    Serum levels of matrix metalloproteinases-2 and-9 and their tissue inhibitors in inflammatory neuromuscular disorders

    We monitored serum levels of matrix metalloproteinases (MMPs) and their tissue inhibitors (TIMPs) before and during intravenously applied immunoglobulin (IVIG) therapy in 33 patients with chronic immune-mediated neuropathies and myopathies and 15 controls. Baseline MMP-2 and TIMP-2 serum levels were lower and MMP-9 and TIMP-1 serum levels higher in all patients compared to age-matched controls. Eight days after IVIG treatment, MMP-2, TIMP-2, and TIMP-1 serum levels increased, while MMP-9 serum levels decreased, indicating tissue repair. After 60 days, MMP-9 levels increased, MMP-2 approached normal levels, while TIMP-1 and TIMP-2 serum levels were below day 8 levels, indicating relapsing tissue damage. Comparing the MMP/TIMP results with the clinical courses, IVIG treatment tended to change MMP/TIMP levels in a way that paralleled clinical improvement and relapse.     Spatio-spectral control of coherent nanophotonics

    Fast modulation of optical signals that carry multidimensional information in the form of wavelength, phase or polarization has fueled an explosion of interest in integrated photonics. This interest however masks a significant challenge which is that independent modulation of multi-wavelength carrier signals in a single waveguide is not trivial. Such challenge is attributed to the longitudinal direction of guided-mode propagation, limiting the spatial separation and modulation of electric-field. Here, we overcome this using a single photonic element that utilizes active coherent (near) perfect absorption. We make use of standing wave patterns to exploit the spatial-degrees-of-freedom of in-plane modes and individually address elements according to their mode number. By combining the concept of coherent absorption in spatio-spectral domain with active phase-change nanoantennas, we engineer and test an integrated, reconfigurable and multi-spectral modulator operating within a single element. Our approach demonstrates for the first time, a non-volatile, wavelength-addressable element, providing a pathway for exploring the tunable capabilities in both spatial and spectral domains of coherent nanophotonics

    Model-based probabilistic frequent itemset mining

    Data uncertainty is inherent in emerging applications such as location-based services, sensor monitoring systems, and data integration. To handle a large amount of imprecise information, uncertain databases have been recently developed. In this paper, we study how to efficiently discover frequent itemsets from large uncertain databases, interpreted under the Possible World Semantics. This is technically challenging, since an uncertain database induces an exponential number of possible worlds. To tackle this problem, we propose a novel methods to capture the itemset mining process as a probability distribution function taking two models into account: the Poisson distribution and the normal distribution. These model-based approaches extract frequent itemsets with a high degree of accuracy and support large databases. We apply our techniques to improve the performance of the algorithms for (1) finding itemsets whose frequentness probabilities are larger than some threshold and (2) mining itemsets with the {Mathematical expression} highest frequentness probabilities. Our approaches support both tuple and attribute uncertainty models, which are commonly used to represent uncertain databases. Extensive evaluation on real and synthetic datasets shows that our methods are highly accurate and four orders of magnitudes faster than previous approaches. In further theoretical and experimental studies, we give an intuition which model-based approach fits best to different types of data sets. © 2012 The Author(s).published_or_final_versio

    Broadband on-chip polarization mode splitters in lithium niobate integrated adiabatic couplers

We report, to the best of our knowledge, the first broadband polarization mode splitter (PMS) based on the adiabatic light passage mechanism in the lithium niobate (LiNbO3) waveguide platform. A broad bandwidth of ~140 nm spanning telecom S, C, and L bands at polarization-extinction ratios (PER) of >20 dB and >18 dB for the TE and TM polarization modes, respectively, is found in a five-waveguide adiabatic coupler scheme whose structure is optimized by an adiabaticity engineering process in titanium-diffused LiNbO3 waveguides. When the five-waveguide PMS is integrated with a three-waveguide “shortcut to adiabaticity” structure, we realize a broadband, high splitting-ratio (ηc) mode splitter for spatial separation of TE- (H-) polarized pump (700-850 nm for ηc>99%), TM- (V-) polarized signal (1510-1630 nm for ηc>97%), and TE- (H-) polarized idler (1480-1650 nm for ηc>97%) modes. Such a unique integrated-optical device is of potential for facilitating the on-chip implementation of a pump-filtered, broadband tunable entangled quantum-state generator

    Reliable and valid NEWS for Chinese seniors: measuring perceived neighborhood attributes related to walking

    Background : The effects of the built environment on walking in seniors have not been studied in an Asian context. To examine these effects, valid and reliable measures are needed. The aim of this study was to develop and validate a questionnaire of perceived neighborhood characteristics related to walking appropriate for Chinese seniors (Neighborhood Environment Walkability Scale for Chinese Seniors, NEWS-CS). It was based on the Neighborhood Environment Walkability Scale - Abbreviated (NEWS-A), a validated measure of perceived built environment developed in the USA for adults. A secondary study aim was to establish the generalizability of the NEWS-A to an Asian high-density urban context and a different age group. Methods : A multidisciplinary panel of experts adapted the original NEWS-A to reflect the built environment of Hong Kong and needs of seniors. The translated instrument was pre-tested on a sample of 50 Chinese-speaking senior residents (65+ years). The final version of the NEWS-CS was interviewer-administered to 484 seniors residing in four selected Hong Kong districts varying in walkability and socio-economic status. Ninety-two participants completed the questionnaire on two separate occasions, 2-3 weeks apart. Test-rest reliability indices were estimated for each item and subscale of the NEWS-CS. Confirmatory factor analysis was used to develop the measurement model of the NEWS-CS and cross-validate that of the NEWS-A. Results : The final version of the NEWS-CS consisted of 14 subscales and four single items (76 items). Test-retest reliability was moderate to good (ICC > 50 or % agreement > 60) except for four items measuring distance to destinations. The originally-proposed measurement models of the NEWS-A and NEWS-CS required 2-3 theoretically-justifiable modifications to fit the data well. Conclusions : The NEWS-CS possesses sufficient levels of reliability and factorial validity to be used for measuring perceived neighborhood environment in Chinese seniors. Further work is needed to assess its construct validity and generalizability to other Asian locations. In general, the measurement model of the original NEWS-A was generalizable to this study context, supporting the feasibility of cross-country and age-group comparisons of the effect of the neighborhood environment on walking using the NEWS-A as a tool to measure the perceived built environment

    Patient safety in dentistry: development of a candidate 'never event' list for primary care

    Introduction The 'never event' concept is often used in secondary care and refers to an agreed list of patient safety incidents that 'should not happen if the necessary preventative measures are in place'. Such an intervention may raise awareness of patient safety issues and inform team learning and system improvements in primary care dentistry. Objective To identify and develop a candidate never event list for primary care dentistry. Methods A literature review, eight workshops with dental practitioners and a modified Delphi with 'expert' groups were used to identify and agree candidate never events. Results Two-hundred and fifty dental practitioners suggested 507 never events, reduced to 27 distinct possibilities grouped across seven themes. Most frequently occurring themes were: 'checking medical history and prescribing' (119, 23.5%) and 'infection control and decontamination' (71, 14%). 'Experts' endorsed nine candidate never event statements with one graded as 'extreme risk' (failure to check past medical history) and four as 'high risk' (for example, extracting wrong tooth). Conclusion Consensus on a preliminary list of never events was developed. This is the first known attempt to develop this approach and an important step in determining its value to patient safety. Further work is necessary to develop the utility of this method
