Comparative bactericidal activities of daptomycin, glycopeptides, linezolid and tigecycline against blood isolates of Gram-positive bacteria in Taiwan
ABSTRACTIn-vitro MICs and minimum bactericidal concentrations (MBCs) of daptomycin, linezolid, tigecycline, vancomycin and teicoplanin against Gram-positive bacteria were determined using the broth microdilution method for ten blood isolates each of methicillin-susceptible Staphylococcus aureus (MSSA), methicillin-resistant S. aureus (MRSA), including two vancomycin-intermediate S. aureus (VISA), vancomycin-resistant Enterococcus faecium and Enterococcus faecalis. One strain of VISA was tested in a time-kill synergism assay of daptomycin combined with oxacillin, imipenem, rifampicin and isepamicin. Daptomycin showed excellent in-vitro bactericidal activity against all the isolates tested, with no tolerance or synergism effects when combined with other agents, except with rifampicin against VISA. Vancomycin had better bactericidal activity against MRSA and MSSA than did teicoplanin. Linezolid had the poorest bactericidal activity against the isolates tested, with 100% tolerance by the MSSA and VRE isolates, and 80% tolerance by the MRSA isolates. Tolerance towards tigecycline was exhibited by 40% of the MRSA isolates, 100% of the MSSA and vancomycin-resistant E. faecalis isolates, and 90% of the vancomycin-resistant E. faecium isolates

Bacteraemia caused by Weissella confusa at a university hospital in Taiwan, 1997â2007
AbstractHuman infections caused by Weissella confusa are rarely reported. Ten patients with bacteraemia caused by W. confusa who were treated at a tertiary-care hospital in Taiwan during 1997â2007 were studied. All isolates were initially misidentified as various Lactobacillus and Leuconostoc species by two commercial automated identification methods, and were confirmed to be W. confusa by 16S rRNA sequencing analysis. MICs of these isolates for ten antimicrobial agents were determined by the agar dilution method. The characteristics of these patients included underlying malignancy (n = 4), presence of a central catheter (n = 6), surgery within the previous 3 months (n = 4) and concomitant polymicrobial bacteraemia (n = 5, 50%). Mortality was directly attributed to bacteraemia in two patients. All isolates exhibited high trimethoprimâsulphamethoxazole and ceftazidime MICs (â„128 mg/L) and were inhibited by linezolid, daptomycin, ceftobiprole and tigecycline at 4, 0.12, 2 and 0.12 mg/L, respectively. In conclusion, W. confusa should be included in the list of organisms causing bacteraemia in immunocompromised hosts. Melioidosis: an emerging infection in Taiwan?
From January 1982 to May 2000, 17 infections caused by Burkholderia pseudomallei were diagnosed in 15 patients in Taiwan; almost all the infections were diagnosed from 1994 to May 2000. Of the 15 patients, 9 (60%) had underlying diseases, and 10 (67%) had bacteremic pneumonia. Thirteen (76%) episodes of infection were considered indigenous. Four patients died of melioidosis. Seventeen B. pseudomallei isolates, recovered from eight patients from November 1996 to May 2000, were analyzed to determine their in vitro susceptibilities to 14 antimicrobial agents, cellular fatty acid and biochemical reaction profiles, and randomly amplified polymorphic DNA patterns. Eight strains (highly related isolates) were identified. All isolates were arabinose non-assimilators and were susceptible to amoxicillin-clavulanate, piperacillin-tazobactam, imipenem, and meropenem. No spread of the strain was documented
A sensitising tool for smart home designers : based on user-oriented product design research into the home life of older adults in the UK
Focusing on the needs of users, design can leverage new product development process by offering insightful knowledge of those needs. This research investigates the technology development of smart homes. Design is utilised as a product research tool to identify key insights of the home life of the older adults living in the UK, and for the purpose of informing the front-end of the new product development process. The review of the literature in the field of smart homes suggests that the developments have lagged by a technology-push approach, the lack of appropriate concepts from usersâ perspectives as well as the lack of development strategy, which has consequently been reflected in consumersâ reluctance towards smart homes. As a result, this doctoral research aimed to âdevelop a user-oriented product design research tool that improves the understanding of the home life of older adults.