    El efecto del Covid-19 en la percepción del bienestar del individuo y la transición hacia la organización digital

    La pandemia del Covid-19 y el proceso de distanciamiento social que se han vivido en el año 2020 han traído diversos impactos en la percepción del bienestar de las personas y en la adaptación al trabajo remoto. Este fenómeno ha conducido a reflexionar en el diseño de estrategias en el corto y mediano plazo para atender las implicaciones de bienestar, tanto a nivel individual como organizacional, tanto en las personas que adaptaron su trabajo a una modalidad remota, como para aquellas que no pudieron adaptar sus labores y perdieron sus trabajos. En este estudio se encuestaron a 587 personas, quienes describen los cambios de comportamiento, economía familiar, hábitos y dinámicas derivados del proceso de resguardo en casa en los hogares de las familias mexicanas, mayormente del estado de Guanajuato, así como las adecuaciones en la adaptación al trabajo remoto. Los resultados señalan las afectaciones emocionales, físicas y económicas para aquellos que adaptaron sus condiciones a trabajos remotos, pero también dan la pauta para describir como la percepción de bienestar fue afectada en esta etapa. Para los trabajadores remotos, estos resultados dan pie al diseño de un modelo de transición de un liderazgo exclusivamente pensado en un entorno presencial hacia un liderazgo humano-tecnológico orientado en promover el bienestar del individuo, que de pauta a un mayor autocuidado del bienestar físico y mental por parte del líder en sus colaboradores y que éste sea un eje fundamental en el desarrollo de la productividad y desempeño de sus colaboradores en la organización netamente digital.&nbsp

    Resumen En los últimos años, algunas sustancias preservantes de madera han sido objeto de restricciones en cuanto a sus aplicaciones debido a problemas de seguridad y toxicidad. Por esta razón, es necesario profundizar en el conocimiento de nuevas sales que aporten ventajas respecto a los ya existentes.     Bronchopulmonary Lophomoniasis, Infection by Endocommensal Protozoa of Intradomiciliary Cockroaches: Presentation of a Case in an Immunocompromised Patient from Querétaro, Mexico

    Infection in humans by the intestinal protozoan of cockroaches and termites called Lophomonas blattarum has been diagnosed in respiratory infections of children aged 2–5 years contaminated orally or by air, with cysts or trophozoites contained in the feces of the cockroach Periplaneta americana. In respiratory infections of adults, it is difficult to diagnose since the cyst or trophozoite is not recognized as a human pathogen and is only related to immunosuppressed patients, transplant patients with severe lung disease and those living in poor and unhealthy sanitary conditions. Normally, its presence is manifested with fevers of 38–39°C, cough with thick expectoration, respiratory insufficiency and pulmonary abscesses. The laboratory diagnosis is mainly based on bronchoscopic cytologies and bronchoalveolar lavage biopsies. The case in question is about a 60-year-old male. Single, he lives alone, with a diagnosis of 9 baths behind non-Hodgkin lymphoma, undergoing treatment with radiotherapy and chemotherapy. For edema after treatment, thoracentesis and pericardiocentesis were performed, as well as gastrostomy, which he maintained for 1 year. He started with throat discomfort, followed by production of productive cough without blood, general weakness, and difficulty breathing, with apparent diagnosis of possible respiratory failure due to mycobacteria. It was possible to visualize the protozoan, in fresh preparations of bronchial aspirate and expectoration in wet assembly with saline solution and stained with Pap smears, Harris Hematoxylin and Eosin (H/E), and Giemsa

    El presente proyecto surge de la necesidad de dar continuidad a un trabajo que ya se había iniciado en el año de 1992, el cual ha tenido como objetivos centrales: - Contribuir al cambio social construyendo con el nuevo maestro su papel frente al saber pedagógico. - Reflexionar sobre la educación ambiental como planteamiento integrante e integrador. - Cualificar los procesos de lectura y escritura de los niños del Colegio Fe y Alegría El Limonar. - Establecer relaciones entre la lectura, la escritura y la educación ambiental Este proyecto se desarrolló básicamente en tres niveles; primero se desarrolló el diagnóstico pedagógico de la institución, luego se construyeron y se ejecutaron los proyectos de aula; por último se elaboraron las memorias del trabajo. El trabajo expuesto a continuación se convierte en herramienta de apoyo y en derrotero para futuros proyectos pedagógicos, donde a partir de la lectura y la escritura y la educación ambiental se generen nuevos procesos educativos que contribuyan a mejorar la calidad de la educación

