    Graduate and Undergraduate Faculty Collaboration Utilizing Peer Observation to Enhance Educational Opportunities for Students and Faculty: A Case Example

    While evidence supports the use of cadavers to facilitate the teaching and learning of human anatomy, cadaver-based teaching may not be present at the undergraduate level at many institutions due to limited laboratory access, financial constraints, and the lack of qualified faculty trained to teach in this type of setting.  The following case example outlines a unique program designed to provide cadaver-based instruction to undergraduate students, while simultaneously training undergraduate faculty to teach in this setting through peer observational methods. More specifically, the following teaching collaboration was designed with the intent to achieve the following: 1) expose undergraduate anatomy students to cadaver-based learning; 2) provide education and training to undergraduate faculty so they are better qualified to teach human anatomy in the context of a cadaver laboratory; 3) provide graduate physical therapy students additional opportunities to dissect and reinforce their anatomy knowledge; and 4) demonstrate the value of interdisciplinary collaboration.  Eighty-one undergraduate students were exposed to the cadaver laboratory for four educational sessions throughout the semester. Course evaluations revealed that 93% of the undergraduate students reported that their experiences in the cadaver laboratory served to enhance their learning, and 97% reported that the use of human cadavers should be continued in future courses. This interdisciplinary model allowed anatomy instruction to be expanded to previously unserved student groups as well as provided a mechanism for professional development of undergraduate anatomy faculty. This model may serve as a template to promote new program development to enhance faculty and student learning, while simultaneously encouraging interdisciplinary collaboration across the university.     Incidence, risk factors, clinical presentation and treatment of ectopic pregnancy in the Limbe and Buea Regional Hospitals in Cameroon

    INTRODUCTION : ectopic pregnancy (EP) is the leading cause of maternal mortality in the first trimester of pregnancy in our environment. This study aimed at evaluating the incidence, risk factors, clinical presentation and treatment of ectopic pregnancy in the Limbe and Buea Regional Hospitals in Cameroon. METHODS : this was a retrospective nested case control study carried out from December 2006 to December 2016. A ratio for control vs cases of 3:1 was obtained. Any pregnancy implanted outside the normal uterine cavity was considered as an ectopic pregnancy. Student's t-test was used to compare continuous variables and Pearson's Chi-square test for categorical variables. The association between EP and the demographical and clinical variables was estimated using logistic regression. Statistical significance was set at p-values<0.05. RESULTS : a total of 247 cases of EP were registered out of 17221 deliveries giving an incidence of 1.43% in ten years. History of pelvic inflammatory disease (OR = 3.10, CI (1.76-5.44), p < 0.001), previous EP (OR = 10.22, CI (2.61-14.82), p < 0.001), History of induced abortion (OR = 2.68, CI (3.32-9.73), p< 0.001), history of adnexa surgery (OR = 4.37, CI (2.17-10.32), p < 0.001) and history of appendectomy (OR = 2.16, CI (0.99-6.64) p< 0.001), were also found to be associated with increased risk of EP. More than five percent (5.52%) of the patients were in shock at presentation. Diagnosis was confirmed mainly by use of ultrasound (78.53%) and treatment was principally by laparotomy (97.55%) with salpingectomy (95.60%). Most (90.18%) of ectopic pregnancies were ruptured at presentation. Only 2.45% of cases were manage medically with the use of methotrexate. CONCLUSION : the incidence of ectopic pregnancy (EP) in our environment is within the global range (hospital-based incidence of 1.43%) and is rising.     Ricin Toxin Activates the NALP3 Inflammasome

    Ricin exhibits well characterized ribotoxic actions that lead to the inhibition of protein synthesis and the phosphorylation of stress activated protein kinases (SAPKs). Proinflammatory effects of ricin are thought to be caused by upregulation of genes encoding proinflammatory transcripts as a result of the activation of c-Jun N-terminal kinase (JNK) and p38 MAPK. We reported previously that macrophages and interleukin-1β (IL-1β) signaling are required for murine host immune responses to ricin delivered to the lungs. Here we report that ricin-mediated IL-1β release from bone-marrow derived macrophages is dependent on the NALP3 inflammasome, a scaffolding complex that mediates pro-IL-1β cleavage to active IL-1β by caspase-1. Release of IL-1β from macrophages was suppressed by the reactive oxygen species (ROS) scavenger N-acetyl cysteine (NAC) and high extracellular K+, which are two agents known to inhibit NALP3/cryopyrin/CIAS1 inflammasome formation. By employing inhibitors of p38 MAPK and JNK, we demonstrated that ricin-mediated release of IL-1β was enhanced, rather than suppressed, by inhibition of SAPK phosphorylation. In contrast, proteasomal inhibitors bortezomib and MG-132 completely suppressed ricin-induced IL-1β release from macrophages. These data suggest that ricin-mediated translational inhibition itself, by fostering the disappearance of labile protein(s) that normally suppress inflammasome formation, may constitute the mechanism underlying IL-1-dependent inflammatory signaling by ricin

    Every regular Chinese syllable has a syllable tone (the tone we get when the syllable is read in isolation). In some Chinese languages, the tonal pattern of a multisyllabic expression is basically a concatenation of the syllable tones. In other Chinese languages, the tonal pattern of a multisyllabic expression is determined solely by the initial syllable. I call the former M -languages (represented by Mandarin) and the latter S -languages (represented by Shanghai). I argue that there is an additional difference in rime structures between the two language groups. In S-languages, all rimes are simple, i.e., there are no underlying diphthongs or codas. In M-languages, all regular rimes are heavy. I further argue that a syllable keeps its underlying tones only if it has stress. Independent metrical evidence tells us that heavy rimes may carry inherent stress. Thus, in M-languages, all regular syllables are stressed and retain their underlying tones (which may or may not undergo further changes).
