    Quantification of micro-scale variability in fibre bundles

    Local variations in the random filament arrangement in carbon fibre bundles were determined by optical microscopy and automated image analysis. Successive steps of abrading, polishing and acquiring micro¬graphs of the sample surface made it feasible to analyse the micro-structure over a series of cross-sections along the fibre bundle path. Random and systematic changes in local filament arrangements were determined. Systematic changes were related to the interaction of a fibre bundle with an intersect¬ing binder thread leading to a local increase of the fibre volume fraction at the interface. Random clus¬tering of filaments in areas of high or low fibre volume fractions within the fibre bundles were found to be unaffected by the relative position of the bundle

    Effects of layer shift and yarn path variability on mechanical properties of a twill weave composite

    Experimental and numerical analysis of a woven composite were performed in order to assess the effect of yarn path and layer shift variability on properties of the composite. Analysis of the geometry of a 12K carbon fibre 2×2 twill weave at the meso- and macro-scales showed the prevalence of the yarn path variations at the macro-scale over the meso-scale variations. Numerical analysis of yarn path variability showed that it is responsible for a Young’s modulus reduction of 0.5% and CoV of 1% which makes this type of variability in the selected reinforcement almost insignificant for an elastic analysis. Finite element analysis of damage propagation in laminates with layer shift showed good agreement with the experiments. Both numerical analysis and experiments showed that layer shift has a strong effect on the shape of the stress-strain curve. In particular, laminates with no layer shift tend to exhibit a kink in the stress-strain curve which was attributed solely to the layer configuration

    Analytical method using gamma functions for determining areas of power elliptical shapes for use in geometrical textile models

    Textile models are often assumed to have homogenous and well defined cross-sections. For these models, the use of a power elliptical cross-sectional shape has been found to be beneficial as different shapes can be created, e.g. lenticular, elliptical or rectangular, with a single function. The cross-sectional area of a power ellipse is usually determined numerically as the analytical determination of the cross-sectional area is not straightforward. This short communication presents an analytical solution for this shape

    Stochastic reconstruction of filament paths in fibre bundles based on two-dimensional input data

    Conventional optical microscopy is an inexpensive technique to analyse fibre bundle micro-structures. Compared to micro-computed tomography, the resolution is higher and larger samples sizes can be analysed. This provides, for example, detailed information of filament spacing which will affect subsequent properties. This paper proposes a probabilistic method to reconstruct paths of individual filaments from a series of two-dimensional micrographs. This allows three-dimensional filament arrangements to be reconstructed and the effects of local arrangements on properties to be quantified. For example, for the small sample analysed in this work, a 15% difference in strength values was found

    Analysis of filament arrangements and generation of statistically equivalent composite micro-structures

    An efficient method to describe and quantify the filament arrangement in fibre bundles based on the analysis of micrographs was developed. Quantitative measurement of relative filament positions indicated that the initially random arrangement of filaments shows increasingly strong characteristics of square and hexagonal configurations with increasing level of transverse compaction. An existing micro-structure generator was extended to incorporate the measured data allowing statistically equivalent filament arrangements to be generated at any fibre volume fraction. These can be used to determine micro-structural properties of actual fibre reinforced composites

    Influence of Anesthesia and Clinical Variables on the Firing Rate, Coefficient of Variation and Multi-Unit Activity of the Subthalamic Nucleus in Patients with Parkinson's Disease

    BACKGROUND: Microelectrode recordings (MER) are used to optimize lead placement during subthalamic nucleus deep brain stimulation (STN-DBS). To obtain reliable MER, surgery is usually performed while patients are awake. Procedural sedation and analgesia (PSA) is often desirable to improve patient comfort, anxiolysis and pain relief. The effect of these agents on MER are largely unknown. The objective of this study was to determine the effects of commonly used PSA agents, dexmedetomidine, clonidine and remifentanil and patient characteristics on MER during DBS surgery. METHODS: Data from 78 patients with Parkinson's disease (PD) who underwent STN-DBS surgery were retrospectively reviewed. The procedures were performed under local anesthesia or under PSA with dexmedetomidine, clonidine or remifentanil. In total, 4082 sites with multi-unit activity (MUA) and 588 with single units were acquired. Single unit firing rates and coefficient of variation (CV), and MUA total power were compared between patient groups. RESULTS: We observed a significant reduction in MUA, an increase of the CV and a trend for reduced firing rate by dexmedetomidine. The effect of dexmedetomidine was dose-dependent for all measures. Remifentanil had no effect on the firing rate but was associated with a significant increase in CV and a decrease in MUA. Clonidine showed no significant effect on firing rate, CV or MUA. In addition to anesthetic effects, MUA and CV were also influenced by patient-dependent variables. CONCLUSION: Our results showed that PSA influenced neuronal properties in the STN and the dexmedetomidine (DEX) effect was dose-dependent.     The implementation of integrated care: the empirical validation of the Development Model for Integrated Care

