Surgery for portal hypertension in children: A 12-year review
Background. Portal hypertension is a common and potentially devastating condition in children. Notwithstanding advances in the nonsurgical management of portal hypertension, surgery remains an important treatment modality in select patients. We report here on our experience in the past 12 years.Objectives. To describe the profile of, indication for, and complications of shunt surgery in children with portal hypertension.Methods. Twelve children underwent shunt surgery between 2005 and 2017. Patient records were reviewed.Results. Fourteen procedures were performed on 12 patients during the study period. The median age at surgery was 6.5 (range 1 - 18) years. Six patients were male. Gastrointestinal bleeding that was not amenable to endoscopic control was the most common indication for surgery. Portal vein thrombosis was the most common cause of portal hypertension in our series (n=11). Two-thirds (8/12) of all patients had an identifiable underlying risk factor for portal vein thrombosis. One-third of all patients (4/12) underwent a meso-portal bypass procedure (Rex shunt), while 58% (7/12) were managed with a distal splenorenal shunt. All patients received postoperative thromboprophylaxis. We experienced a single mortality, 1 patient experienced shunt thrombosis that required revision shunt surgery, and 2 patients experienced anastomotic strictures, with one being managed with revision surgery and the other currently awaiting radiological venoplasty.Conclusions. Surgery is a safe and important tool in the management of children with non-cirrhotic portal hypertension and those with sufficient hepatic reserve who fail to respond to more conservative methods for the treatment of side effects of portal hypertension
Prospective individual patient data meta-analysis of two randomized trials on convalescent plasma for COVID-19 outpatients
Data on convalescent plasma (CP) treatment in COVID-19 outpatients are scarce. We aimed to assess whether CP administered during the first week of symptoms reduced the disease progression or risk of hospitalization of outpatients. Two multicenter, double-blind randomized trials (NCT04621123, NCT04589949) were merged with data pooling starting when = 50 years and symptomatic for <= 7days were included. The intervention consisted of 200-300mL of CP with a predefined minimum level of antibodies. Primary endpoints were a 5-point disease severity scale and a composite of hospitalization or death by 28 days. Amongst the 797 patients included, 390 received CP and 392 placebo; they had a median age of 58 years, 1 comorbidity, 5 days symptoms and 93% had negative IgG antibody-test. Seventy-four patients were hospitalized, 6 required mechanical ventilation and 3 died. The odds ratio (OR) of CP for improved disease severity scale was 0.936 (credible interval (CI) 0.667-1.311); OR for hospitalization or death was 0.919 (CI 0.592-1.416). CP effect on hospital admission or death was largest in patients with <= 5 days of symptoms (OR 0.658, 95%CI 0.394-1.085). CP did not decrease the time to full symptom resolution

Acquired haemophilia A in a patient with breast cancer and lung carcinoma: a case report and literature review
Acquired haemophilia A is a rare disorder caused by spontaneous formation of auto-antibodies (inhibitors) against coagulation factor VIII. This can lead to life-threatening haemorrhages. Six to twenty-two percent of patients with acquired haemophilia have an underlying malignancy. We describe a 69-year-old woman with metastatic breast cancer and non-small cell lung carcinoma who presented at the emergency room with spontaneous bruising, and who was using a vitamin K antagonist. She had a prolonged activated partial thromboplastin time (aPTT) due to a coagulation factor VIII deficiency caused by factor VIII antibodies. Increasing surgical safety through serious gaming: "Nobody is perfect, but a team can be."
Safety is one of the major issues for surgical teams. Game technology could be a means to increase dialogue and raise awareness on safety within and between surgery teams. A participatory design process, where end-users are involved in the development process of such a "safety-game', can foster safety awareness and acceptance of the game as intervention. Involving the target group at an early stage also enables game designers to find a balance between the dimensions of reality, meaning and play of the game. In this paper we present the first steps of a participatory design approach for 3D-serious games that aim to stimulate learning and development of team oriented working in surgical teams. Using a serious game to motivate or educate is not new, but consciously using the design phase of such a game to stimulate both motivation and learning is. In this paper we report on the first phase of the project, the design phase and the first user-tests. The findings lead us to the tentative conclusion that learning, motivation and the intention to alter behaviour that should lead to less human induced errors or casualties are positively influenced
