Basic biochemical investigations as rationale for the design of original antimalarial drugs. An example of phospholipid metabolism
The future of antimalarial chemotherapy is particulary alarming in view of the spread of parasite cross-resistances to drugs that are not even structurally related. Only the availability of new pharmacological models will make it possible to select molecules with novel mechanisms of action, thus delaving resistance and allowing the development of new chemotherapeutic strategies. We reached this objective in mice. Our approach is hunged on fundamental and applied research begun in 1980 to investigate to phospholipid (PL) metabolism of intraerythrocytic Plasmodium. This metabolism is abundant, specific and indispensable for the production of Plasmodium membranes. Any drug to interfere with this metabolism blocks parasitic development. The most effective interference yet found involves blockage of the choline transporter, which supplies Plasmodium with choline for the synthesis of phosphatidylcholine, its major PL, this is a limiting step in the pathway. The drug sensitivity thereshold is much lower for the parasite, which is more dependent on this metabolism than host cells. The compounds show in vitro activity against P. falciparum at 1 to 10 nM. They show a very low toxicity against a lymphblastoid cell line, demonstrating a total abscence of correlation between growth inhibition of parasites and lymphoblastoid cells. They show antimalarial activity in vivo, in the P. berghei or P. chabaudi/mouse system, at doses 20-to 100-fold lower than their in acute toxicity limit. The bioavailability of a radiolabeled form of the product seemed to be advantageous (slow blood clearance and no significant concentration in tissues). Lastly, the compounds are inexpensive to produce. They are stable and water-soluble
Impact of age on susceptibility to aphanomycosis diseases in signal crayfish Pacifastacus leniusculus (Dana, 1852)
The signal crayfish Pacifastacus leniusculus (Dana, 1852) is an American species that was introduced into Spanish rivers. This species carries the fungus responsible for aphanomycosis, Aphanomyces astaci (Schikora), and can transmit it to the native European crayfish species. Although P. leniusculus is often a carrier of this fungus, it generally does not suffer from aphanomycosis; however, if individuals are under stressful conditions, such as the presence of another pathogen or parasite, the chronic infection can develop into an acute, fatal condition. The present paper describes an experiment in which four groups of P. leniusculus, of different ages, were exposed to A. astaci zoospores, in order to compare differences in susceptibility. Our results show that P. leniusculus can die of this disease at concentrations formerly considered sublethal for this species, and also that crayfish juveniles can suffer from this disease. However, significant inter-group differences were not found.
Mortality Prediction after the First Year of Kidney Transplantation: An Observational Study on Two European Cohorts.
After the first year post transplantation, prognostic mortality scores in kidney transplant recipients can be useful for personalizing medical management. We developed a new prognostic score based on 5 parameters and computable at 1-year post transplantation. The outcome was the time between the first anniversary of the transplantation and the patient's death with a functioning graft. Afterwards, we appraised the prognostic capacities of this score by estimating time-dependent Receiver Operating Characteristic (ROC) curves from two prospective and multicentric European cohorts: the DIVAT (Données Informatisées et VAlidées en Transplantation) cohort composed of patients transplanted between 2000 and 2012 in 6 French centers; and the STCS (Swiss Transplant Cohort Study) cohort composed of patients transplanted between 2008 and 2012 in 6 Swiss centers. We also compared the results with those of two existing scoring systems: one from Spain (Hernandez et al.) and one from the United States (the Recipient Risk Score, RRS, Baskin-Bey et al.). From the DIVAT validation cohort and for a prognostic time at 10 years, the new prognostic score (AUC = 0.78, 95%CI = [0.69, 0.85]) seemed to present significantly higher prognostic capacities than the scoring system proposed by Hernandez et al. (p = 0.04) and tended to perform better than the initial RRS (p = 0.10). By using the Swiss cohort, the RRS and the the new prognostic score had comparable prognostic capacities at 4 years (AUC = 0.77 and 0.76 respectively, p = 0.31). In addition to the current available scores related to the risk to return in dialysis, we recommend to further study the use of the score we propose or the RRS for a more efficient personalized follow-up of kidney transplant recipients
Serum Calcium Levels Are Associated with Novel Cardiometabolic Risk Factors in the Population-Based CoLaus Study
BACKGROUND: Associations of serum calcium levels with the metabolic syndrome and other novel cardio-metabolic risk factors not classically included in the metabolic syndrome, such as those involved in oxidative stress, are largely unexplored. We analyzed the association of albumin-corrected serum calcium levels with conventional and non-conventional cardio-metabolic risk factors in a general adult population.
