Acrokeratosis Can Be a Warning Sign of an Underlying Malignancy
Introduction: Some cutaneous manifestations can be the first presentation of an underlying malignancy. These so-called paraneoplastic syndromes can sometimes be very subtle or strongly resemble other benign cutaneous diseases.
Aims: In this report, we want to emphasize the need for further investigation of eczema-like cutaneous presentations which develop at a later age and are recalcitrant to therapy. Exclusion of an underlying malignancy needs to be considered.
Patients and Methods: A 53-year-old man with thickening of the skin, more pronounced on the hands, feet and face, was diagnosed with classic nodular sclerosing Hodgkin disease based on cutaneous presentation.
Results: After two rounds of chemotherapy (adriamycin, bleomycin, vinblastine and dacarbazine), the cutaneous symptoms had disappeared. After six rounds of chemotherapy and 14 months of follow-up, the patient is still in remission without recurrence of the cutaneous symptoms.
Conclusions: Cutaneous symptoms recalcitrant to adequate treatment should raise suspicion and prompt further investigation to exclude an underlying malignancy. A multidisciplinary approach with the dermatology department can accelerate diagnosis and improve the patient’s prognosis
Diseño hidráulico del alcantarillado pluvial con metodología BIM en la localidad de San Antonio de Cumbaza, Tarapoto 2021
En la presente tesis el objetivo es la realización del diseño de alcantarillado pluvial
con metodología Bim en la localidad de san Antonio de Cumbaza. Hemos elaborado
procesos estadísticos para la elaboración de un método tradicional de diseño de
alcantarillado pluvial y tener una comparación con la metodología Bim. También
tenemos una consideración crucial con la obtención de datos del levantamiento
topográfico ya que son imprescindibles para un buen diseño. Reconocemos la
importancia de la diferencia de las alturas, ya que estas nos orientan a un bosquejo
óptimo para el sistema de alcantarillado pluvial. El diseño del alcantarillado pluvial
con el método tradicional en terrenos extensos nos demuestra que la sección de
alcantarilla es muy variada por lo que nosotros optamos de dar dimensiones
homogéneas en tramos parecidos. Se concluyó que la aplicación de la metodología
Bim en el diseño de obras de drenaje, dependiendo del área de diseño es muy
favorable para reducir tiempos de diseño y cumplir con las normas vigentes;
asimismo se recomienda aplicar la metodología Bim en diseños de gran magnitud
o de proceso de diseño complejo, para así favorecer y facilitar el proceso de diseño
de futuras obras de drenaje
The international EAACI/GA²LEN/EuroGuiDerm/APAAACI guideline for the definition, classification, diagnosis, and management of urticaria
This update and revision of the international guideline for urticaria was developed following the methods recommended by Cochrane and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) working group. It is a joint initiative of the Dermatology Section of the European Academy of Allergology and Clinical Immunology (EAACI), the Global Allergy and Asthma European Network (GA(2)LEN) and its Urticaria and Angioedema Centers of Reference and Excellence (UCAREs and ACAREs), the European Dermatology Forum (EDF; EuroGuiDerm), and the Asia Pacific Association of Allergy, Asthma and Clinical Immunology with the participation of 64 delegates of 50 national and international societies and from 31 countries. The consensus conference was held on 3 December 2020. This guideline was acknowledged and accepted by the European Union of Medical Specialists (UEMS). Urticaria is a frequent, mast cell-driven disease that presents with wheals, angioedema, or both. The lifetime prevalence for acute urticaria is approximately 20%. Chronic spontaneous or inducible urticaria is disabling, impairs quality of life, and affects performance at work and school. This updated version of the international guideline for urticaria covers the definition and classification of urticaria and outlines expert-guided and evidence-based diagnostic and therapeutic approaches for the different subtypes of urticaria

Anaphylaxis: subjective symptom reports of patients with a special focus on “near-death” experiences
<jats:title>Summary</jats:title><jats:p>Almost all anaphylactic reactions are associated with subjective symptoms, which are also referred to “prodromi”, and are often not taken seriously. In 100 patients categorized with different severity grades, subjective symptoms were investigated in a personal interview with free/open questions regarding subjective feelings and symptoms (duration 5–90 min). After the interview, the patients were asked to complete a questionnaire about the exact occurrence of the reaction. Special focus was given to near-death experiences in severe reactions. Patients described feelings of “elevation”, “looking at themselves from outside”, “tunnel experiences” and “bright light”. In the kinetics of the anaphylactic reaction, cutaneous symptoms were most often reported as the first or second symptom, while cardiovascular and airway symptoms were also reported as the 3rd, 4th or 5th symptom. In 37% of the patients with severe reactions, everyday life has changed since suffering the anaphylactic episode.</jats:p>
Failure of omalizumab and successful control with ketotifen in a patient with vibratory angio-oedema.
Vibratory angio-oedema is a rare form of physical urticaria characterized by pruriginous weals and angio-oedema at the site of exposure to vibration. Severe treatment-resistant disease can occur, and is associated with significant disability. Therapy with omalizumab, a monoclonal IgG anti-IgE antibody, has been shown to be successful in several types of physical urticaria. We report a patient with vibratory angio-oedema for whom all standard treatments for urticaria, including omalizumab, failed to show a clinical benefit. Finally, ketotifen was tried, and unexpectedly reduced symptoms significantly. Ketotifen may thus represent a therapeutic option in patients with treatment-resistant vibratory angio-oedema
