    Enabling self, intimacy and a sense of home in dementia

    Design and digital technologies to support a sense of self and human relationships for people living with dementia are both urgently needed. We present an enquiry into design for dementia facilitated by a public art commission for an adult mental health unit in a hospital in the UK. The interactive art piece was informed by the notion of personhood in dementia that foregrounds the person's social being and interpersonal relationships as sites where self is maintained and constructed. How clients, clients' family members and staff used the piece is reported and insights related to the notions of home, intimacy, possessions and self are presented. The art piece served as window on both dementia and the institution leading to a number of insights and implications for design

    The benthic regeneration of N and P in the Great Brak estuary, South Africa

    The Great Brak is a temporarily open/closed estuary (TOCE) located on the south coast of South Africa. The construction of the Wolwedans Dam in 1989 reduced baseflow to the estuary by 56%, decreasing the intensity of flushing events and causing the mouth to breach less often. The aim of this study was to  investigate the flux of inorganic nutrients (NH4 +, TOxN [NO3 - + NO2 -], SRP) as well as total N and P across the  sediment–water interface in the estuary. There have been very few studies on nutrient cycling and benthic  pelagic coupling in South African estuaries. This study showed that the sediment had a net efflux of NH4 +, SRP, TN and TP while TOxN was taken up or converted to other forms of N. The estuary acted as a source of N and P during both summer and winter. If the estuary  remains closed for a prolonged period (12 months), with an increased organic load present on the benthos, the associated rates of efflux of N and P would increase. In order to reduce the organic load to the system better flushing methods or, more importantly, an increase in base flow, is needed to reduce residence times of water in the estuary.Keywords: water quality, nutrient cycling, benthic-pelagic coupling, estuar

    Knowledge and attitudes towards HIV vaccines among Soweto adolescents

    <p>Abstract</p> <p>Background</p> <p>To explore adolescent HIV risk perception, HIV vaccine knowledge, willingness to participate in future HIV vaccine clinical trials, and the factors that influence willingness to participate among high school students in Soweto, South Africa, we recruited school-going youth through randomly selected local high schools. All pupils within the selected schools from whom parental consent and child assent could be obtained were eligible for participation. A self-administered, facilitated questionnaire was completed by all participants.</p> <p>Findings</p> <p>Perception of adolescent HIV risk was high. Some misconceptions regarding vaccine research were common, particularly regarding placebo and potential eligibility criteria for prophylactic vaccine trials. Of 240 responses to the willingness item, 84 (35%) indicated they were "probably willing" and 126 (52.5%) that they were "definitely willing to participate". There were no significant differences in willingness by gender, age, school grade, or institution. Factors that were rated as "very important" in determining willingness included receiving current information about HIV research [n = 201 (88.9%)], doing something to honour people who have HIV or have died of AIDS [n = 168 (70.9%)], getting free counselling and testing [n = 167 (70.5)], that participants may receive some protection against HIV infection from the vaccine [n = 160 (70.2%)], and improving motivation to avoid risky behaviour [n = 134 (59%)].</p> <p>Conclusion</p> <p>Soweto school-going youth report high degrees of willingness to participate in HIV vaccine trials. This may be related to the high levels of adolescent HIV risk perception. Whether hypothetical willingness translates to participation will await data from adolescent HIV vaccine trials.</p

    The effect of parity on longitudinal maternal hemodynamics

    BACKGROUND: Parous women have a lower risk for pregnancy complications, such as preeclampsia (PE) or delivery of small for gestational age (SGA) neonates. However, parous women are a heterogeneous group of patients as they contain a low-risk cohort with previously uncomplicated pregnancies and a high-risk cohort with previous pregnancies complicated by PE and / or SGA. Previous studies examining the effect of parity on maternal hemodynamics, including cardiac output (CO) and peripheral vascular resistance (PVR), did not distinguish between parous women with and without a history of PE or SGA and reported contradictory results.OBJECTIVES: To compare maternal hemodynamics, in nulliparous women and in parous women with and without previous PE and / or SGA.STUDY DESIGN: This was a prospective, longitudinal study of maternal hemodynamics, assessed by a bioreactance method, measured at 11+0-13+6, 19+0-24+0, 30+0-34+0 and 35+0-37+0 weeks' gestation in three groups of women. Group 1 was composed of parous women without history of PE and / or SGA (n=632), Group 2 was of nulliparous women (n=829) and Group 3 was composed of parous women with history of PE and / or SGA (n=113). Multilevel linear mixed-effects model was performed to compare the repeated measures of hemodynamic variables controlling for maternal characteristics, medical history, and development of PE or SGA in the current pregnancy.RESULTS: In groups 1 and 2, CO increased with gestational age to a peak at 32 weeks and PVR showed a reversed pattern with its nadir at 32 weeks; in group 1, compared to group 2, there was better cardiac adaptation, reflected in higher CO and lower PVR. In group 3 there was a hyperdynamic profile of higher CO and lower PVR at first trimester followed by an earlier sharp decline of CO and increase of PVR from mid-gestation. The incidence of PE and SGA was highest in group 3 and lowest in group1.CONCLUSION: There are parity-specific differences in maternal cardiac adaptation in pregnancy.</p

