    This paper reviews post-apartheid South Africa through Fanonā€™s critical analysis of decolonization. Since, for Fanon, apartheid represented the purest form of the Manichean politics of space that characterizes colonialism, a Fanonian perspective on South Africa asks to what extent has the geographical layout of apartheid been remapped? Addressing this question necessitates shifting the ā€œgeography of reasonā€ from technical discourses of policy-makers to the lived reality of the ā€œdamned of the earthā€. From this perspective, Fanonā€™s critique becomes relevant in two ways, first as a prism to understand the rise of xenophobic violence as a symptom of the degeneration of the idea of South Africaā€™s ā€œpromised landā€ and second as a way to listen to a new grassroots shack dweller movement, Abahlali baseMjondolo, that is challenging both neoliberal and progressive assumptions by advocating a quite different geographic layout for a ā€œtruly democraticā€ society

In addition, I am interested in how Fanonā€™s psychiatry papers, written while he was practising as a doctor in North Africa, provide another terrain to help elucidate Fanonā€™s active involvement as a situational critique

    What does it mean to speak of Fanonā€™s relevance fifty years after his death? I suggest that one element to consider might be an epistemological challenge signaled by social movements of the wretched of the earth. Fanonā€™s wretched of the earth is not a story of wretchedness and suffering but of self-fashioning movements which through action become aware of themselves as subjects as they become aware of the betrayals of the leaders who speak in their name. The challenge to university-trained intellectuals who want to aid these popular movements is to hear these voices and support their articulation. Fanonian practices call for a shift in epistemology. Critical of research paradigms developed by the World Bank or other international funding agencies, and focused instead on the articulation of needs from the bottom up, dialogues about education (in and outside the academy) for liberation can be rekindled

    Associations of Vitamin D with Inter- and Intra-Muscular Adipose Tissue and Insulin Resistance in Women with and without Polycystic Ovary Syndrome

    Low vitamin D and insulin resistance are common in polycystic ovary syndrome (PCOS) and associated with higher inter- and intra-muscular adipose tissue (IMAT). We investigated associations between vitamin D, IMAT and insulin resistance in a cross-sectional study of 40 women with PCOS and 30 women without PCOS, and pre- and post-exercise in a 12-week intervention in 16 overweight participants (10 with PCOS and six without PCOS). A non-classical body mass index (BMI) threshold was used to differentiate lean and overweight women (BMI ā‰„ 27 kg/mĀ²). Measurements included plasma 25-hydroxyvitamin D (25OHD), insulin resistance (glucose infusion rate (GIR; mg/mĀ²/min), fasting glucose and insulin, and glycated haemoglobin), visceral fat, mid-thigh IMAT (computed tomography) and total body fat (dual-energy X-ray absorptiometry). Women with both PCOS and low 25OHD levels had the lowest GIR (all p < 0.05). Higher IMAT was associated with lower 25OHD (B = -3.95; 95% CI -6.86, -1.05) and GIR (B = -21.3; 95% CI -37.16, -5.44) in women with PCOS. Overweight women with pre-exercise 25OHD ā‰„30 nmol/L had significant increases in GIR, and decreases in total and visceral fat (all p < 0.044), but no associations were observed when stratified by PCOS status. Women with PCOS and low 25OHD levels have increased insulin resistance which may be partly explained by higher IMAT. Higher pre-training 25OHD levels may enhance exercise-induced changes in body composition and insulin resistance in overweight women

    Reaching Inward Not Outward: Marketing via the Internet at the UK 2010 General Election

    The Internet has been to date used as a space for simple promotion by political parties; websites present an opportunity for the delivery of non-mediated communication directly to the online audience and nothing more. However, new patterns in usage during campaigns, particularly that of Barack Obama, aided by the technological innovations that fall under the umbrella of Web 2.0, offer new models of online political communication. Through an analysis of the websites and linked online presences of six parties that stood across the UK at the 2010 General Election, we find a dual strategy for Internet campaigning emerging. The persuasive traditions of electioneering remain a feature; however, the key emergent function is one of internal marketing to party supporters and activists. Large sections of party websites are being dedicated to harnessing supporters and converting them to being donators, promoters, and campaigners both online and offline. This suggests that the Internet is increasingly embedded within election communication and online communication strategies are becoming a feature of most of the parties' marketing communication mix.     Exercise and insulin resistance in PCOS: muscle insulin signalling and fibrosis

