    Non – Hodgkin’s Lymphoma presenting as Garcin’s Syndrome – A case report and Literature Review

    An 80 year old man presented with a twelve month history of progressive multiple unilateral cranial nerve palsies. A gadolinium enhanced MRI scan of his head and neck was performed. This showed an extra-axial enhancing lesion, which enveloped the hemi-mandible at the level of the left inferior alveolar nerve, and a dumbbell shaped lesion within the cerebellopontine angle. An incisional biopsy was performed to gain a tissue diagnosis, which demonstrated a Non-Hodgkin’s Lymphoma. The combination of signs, symptoms and radiological findings enabled a diagnosis of Garcin’s Syndrome secondary to Non-Hodgkin’s Lymphoma

In particular, we consider those algebras in which this lattice is dually atomistic, lower or upper semimodular, or in which every restricted subalgebra is a quasi-ideal. The fact that there are one-dimensional subalgebras which are not restricted results in some of these conditions being weaker than for the corresponding conditions in the non-restricted case

    El següent treball tracta de com es va realitzar la implementació d'un edifici d'habitatges en el territori andorrà, amb totes les seves fases. Primer de tot farem una contextualització de l'edifici per veure les seves principals característiques, la seva localització, la climatologia i morfologia de la zona i el marc legal en el qual es du a terme aquesta construcció. Després hi haurà una segona part on fonamentalment es veurà com es va recondicionar el terreny per poder fer l'edifici. Amb els mètodes emprats per al moviment de terres i la duració d'aquesta fase del projecte. Un cop ja hem vist aquesta part importantíssima en l'edificació andorrana passarem a analitzar la fase d'execució de l'obra amb totes les fases de construcció, elements utilitzats, temps de dedicació de cada fase i finalment solucions preses per l'execució. En veure tot el procés constructiu farem una repassada als materials seleccionats per l'acabament de l'obra. Per tant, en acabar el treball, podrem tenir una visió global tant de la zona geogràfica, com de la seva geologia i climatologia. A més també podrem tenir una petita apreciació de com funciona la legislació andorrana en la construcció. En últim lloc, el treball ens ofereix la possibilitat de veure tot el procés que s'ha dut a terme per al desenvolupament de l'edifici i les diferències que es poden apreciar amb la construcció a una zona més plana o sense tanta muntanya, així com la duració total de l'obraEl siguiente trabajo trata sobre cómo se realizó la implementación de un edificio de viviendas en el territorio andorrano, con todas sus fases. Antes de nada, haremos una contextualización del edificio para ver sus principales características, su localización, la climatología y morfología de la zona y el marco legal en el cual se lleva a cabo esta construcción. Después habrá una segunda parte donde fundamentalmente se explicará cómo se reacondicionó el terreno para poder hacer el edificio. Con los métodos empleados para el movimiento de tierras y la duración de esta fase del proyecto. Una vez ya hemos visto esta parte importantísima en la edificación andorrana pasaremos a analizar la fase de ejecución de la obra con todas las fases de construcción, elementos utilizados, tiempos de dedicación de cada fase y finalmente soluciones tomadas para la ejecución. Al ver todo el proceso constructivo haremos una repasada a los materiales seleccionados para el acabado de la obra tanto de puertas como ventanas como todas las soluciones seleccionadas. Por lo tanto, al acabar el trabajo, podremos tener una visión global tanto de la zona geográfica, como de su geología y climatología. Además también podremos tener una pequeña apreciación de cómo funciona la legislación andorrana en la construcción. Por último, el trabajo nos ofrece la posibilidad de ver todo el proceso que se ha llevado a cabo para el desarrollo del edificio y las diferencias que se pueden apreciar con la construcción a una zona más plana o sin tanta montaña, así como la duración total de la obraThe following work is about how the implementation of a housing building in the Andorran territory was carried out, with all its phases. First of all, we will contextualise the building to see its main characteristics, its location, the climatology and morphology of the area and the legal framework in which this construction is carried out. Then there will be a second part where you will basically see how the terrain was rebuilt to make the building. With the methods used for the movement of land and the duration of this phase of the project. Once we have seen this very important part in the Andorran building, we will go on to analyse the implementation phase of the work with all the construction phases, elements used, dedication time of each phase and finally solutions taken by the execution.     Modularity conditions in Leibniz algebras

    In this paper we continue the study of the subalgebra lattice of a Leibniz algebra. In particular, we find out that solvable Leibniz algebras with an upper semi-modular lattice are either almost-abelian or have an abelian ideal spanned by the elements with square zero. We also study Leibniz algebras in which every subalgebra is a weak quasi-ideal, as well as modular symmetric Leibniz algebras

    Trastornos de personalidad en pacientes aquejados de anorexia y bulimia nerviosa : un estudio piloto

    In this paper was compared the personality profile in 25 outpatients with eating disorders (5 with restricting anorexia nervosa, 7 with purging anorexia nervosa, 5 with purging bulimia nervosa and 8 with eating disorder not otherwise specified). Although 72% of the females showed at least one personality disorder, there were not differences among groups. Later on, all the patients were divided in two groups, according to the presence or absence of purgative behaviors. Patients with purgative behaviors showed more frequently and had a higher number of personality disorders than patients without purging behaviors. Finally implications for further research in this area are commented upon.En este artículo se comparó el perfil de trastornos de personalidad en 25 pacientes con un trastorno de la conducta alimentaria (5 con anorexia restrictiva, 7 con anorexia purgativa, 5 con bulimia purgativa y 8 con un trastorno de la conducta alimentaria inespecífico). El 72% de las pacientes presentaba, al menos, un trastorno de personalidad, pero no había diferencias de frecuencia entre los cuatro grupos. Más tarde, las 25 pacientes se dividieron en función de la presencia o ausencia de conductas purgativas. Las pacientes purgativas presentaron trastornos de personalidad, de forma estadísticamente significativa, con mayor frecuencia y, además, en mayor número. Se comentan las líneas futuras de investigación en relación con este tema

