    The second and updated edition of this new study of the genesis of the Indonesian national state is based on the notion that the birth of that nation grew out of not only the liberation movement but also from the Dutch rule that the nationalists agitated against. The book places a clear emphasis on the ways in which Dutch rule was established in the Indonesian archipelago in the course of three centuries and examines the developments of Dutch colonial policies. This feeds into chapters that focus on the Indonesian nationalist movement and the Japanese occupation of the colony in 1942-1945. The occupation helped to enable the proclamation of Indonesian indepence and the creation of the Republic in August 1945. The conflict that erupted between the Repub-lic and the Netherlands was brought to an incomplete ‘solution’ in 1949, but the dis-pute about West-Irian led to a sequel that lasted for another thirteen years. More than half of this book is dedicated to the conflict and its aftermath. Much attention is paid to the sentiments and ideas that informed Dutch policy. Various issues that have received scant attention in the historiography are now dis-cussed. The author based his study on Dutch and international literature, contemporary newspapers and policy documents, and his own memories. In the book’s title, the stork represents the Dutch and the garuda functions as a symbol of Indonesia. J. Herman Burgers (1926) worked at the Dutch Department of Foreign Affairs. He studied Law in Amsterdam and Political Science at Stanford University. He was deeply interested in the conflict between the Netherlands and the Republic of Indone-sia, as it broke out in 1945. This fascination has never left him, and he has continued to study the conflict and its aftermath, especially during the years 1948-1950 when he was in Indonesia for his Dutch military service

    The appropriateness of prescribing antibiotics in the community in Europe: study design

    Contains fulltext : 97417.pdf (publisher's version ) (Open Access)ABSTRACT: BACKGROUND: Over 90% of all antibiotics in Europe are prescribed in primary care. It is important that antibiotics are prescribed that are likely to be effective; however, information about antibiotic resistance in the community is incomplete. The aim of our study is to investigate the appropriateness of antibiotic prescribing in primary care in Europe by collecting and combining patterns of antibiotic resistance patterns and antibiotic prescription patterns in primary care. We will also evaluate the appropriateness of national antibiotic prescription guidelines in relation to resistance patterns. METHODS/DESIGN: Antibiotic resistance will be studied in an opportunistic sample from the community in nine European countries. Resistance data will be collected by taking a nose swab of persons (N = 4,000 per country) visiting a primary care practice for a non-infectious disease. Staphylococcus aureus and Streptococcus pneumoniae will be isolated and tested for resistance to a range of antibiotics in one central laboratory. Data on antibiotic prescriptions over the past 5 years will be extracted from the electronic medical records of General Practitioners (GPs). The results of the study will include the prevalence and resistance data of the two species and 5 years of antibiotic prescription data in nine European countries.The odds of receiving an effective antibiotic in each country will be calculated as a measure for the appropriateness of prescribing. Multilevel analysis will be used to assess the appropriateness of prescribing. Relevant treatment guidelines of the nine participating countries will be evaluated using a standardized instrument and related to the resistance patterns in that country. DISCUSSION: This study will provide valuable and unique data concerning resistance patterns and prescription behaviour in primary care in nine European countries. It will provide evidence-based recommendations for antibiotic treatment guidelines that take resistance patterns into account which will be useful for both clinicians and policy makers. By improving antibiotic use we can move towards controlling the resistance problem globally

    Gas-kinetic derivation of Navier-Stokes-like traffic equations

    Macroscopic traffic models have recently been severely criticized to base on lax analogies only and to have a number of deficiencies. Therefore, this paper shows how to construct a logically consistent fluid-dynamic traffic model from basic laws for the acceleration and interaction of vehicles. These considerations lead to the gas-kinetic traffic equation of Paveri-Fontana. Its stationary and spatially homogeneous solution implies equilibrium relations for the `fundamental diagram', the variance-density relation, and other quantities which are partly difficult to determine empirically. Paveri-Fontana's traffic equation allows the derivation of macroscopic moment equations which build a system of non-closed equations. This system can be closed by the well proved method of Chapman and Enskog which leads to Euler-like traffic equations in zeroth-order approximation and to Navier-Stokes-like traffic equations in first-order approximation. The latter are finally corrected for the finite space requirements of vehicles.     AN UPWARD TREND IN DNA P16INK4A METHYLATION PATTERN AND HIGH RISK HPV INFECTION ACCORDING TO THE SEVERITY OF THE CERVICAL LESION

    SUMMARY High-risk human papillomavirus (hr-HPV) infection is necessary but not sufficient for cervical cancer development. Recently, P16INK4A gene silencing through hypermethylation has been proposed as an important cofactor in cervical carcinogenesis due to its tumor suppressor function. We aimed to investigate P16INK4A methylation status in normal and neoplastic epithelia and evaluate an association with HPV infection and genotype. This cross-sectional study was performed with 141 cervical samples from patients attending Hospital Moncorvo Filho, Rio de Janeiro. HPV detection and genotyping were performed through PCR and P16INK4A methylation by nested-methylation specific PCR (MSP). HPV frequency was 62.4% (88/141). The most common HPV were HPV16 (37%), HPV18 (16.3%) and HPV33/45(15.2%). An upward trend was observed concerning P16INK4A methylation and lesion degree: normal epithelia (10.7%), low grade lesions (22.9%), high grade (57.1%) and carcinoma (93.1%) (p < 0.0001). A multivariate analysis was performed to evaluate an association between methylation, age, tobacco exposure, HPV infection and genotyping. A correlation was found concerning methylation with HPV infection (p < 0.0001), hr-HPV (p = 0.01), HSIL (p < 0.0007) and malignant lesions (p < 0.0001). Since viral infection and epigenetic alterations are related to cervical carcinoma, we suggest that P16INK4A methylation profile maybe thoroughly investigated as a biomarker to identify patients at risk of cancer