    Adiponectin accounts for gender differences in hepatocellular carcinoma incidence

    Hepatocellular carcinoma (HCC) is the sixth most common cancer type and the fourth leading cause of cancer-related death. This cancer appears with higher incidence in men and during obesity; however, the specific mechanisms underlying this correlation are unknown. Adipose tissue, a key organ in metabolic syndrome, shows evident gender disparities in the production of adipokines. Levels of the important adipokine adiponectin decrease in men during puberty, as well as in the obese state. Here, we show that this decrease in adiponectin levels is responsible for the increased liver cancer risk in males. We found that testosterone activates the protein JNK in mouse and human adipocytes. JNK-mediated inhibition of adiponectin secretion increases liver cancer cell proliferation, since adiponectin protects against liver cancer development through the activation of AMP-activated protein kinase (AMPK) and p38α. This study provides insight into adipose tissue to liver crosstalk and its gender relation during cancer development, having the potential to guide strategies for new cancer therapeutics.G.     Binding Potassium to Improve Treatment With Renin-Angiotensin-Aldosterone System Inhibitors: Results From Multiple One-Stage Pairwise and Network Meta-Analyses of Clinical Trials

    This manuscript presents findings from the first dichotomous data pooling analysis on clinical trials (CT) regarding the effectiveness of binding potassium. The results emanated from pairwise and network meta-analyses aiming evaluation of response to commercial potassium-binding polymers, that is, to achieve and maintain normal serum potassium (n = 1,722), and the association between this response and an optimal dosing of renin-angiotensin-aldosterone system inhibitors (RAASi) needing individuals affected by heart failure (HF) or resistant hypertension, who may be consuming other hyperkalemia-inducing drugs (HKID) (e.g., b-blockers, heparin, etc.), and frequently are affected by chronic kidney disease (CKD) (n = 1,044): According to the surface under the cumulative ranking area (SUCRA), sodium zirconium cyclosilicate (SZC) (SUCRA >0.78), patiromer (SUCRA >0.58) and sodium polystyrene sulfonate (SPS) (SUCRA 5.1 mEq/L), and, when normokalemia is achieved, patiromer 16.8–25.2 g/day (SUCRA = 0.94) and patiromer 8.4–16.8 g/day (SUCRA = 0.41) can allow to increase the dose of spironolactone up to 50 mg/day in subjects affected by heart failure (HF) or with resistant hypertension needing treatment with other RAASi. The potential of zirconium cyclosilicate should be explored further, as no data exists to assess properly its capacity to optimize dosing of RAASi, contrarily as it occurs with patiromer. More research is also necessary to discern between benefits of binding potassium among all type of hyperkalemic patients, for example, patients with DM who may need treatment for proteinuria, patients with early hypertension, etc.Fil: Lizaraso Soto, Frank.     Prevalence of bleeding secondary to anticoagulation and mortality in patients with atrial fibrillation admitted with SARS-CoV-2 infection.

    Atrial fibrillation (AF) is common in patients admitted with severe COVID-19. However, there is limited data about the management of chronic anticoagulation therapy in these patients. We assessed the anticoagulation and incidence of major cardiovascular events in hospitalized patients with AF and COVID-19. We retrospectively investigated all consecutive patients with AF admitted with COVID-19 between March and May 2020 in 9 Spanish hospitals. We selected a control group of non-AF patients consecutively admitted with COVID-19. We compared baseline characteristics, incidence of major bleeding, thrombotic events and mortality. We used propensity score matching (PSM) to minimize potential confounding variables, as well as a multivariate analysis to predict major bleeding and death. 305 patients admitted with AF and COVID-19 were included. After PSM, 151 AF patients were matched with 151 control group patients. During admission, low-molecular-weight heparin was the principal anticoagulant and the incidence of major bleeding and mortality were higher in the AF group [16 (10.6%) vs 3 (2%), p=0.003; 52 (34.4%) vs 35 (23.2%), p=0.03, respectively]. The multivariate analysis showed the presence of AF as independent predictor of in-hospital major bleeding and mortality in COVID-19 patients. In AF group, a secondary multivariate analysis identified high levels of D-dimer as independent predictor of in-hospital major bleeding. AF patients admitted with COVID-19 represent a population at high risk for bleeding and mortality during admission. It seems advisable to individualize anticoagulation therapy during admission, considering patient specific bleeding and thrombotic risk.S