    De-novo non-convulsive status epilepticus in adult medical inpatients without known epilepsy: Analysis of mortality related factors and literature review

    BACKGROUND: Non-convulsive status epilepticus (NCSE) often goes unnoticed and is not easily detected in patients with a decreased level of consciousness, especially in older patients. In this sense, lack of data in this population is available. AIMS: The aim of the present study was to examine daily clinical practice and evaluate factors that may influence the prognosis of NCSE in non-epileptic medical inpatients. METHODS: We conducted a retrospective analysis including patients admitted by any cause in an Internal Medicine ward. All patients with compatible symptoms, exclusion of other causes, clinical suspicion or diagnosis of NCSE, and compatible EEG were included. Patients with a previous diagnosis of epilepsy were excluded. We also conducted a literature review by searching the PubMed/Medline database with the terms: Nonconvulsive Status OR Non-Convulsive Status. RESULTS: We included 54 patients, mortality rate reached 37% and the main factors linked to it were hypernatremia (OR = 16.2; 95% CI, 1.6-165.6; P = 0.019) and atrial fibrillation (OR = 6.7; 95% CI, 1.7-26; P = 0.006). There were no differences regarding mortality when comparing different diagnosis approach or treatment regimens. Our literature review showed that the main etiology of NCSE were neurovascular causes (17.8%), followed by antibiotic treatment (17.2%) and metabolic causes (17%). Global mortality in the literature review, excluding our series, reached 20%. DISCUSSION: We present the largest series of NCSE cases in medical patients, which showed that this entity is probably misdiagnosed in older patients and is linked to a high mortality. CONCLUSION: The presence of atrial fibrillation and hypernatremia in patients diagnosed with NCSE should advise physicians of a high mortality risk

    Mediterranean diet and type 2 diabetes risk in the European Prospective Investigation into Cancer and Nutrition (EPIC) study: the InterAct project.

    OBJECTIVE: To study the association between adherence to the Mediterranean dietary pattern (MDP) and risk of developing type 2 diabetes, across European countries. RESEARCH DESIGN AND METHODS: We established a case-cohort study including 11,994 incident type 2 diabetic case subjects and a stratified subcohort of 15,798 participants selected from a total cohort of 340,234 participants with 3.99 million person-years of follow-up, from eight European cohorts participating in the European Prospective Investigation into Cancer and Nutrition (EPIC) study. The relative Mediterranean diet score (rMED) (score range 0-18) was used to assess adherence to MDP on the basis of reported consumption of nine dietary components characteristic of the Mediterranean diet. Cox proportional hazards regression, modified for the case-cohort design, was used to estimate the association between rMED and risk of type 2 diabetes, adjusting for confounders. RESULTS: The multiple adjusted hazard ratios of type 2 diabetes among individuals with medium (rMED 7-10 points) and high adherence to MDP (rMED 11-18 points) were 0.93 (95% CI 0.86-1.01) and 0.88 (0.79-0.97), respectively, compared with individuals with low adherence to MDP (0-6 points) (P for trend 0.013). The association between rMED and type 2 diabetes was attenuated in people <50 years of age, in obese participants, and when the alcohol, meat, and olive oil components were excluded from the score. CONCLUSIONS: In this large prospective study, adherence to the MDP, as defined by rMED, was associated with a small reduction in the risk of developing type 2 diabetes in this European population

    Evolution of the use of corticosteroids for the treatment of hospitalised COVID-19 patients in Spain between March and November 2020: SEMI-COVID national registry

    Objectives: Since the results of the RECOVERY trial, WHO recommendations about the use of corticosteroids (CTs) in COVID-19 have changed. The aim of the study is to analyse the evolutive use of CTs in Spain during the pandemic to assess the potential influence of new recommendations. Material and methods: A retrospective, descriptive, and observational study was conducted on adults hospitalised due to COVID-19 in Spain who were included in the SEMI-COVID- 19 Registry from March to November 2020. Results: CTs were used in 6053 (36.21%) of the included patients. The patients were older (mean (SD)) (69.6 (14.6) vs. 66.0 (16.8) years; p < 0.001), with hypertension (57.0% vs. 47.7%; p < 0.001), obesity (26.4% vs. 19.3%; p < 0.0001), and multimorbidity prevalence (20.6% vs. 16.1%; p < 0.001). These patients had higher values (mean (95% CI)) of C-reactive protein (CRP) (86 (32.7-160) vs. 49.3 (16-109) mg/dL; p < 0.001), ferritin (791 (393-1534) vs. 470 (236- 996) µg/dL; p < 0.001), D dimer (750 (430-1400) vs. 617 (345-1180) µg/dL; p < 0.001), and lower Sp02/Fi02 (266 (91.1) vs. 301 (101); p < 0.001). Since June 2020, there was an increment in the use of CTs (March vs. September; p < 0.001). Overall, 20% did not receive steroids, and 40% received less than 200 mg accumulated prednisone equivalent dose (APED). Severe patients are treated with higher doses. The mortality benefit was observed in patients with oxygen saturation </=90%. Conclusions: Patients with greater comorbidity, severity, and inflammatory markers were those treated with CTs.