â To achieve the aim, this research employs qualitative methodology to develop a research process that utilises the cultural probe, semi-structured interview and video tour. Informed by ethnographic tradition, this research establishes its trustworthiness and credibility by employing a thorough process of analysis (qualitative analysis with computer-assisted software NVivo 8 and peers debriefing) and evaluation (creative workshop and evaluative interview) with practitioners from the field of product design, design management and design education. The result of the field investigation is presented as ten personas and taxonomy of nodes, which form the contribution of this research, a sensitising tool and process. This research contributes a sensitising tool - a design-led, user-inspired and participatory product design research that the offers insightful knowledge of those older adults and their relationships with their homes living in the UK. This sensitising tool is developed for the smart home designers for the purpose of generating new product ideas and challenges designersâ preconception of users and smart homes, and provokes reflections on the practices of user-centred and user-participatory design, as examined in the creative workshop. The Gas Transfer through Polar Sea Ice Experiment: Insights into the Rates and Pathways that Determine Geochemical Fluxes
Sea ice is a defining feature of the polar marine environment. It is a critical domain for marine biota and it regulates ocean-atmosphere exchange, including the exchange of greenhouse gases such as CO2 and CH4. In this study, we determined the rates and pathways that govern gas transport through a mixed sea ice cover. N2O, SF6, 3He, 4He, and Ne were used as gas tracers of the exchange processes that take place at the ice-water and air-water interfaces in a laboratory sea ice experiment. Observation of the changes in gas concentrations during freezing revealed that He is indeed more soluble in ice than in water; Ne is less soluble in ice, and the larger gases (N2O and SF6) are mostly excluded during the freezing process. Model estimates of gas diffusion through ice were calibrated using measurements of bulk gas content in ice cores, yielding gas transfer velocity through ice (kice) of âŒ5 Ă 10â4 m dâ1. In comparison, the effective air-sea gas transfer velocities (keff) ranged up to 0.33 m dâ1 providing further evidence that very little mixed-layer ventilation takes place via gas diffusion through columnar sea ice. However, this ventilation is distinct from air-ice gas fluxes driven by sea ice biogeochemistry. The magnitude of keff showed a clear increasing trend with wind speed and current velocity beneath the ice, as well as the combination of the two. This result indicates that gas transfer cannot be uniquely predicted by wind speed alone in the presence of sea ice
The non-Verbal Structure of Patient Case Discussions in Multidisciplinary Medical Team Meetings
Meeting analysis has a long theoretical tradition in social psychology, with established practical rami?cations in computer science, especially in computer supported cooperative work. More recently, a good deal of research has focused on the issues of indexing and browsing multimedia records of meetings. Most research in this area, however, is still based on data collected in laboratories, under somewhat arti?cial conditions. This paper presents an analysis of the discourse structure and spontaneous interactions at real-life multidisciplinary medical team meetings held as part of the work routine in a major hospital. It is hypothesised that the conversational structure of these meetings, as indicated by sequencing and duration of vocalisations, enables segmentation into individual patient case discussions. The task of segmenting audio-visual records of multidisciplinary medical team meetings is described as a topic segmentation task, and a method for automatic segmentation is proposed. An empirical evaluation based on hand labelled data is presented which determines the optimal length of vocalisation sequences for segmentation, and establishes the competitiveness of the method with approaches based on more complex knowledge sources. Impact of measurement backaction on nuclear spin qubits in silicon
Phosphorus donor nuclear spins in silicon couple weakly to the environment
making them promising candidates for high-fidelity qubits. The state of a donor
nuclear spin qubit can be manipulated and read out using its hyperfine
interaction with the electron confined by the donor potential. Here we use a
master equation-based approach to investigate how the backaction from this
electron-mediated measurement affects the lifetimes of single and multi-donor
qubits. We analyze this process as a function of electric and magnetic fields,
and hyperfine interaction strength. Apart from single nuclear spin flips, we
identify an additional measurement-related mechanism, the nuclear spin
flip-flop, which is specific to multi-donor qubits. Although this flip-flop
mechanism reduces qubit lifetimes, we show that it can be effectively
suppressed by the hyperfine Stark shift. We show that using atomic precision
donor placement and engineered Stark shift, we can minimize the measurement
backaction in multi-donor qubits, achieving larger nuclear spin lifetimes than
single donor qubits
