenoscopy procedure. During the studies, the endoscope was maneuvered with (robotic mode) and without (manual mode) the actuation system to navigate the endoscope’s focus on a predefined track. Results: Results show that the robotic mode performed better than the manual mode on all the measured performance parameters including (a) the total duration to traverse a track, (b) the percentage of time spent outside a track while traversing, and (c) the number of times the scope focus shifts outside the track. Additionally, robotic mode also reduced the perceived workload based on the NASA-TLX scale. Conclusions: The proposed scope actuation system enhances the maneuverability of flexible endoscopes. It also lays the groundwork for future development of modular and generic scope assistant systems that can be used in both laparoscopic and endoscopic procedures

    Ashghal Quality Assurance & Quality Control system - QSD Role in Monitoring & Reporting the Quality Performance in Ashghal Projects

    Ashghal has established a robust system of quality control and quality assurance for all projects constructed by Ashghal. Ashghal Departments are supervising a sophisticated system of quality control and quality assurance processes and procedures, implemented by the Consultants, Contractors, and Key Supply Chain Partners of Ashghal projects (e.g., Sub-Contractors, Key Suppliers, and Independent Laboratories). This paper illustrates how Ashghal Quality & Safety Department (QSD) monitors and reports the quality performance in Ashghal constructed projects to ensure continuous improvement and adherence to Ashghal quality strategy, policy mission, and targets. QSD is operating as Third Party, reporting directly to Ashghal President, and is monitoring and reporting the compliance of the industry to the quality requirements defined in Qatar Construction Specifications (QCS, 2014), Contracts, Professional Services Agreements, ISO 9001:2015 Standard and specific Ashghal requirements. Effective root cause analysis carried out by the auditing team has led to value-adding activities. QSD has established specific Key Performance Indicators (KPIs) for the management of non-conformities and corrective actions, to improve the performance and minimize the deviations and failures of compliance. QSD is conducting focused and system audits at Ashghal projects and the supply chain and is liaising with departments and individual projects as necessary, to agree on Lessons Learned and Actions to improve the quality of works. These activities include the establishment of consistent criteria of the qualifications and experience of the quality staff of Ashghal departments and Consultants' teams. QSD is making quarterly presentations to the Ashghal President regarding the performance of the Departments (including Contractors' and Consultants' performance) and of the Ashghal Project Managers. The established criteria and evaluation related to the quality are dynamic and reviewed/updated periodically to ensure the improvement of the Quality Performance of Ashghal Projects

    Molecular epidemiology and genotype distribution of Human Papillomavirus (HPV) among Arab women in the state of Qatar

    Background: Human Papilloma Virus (HPV) infection is the major cause of cervical cancer worldwide. With limited data available on HPV prevalence in the Arab countries, this study aimed to identify the prevalence and genotypic distribution of HPV in the State of Qatar. Methods: 3008 cervical samples, exclusively of women with Arabic origin residing in Qatar were collected from the Women’s Hospital and Primary Health Care Corporation in Doha, State of Qatar. HPV DNA detection was done using GP5+/6+ primers based real time-polymerase chain reaction (RT-PCR) assay followed by the usage of HPV type specific primers based RT- PCR reactions and Sanger sequencing for genotype identification. Results: Similar prevalence rates of HPV infection was identified in both Qatari and non-Qatari women at 6.2% and 5.9% respectively. HPV prevalence rate of 5.8% and 18.4% was identified in women with normal cytology and in women with abnormal cytology respectively. HPV 81, 11 and 16, in decreasing order were the most commonly identified genotypes. HPV 81 was the most frequent low-risk genotype among women with both normal (74.0%) and abnormal (33.3%) cytology. HPV 16 (4.6%) was identified as the predominant high-risk HPV genotype among women with normal cytology and HPV 16, HPV 18, and HPV 56 (22.2% each) were the most common identified high-risk genotypes in women with abnormal cytology Conclusions: The overall HPV prevalence in Arab women in Qatar was identified as 6.1% with an increased HPV prevalence seen in women with abnormal cytology results and no significant trends seen with age. In contrast to Western countries, we report a varied genotypic profile of HPV with a high prevalence of low-risk HPV genotype 81 among the Arab women residing in Qatar.Weill Cornell Medical College in Qatar; and by a grant from the Qatar National Research Fund (NPRP- 09-344-3-082)