    Next-Generation EU DataGrid Data Management Services

    We describe the architecture and initial implementation of the next-generation of Grid Data Management Middleware in the EU DataGrid (EDG) project. The new architecture stems out of our experience and the users requirements gathered during the two years of running our initial set of Grid Data Management Services. All of our new services are based on the Web Service technology paradigm, very much in line with the emerging Open Grid Services Architecture (OGSA). We have modularized our components and invested a great amount of effort towards a secure, extensible and robust service, starting from the design but also using a streamlined build and testing framework. Our service components are: Replica Location Service, Replica Metadata Service, Replica Optimization Service, Replica Subscription and high-level replica management.     Surviving the cold: molecular analyses of insect cryoprotective dehydration in the Arctic springtail Megaphorura arctica (Tullberg)

    <p>Abstract</p> <p>Background</p> <p>Insects provide tractable models for enhancing our understanding of the physiological and cellular processes that enable survival at extreme low temperatures. They possess three main strategies to survive the cold: freeze tolerance, freeze avoidance or cryoprotective dehydration, of which the latter method is exploited by our model species, the Arctic springtail <it>Megaphorura arctica</it>, formerly <it>Onychiurus arcticus </it>(Tullberg 1876). The physiological mechanisms underlying cryoprotective dehydration have been well characterised in <it>M. arctica </it>and to date this process has been described in only a few other species: the Antarctic nematode <it>Panagrolaimus davidi</it>, an enchytraied worm, the larvae of the Antarctic midge <it>Belgica antarctica </it>and the cocoons of the earthworm <it>Dendrobaena octaedra</it>. There are no in-depth molecular studies on the underlying cold survival mechanisms in any species.</p> <p>Results</p> <p>A cDNA microarray was generated using 6,912 <it>M. arctica </it>clones printed in duplicate. Analysis of clones up-regulated during dehydration procedures (using both cold- and salt-induced dehydration) has identified a number of significant cellular processes, namely the production and mobilisation of trehalose, protection of cellular systems via small heat shock proteins and tissue/cellular remodelling during the dehydration process. Energy production, initiation of protein translation and cell division, plus potential tissue repair processes dominate genes identified during recovery. Heat map analysis identified a duplication of the trehalose-6-phosphate synthase (TPS) gene in <it>M. arctica </it>and also 53 clones co-regulated with TPS, including a number of membrane associated and cell signalling proteins. Q-PCR on selected candidate genes has also contributed to our understanding with glutathione-S-transferase identified as the major antioxdidant enzyme protecting the cells during these stressful procedures, and a number of protein kinase signalling molecules involved in recovery.</p> <p>Conclusion</p> <p>Microarray analysis has proved to be a powerful technique for understanding the processes and genes involved in cryoprotective dehydration, beyond the few candidate genes identified in the current literature. Dehydration is associated with the mobilisation of trehalose, cell protection and tissue remodelling. Energy production, leading to protein production, and cell division characterise the recovery process. Novel membrane proteins, along with aquaporins and desaturases, have been identified as promising candidates for future functional analyses to better understand membrane remodelling during cellular dehydration.</p

    The role of optic nerve sheath diameter ultrasound in brain infection.

    Brain infections cause significant morbidity and mortality worldwide, especially in resource-limited settings with high HIV co-infection rates. Raised intracranial pressure [ICP] may complicate brain infection and worsen neurological injury, yet invasive ICP monitoring is often unavailable. Optic nerve sheath diameter [ONSD] ultrasound may allow detection of raised ICP at the bedside; however, pathology in brain infection is different to traumatic brain injury, in which most studies have been performed. The use of ONSD ultrasound has been described in tuberculous meningitis, cryptococcal meningitis and cerebral malaria; however correlation with invasive ICP measurement has not been performed. Normal optic nerve sheath values are not yet established for most populations, and thresholds for clinical intervention cannot be assumed to match those used in non-infective brain pathology. ONSD ultrasound may be suitable for use in resource-limited settings by clinicians with limited ultrasound training. Standardisation of scanning technique, consensus on normal ONSD values, and action on abnormal results, are areas for future research. This scoping review examines the role of ONSD ultrasound in brain infection. We discuss pathophysiology, and describe the rationale, practicalities, and challenges of utilising ONSD ultrasound for brain infection monitoring and management. We discuss the existing evidence base for this technique, and identify knowledge gaps and future research priorities

    A Highly Stable Blood Meal Alternative for Rearing \u3cem\u3eAedes\u3c/em\u3e and \u3cem\u3eAnopheles\u3c/em\u3e Mosquitoes

    We investigated alternatives to whole blood for blood feeding of mosquitoes with a focus on improved stability and compatibility with mass rearing programs. In contrast to whole blood, an artificial blood diet of ATP-supplemented plasma was effective in maintaining mosquito populations and was compatible with storage for extended periods refrigerated, frozen, and as a lyophilized powder. The plasma ATP diet supported rearing of both Anopheles and Aedes mosquitoes. It was also effective in rearing Wolbachia-infected Aedes mosquitoes, suggesting compatibility with vector control efforts