    OBJECTIVE:Mechanisms of insulin resistance in polycystic ovary syndrome (PCOS) remain ill-defined, contributing to sub-optimal therapies. Recognising skeletal muscle plays a key role in glucose homeostasis we investigated early insulin signalling, its association with aberrant transforming growth factor Ī² (TGFĪ²) regulated tissue fibrosis. We also explored the impact of aerobic exercise on these molecular pathways. METHODS:A secondary analysis from a cross-sectional study was undertaken in women with (n=30) or without (n=29) PCOS across lean and overweight BMIs. A subset of participants with (n=8) or without (n=8) PCOS who were overweight completed 12-weeks of aerobic exercise training. Muscle was sampled before and 30 min into a euglycaemic-hyperinsulinaemic clamp pre- and post-training. RESULTS:We found reduced signalling in PCOS of mechanistic target of rapamycin (mTOR). Exercise training augmented but did not completely rescue this signalling defect in women with PCOS. Genes in the TGFĪ² signalling network were upregulated in skeletal muscle in the overweight women with PCOS but were unresponsive to exercise training except for genes encoding LOX, collagen 1 and 3. CONCLUSIONS:We provide new insights into defects in early insulin signalling, tissue fibrosis, and hyperandrogenism in PCOS-specific insulin resistance in lean and overweight women. PCOS-specific insulin-signalling defects were isolated to mTOR, while gene expression implicated TGFĪ² ligand regulating a fibrosis in the PCOS-obesity synergy in insulin resistance and altered responses to exercise. Interestingly, there was little evidence for hyperandrogenism as a mechanism for insulin resistance

    Health status, sexual activity and satisfaction among older people in Britain: A mixed methods study.

    BACKGROUND: Men and women are increasingly likely to stay sexually active into later life, but research shows that sexual activity and satisfaction decrease with increasing age. Ill health and medical treatments may affect sexual activity but there is little research on why some older people with a health problem affecting their sexual activity are satisfied with their sex life, and others are not. METHODS: A mixed method study integrating data and analyses from the third British National Survey of Sexual Attitudes and Lifestyles (n = 3343 aged 55-74 years) and from follow-up in-depth interviews with 23 survey participants who reported having a health condition or taking medication affecting their sex life. RESULTS: Overall, 26.9% of men and 17.1% of women aged 55-74 reported having a health condition that affected their sex life. Among this group, women were less likely than men to be sexually active in the previous 6 months (54.3% vs 62.0%) but just as likely to be satisfied with their sex life (41.9% vs 42.1%). In follow-up interviews, participants sometimes struggled to tease out the effects of ill health from those of advancing age. Where effects of ill health were identified, they tended to operate through the inclination and capacity to be sexually active, the practical possibilities for doing so and the limits placed on forms of sexual expression. In close relationships partners worked to establish compensatory mechanisms, but in less close relationships ill health provided an excuse to stop sex or deterred attempts to resolve difficulties. Most fundamentally, ill health may influence whether individuals have a partner with whom to have sex. CONCLUSION: The data show complex interactions between health, lifestyle and relationship factors that affect sexual activity/satisfaction. When dealing with sexual problems in older people, practitioners need to take account of individual lifestyle, needs and preferences

    The Genetics of Polycystic Ovary Syndrome: An Overview of Candidate Gene Systematic Reviews and Genome-Wide Association Studies

    Polycystic Ovary Syndrome (PCOS) is a complex condition with mechanisms likely to involve the interaction between genetics and lifestyle. Familial clustering of PCOS symptoms is well documented, providing evidence for a genetic contribution to the condition. This overview aims firstly to systematically summarise the current literature surrounding genetics and PCOS, and secondly, to assess the methodological quality of current systematic reviews and identify limitations. Four databases were searched to identify candidate gene systematic reviews, and quality was assessed with the AMSTAR tool. Genome-wide association studies (GWAS) were identified by a semi structured literature search. Of the candidate gene systematic reviews, 17 were of high to moderate quality and four were of low quality. A total of 19 gene loci have been associated with risk of PCOS in GWAS, and 11 of these have been replicated across two different ancestries.     Feeding the Worlth Healthily: the Challenge of Measuring the effects of Agriculture on Health

    Agricultural production, food systems and population health are intimately linked. While there is a strong evidence base to inform our knowledge of what constitutes a healthy human diet, we know little about actual food production or consumption in many populations and how developments in the food and agricultural system will affect dietary intake patterns and health. The paucity of information on food production and consumption is arguably most acute in low- and middle-income countries, where it is most urgently needed to monitor levels of under-nutrition, the health impacts of rapid dietary transition and the increasing ā€˜double burdenā€™ of nutrition-related disease. Food availability statistics based on food commodity production data are currently widely used as a proxy measure of national-level food consumption, but using data from the UK and Mexico we highlight the potential pitfalls of this approach. Despite limited resources for data collection, better systems of measurement are possible. Important drivers to improve collection systems may include efforts to meet international development goals and partnership with the private sector. A clearer understanding of the links between the agriculture and food system and population health will ensure that health becomes a critical driver of agricultural change