    Internet-based search of randomised trials relevant to mental health originating in the Arab world

    BACKGROUND: The internet is becoming a widely used source of accessing medical research through various on-line databases. This instant access to information is of benefit to busy clinicians and service users around the world. The population of the Arab World is comparable to that of the United States, yet it is widely believed to have a greatly contrasting output of randomised controlled trials related to mental health. This study was designed to investigate the existence of such research in the Arab World and also to investigate the availability of this research on-line. METHODS: Survey of findings from three internet-based potential sources of randomised trials originating from the Arab world and relevant to mental health care. RESULTS: A manual search of an Arabic online current contents service identified 3 studies, MEDLINE, EMBASE, and PsycINFO searches identified only 1 study, and a manual search of a specifically indexed, study-based mental health database, PsiTri, revealed 27 trials. CONCLUSION: There genuinely seem to be few trials from the Arab world and accessing these on-line was problematic. Replication of some studies that guide psychiatric/psychological practice in the Arab world would seem prudent

    Tooth brushing, tongue cleaning and snacking behaviour of dental technology and therapist students

    Objective: To determine the tooth brushing, tongue cleaning and snacking behaviour of dental technology and therapist students. Methods: A descriptive cross-sectional study of students of Federal School of Dental Therapy and Technology Enugu, Nigeria. Self-administered questionnaire was used to obtain information on demography, frequency, duration and technique of tooth brushing and tongue cleaning as well as information on consumption of snacks. Results: A total of 242 students responded. Dental technology students made up 52.5% of the respondents and dental therapist in training made up 47.5%. Majority (63.2%) of the respondents considered the strength of tooth brush when purchasing a tooth brush and 78.9% use tooth brushes with medium strength. Seven-tenth (71.9%) of the respondents brush their teeth twice daily and 52.1% brush for 3–5 minutes. About one-third (30.2%) brush their teeth in front of a mirror. Chewing stick was used by 51.7% of respondents in addition to the use of tooth brush. Tongue cleaning was done by 94.2% with only 9.5% using a tongue cleaner. Only 20.2% reported regular snacks consumption. Nine-tenth (90.4%) of respondents were previously involved in educating others, apart from their colleagues, on tooth brushing. Conclusion: This survey revealed that most of the dental therapy and technology students had satisfactory tooth-brushing behaviour. The zeal to educate others about proper tooth brushing revealed in this study suggests that the students may be helpful in oral health promotion

    Prospects for Improving the Intrinsic and Extrinsic Properties of Magnesium Diboride Superconducting Strands

    The magnetic and transport properties of magnesium diboride films represent performance goals yet to be attained by powder-processed bulk samples and conductors. Such performance limits are still out of the reach of even the best magnesium diboride magnet wire. In discussing the present status and prospects for improving the performance of powder-based wire we focus attention on (1) the intrinsic (intragrain) superconducting properties of magnesium diboride, Hc2 and flux pinning, (2) factors that control the efficiency with which current is transported from grain-to-grain in the conductor, an extrinsic (intergrain) property. With regard to Item-(1), the role of dopants in Hc2 enhancement is discussed and examples presented. On the other hand their roles in increasing Jc, both via Hc2 enhancement as well as direct fluxoid/pining-center interaction, are discussed and a comprehensive survey of Hc2 dopants and flux-pinning additives is presented. Current transport through the powder-processed wire (an extrinsic property) is partially blocked by the inherent granularity of the material itself and the chemical or other properties of the intergrain surfaces. These and other such results indicate that in many cases less than 15% of the conductor's cross sectional area is able to carry transport current. It is pointed out that densification in association with the elimination of grain-boundary blocking phases would yield five-to ten-fold increases in Jc in relevant regimes, enabling the performance of magnesium diboride in selected applications to compete with that of Nb-Sn