    Background: Integrated care is considered as a strategy to improve the delivery, efficiency, client outcomes and satisfaction rates of health care. To integrate the care from multiple providers into a coherent client-focused service, a large number of activities and agreements have to be implemented like streamlining information flows and patient transfers. The Development Model for Integrated care (DMIC) describes nine clusters containing in total 89 elements that contribute to the integration of care. We have empirically validated this model in practice by assessing the relevance, implementation and plans of the elements in three integrated care service settings in The Netherlands: stroke, acute myocardial infarct (AMI), and dementia. Methods. Based on the DMIC, a survey was developed for integrated care coordinators. We invited all Dutch stroke and AMI-services, as well as the dementia care networks to participate, of which 84 did (response rate 83%). Data were collected on relevance, presence, and year of implementation of the 89 elements. The data analysis was done by means of descriptive statistics, Chi Square, ANOVA and Kruskal-Wallis H tests. Results: The results indicate that the integrated care practice organizations in all three care settings rated the nine clusters and 89 elements of the DMIC as highly relevant. The average number of elements implemented was 50 18, 42 13, and 45 22 for stroke, acute myocardial infarction, and dementia care services, respectively. Although the dementia networks were significantly younger, their numbers of implemented elements were comparable to those of the other services. The analyses of the implementation timelines showed that the older integrated care services had fewer plans for further implementation than the younger ones. Integrated care coordinators stated that the DMIC helped them to assess their integrated care development in practice and supported them in obtaining ideas for expanding their integrated car

    An intervention program with the aim to improve and maintain work productivity for workers with rheumatoid arthritis: design of a randomized controlled trial and cost-effectiveness study

    <p>Abstract</p> <p>Background</p> <p>Workers with rheumatoid arthritis (RA) often experience restrictions in functioning at work and participation in employment. Strategies to maintain work productivity exist, but these interventions do not involve the actual workplace. Therefore the aim of this study is to investigate the (cost)effectiveness of an intervention program at the workplace on work productivity for workers with RA.</p> <p>Methods/design</p> <p>This study is a randomized controlled trial (RCT) in specialized rheumatology treatment centers in or near Amsterdam, the Netherlands. Randomisation to either the control or the intervention group is performed at patient level. Both groups will receive care as usual by the rheumatologist, and patients in the intervention group will also take part in the intervention program. The intervention program consists of two components; integrated care, including a participatory workplace intervention. Integrated care involves a clinical occupational physician, who will act as care manager, to coordinate the care. The care manager has an intermediate role between clinical and occupational care. The participatory workplace intervention will be guided by an occupational therapist, and involves problem solving by the patient and the patients’ supervisor. The aim of the workplace intervention is to achieve consensus between patient and supervisor concerning feasible solutions for the obstacles for functioning at work. Data collection will take place at baseline and after 6 and 12 months by means of a questionnaire. The primary outcome measure is work productivity, measured by hours lost from work due to presenteeism. Secondary outcome measures include sick leave, quality of life, pain and fatigue. Cost-effectiveness of the intervention program will be evaluated from the societal perspective.</p> <p>Discussion</p> <p>Usual care of primary and outpatient health services is not aimed at improving work productivity. Therefore it is desirable to develop interventions aimed at improving functioning at work. If the intervention program will be (cost)effective, substantial improvements in work productivity might be obtained among workers with RA at lower costs.     Measurement of an indentation size effect for Indium as a function of temperature

    We present compression and instrumented indentation experiments on high-purity Indium, at temperatures from 22 to −100 °C. A strong indentation size effect is detected at all temperatures, corresponding to a large material length-scale. Therefore the large length-scale is not a consequence of creep mechanisms but an intrinsic property of the material