METHODOLOGY/PRINCIPAL FINDINGS: The CoLaus study is a population-based study including Caucasians from Lausanne, Switzerland. The metabolic syndrome was defined using the Adult Treatment Panel III criteria. Non-conventional cardio-metabolic risk factors considered included: fat mass, leptin, LDL particle size, apolipoprotein B, fasting insulin, adiponectin, ultrasensitive CRP, serum uric acid, homocysteine, and gamma-glutamyltransferase. We used adjusted standardized multivariable regression to compare the association of each cardio-metabolic risk factor with albumin-corrected serum calcium. We assessed associations of albumin-corrected serum calcium with the cumulative number of non-conventional cardio-metabolic risk factors. We analyzed 4,231 subjects aged 35 to 75 years. Corrected serum calcium increased with both the number of the metabolic syndrome components and the number of non-conventional cardio-metabolic risk factors, independently of the metabolic syndrome and BMI. Among conventional and non-conventional cardio-metabolic risk factors, the strongest positive associations were found for factors related to oxidative stress (uric acid, homocysteine and gamma-glutamyltransferase). Adiponectin had the strongest negative association with corrected serum calcium.
CONCLUSIONS/SIGNIFICANCE: Serum calcium was associated with the metabolic syndrome and with non-conventional cardio-metabolic risk factors independently of the metabolic syndrome. Associations with uric acid, homocysteine and gamma-glutamyltransferase were the strongest. These novel findings suggest that serum calcium levels may be associated with cardiovascular risk via oxidative stress
Health care and societal costs of the management of children and adolescents with attention-deficit/hyperactivity disorder in Spain: a descriptive analysis
Background: Attention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental condition in
childhood (5.3% to 7.1% worldwide prevalence), with substantial overall financial burden to children/adolescents,
their families, and society. The aims of this study were to describe the clinical characteristics of children and
adolescents with ADHD in Spain, estimate the associated direct/indirect costs of the disorder, and assess whether
the characteristics and financial costs differed between children/adolescents adequately responding to currently
available pharmacotherapies compared with children/adolescents for whom pharmacotherapies failed.
Methods: This was a multicenter, cross-sectional, descriptive analysis conducted in 15 health units representative of
the overall Spanish population. Data on demographic characteristics, socio-occupational status, social relationships,
clinical variables of the disease, and pharmacological and non-pharmacological treatments received were collected
in 321 children and adolescents with ADHD. Direct and indirect costs were estimated over one year from both a
health care system and a societal perspective.
Results: The estimated average cost of ADHD per year per child/adolescent was €5733 in 2012 prices; direct costs
accounted for 60.2% of the total costs (€3450). Support from a psychologist/educational psychologist represented
45.2% of direct costs and 27.2% of total costs. Pharmacotherapy accounted for 25.8% of direct costs and 15.5% of
total costs. Among indirect costs (€2283), 65.2% was due to caregiver expenses. The total annual costs were
significantly higher for children/adolescents who responded poorly to pharmacological treatment (€7654 versus
€5517; P = 0.024), the difference being mainly due to significantly higher direct costs, particularly with larger
expenses for non-pharmacological treatment (P = 0.012).
Conclusions: ADHD has a significant personal, familial, and financial impact on the Spanish health system and
society. Successful pharmacological intervention was associated with lower overall expenses in the management of
the disorde
Urinary tract infections in children after renal transplantation
Urinary tract infections (UTI) after pediatric kidney transplantation (KTX) are an important clinical problem and occur in 15–33% of patients. Febrile UTI, whether occurring in the transplanted kidney or the native kidney, should be differentiated from afebrile UTI. The latter may cause significant morbidity and is usually associated with acute graft dysfunction. Risk factors for (febrile) UTI include anatomical, functional, and demographic factors as well as baseline immunosuppression and foreign material, such as catheters and stents. Meticulous surveillance, diagnosis, and treatment of UTI is important to minimize acute morbidity and compromise of long-term graft function. In febrile UTI, parenteral antibiotics are usually indicated, although controlled data are not available. As most data concerning UTI have been accumulated retrospectively, future prospective studies have to be performed to clarify pathogenetic mechanisms and risk factors, improve prophylaxis and treatment, and ultimately optimize long-term renal graft survival
Poboljšana elektromagnetska kompatibilnost (EMC) sklopnog energetskog pretvarača sa slučajnom modulacijom
This paper surveys some analytical and experimental results concerning different randomized modulation strategies in switched-mode power converters (SMPCs). After a short review of practical experiences within the literature it presents the benefits of several randomized schemes for power converters (i.e. reduced electromagnetic interference - EMI, and lower acoustic noise). It also introduces the mathematical background for dealing with randomized modulation within the medium-frequency range: power spectrum density (PSD). Finally, the EMI measurements confirm the improved EMC performances of the randomized boost rectifier, as also in the DC-DC buck converter.
