Miliary Tuberculosis and the postpartum state
Journal ArticleIn 1985, a resurgence of tuberculosis began in the United States. In conjunction with this resurgence, there has been an increase in the number of atypical presentations of the disease. We recently treated a patient who had disseminated tuberculosis that became manifest in the postpartum state. Whereas postpartum progression of tuberculosis was a well-recognized clinical entity in the preantibiotic era, few reports have dealt with the issue recently. We present this case as a timely reminder of the many faces of tuberculosis
Being Ready to Treat Ebola Virus Disease Patients
As the outbreak of Ebola virus disease (EVD) in West Africa continues, clinical preparedness is needed in countries at risk for EVD (e.g., United States) and more fully equipped and supported clinical teams in those countries with epidemic spread of EVD in Africa. Clinical staff must approach the patient with a very deliberate focus on providing effective care while assuring personal safety. To do this, both individual health care providers and health systems must improve EVD care. Although formal guidance toward these goals exists from the World Health Organization, Medecin Sans Frontières, the Centers for Disease Control and Prevention, and other groups, some of the most critical lessons come from personal experience. Autistic Disorder in Patients with Williams-Beuren Syndrome: A Reconsideration of the Williams-Beuren Syndrome Phenotype
International audienceBackground: Williams-Beuren syndrome (WBS), a rare developmental disorder caused by deletion of contiguous genes at 7q11.23, has been characterized by strengths in socialization (overfriendliness) and communication (excessive talkativeness). WBS has been often considered as the polar opposite behavioral phenotype to autism. Our objective was to better understand the range of phenotypic expression in WBS and the relationship between WBS and autistic disorder. Methodology: The study was conducted on 9 French individuals aged from 4 to 37 years old with autistic disorder associated with WBS. Behavioral assessments were performed using Autism Diagnostic Interview-Revised (ADI-R) and Autism Diagnostic Observation Schedule (ADOS) scales. Molecular characterization of the WBS critical region was performed by FISH. Findings: FISH analysis indicated that all 9 patients displayed the common WBS deletion. All 9 patients met ADI-R and ADOS diagnostic criteria for autism, displaying stereotypies and severe impairments in social interaction and communication (including the absence of expressive language). Additionally, patients showed improvement in social communication over time. Conclusions: The results indicate that comorbid autism and WBS is more frequent than expected and suggest that the common WBS deletion can result in a continuum of social communication impairment, ranging from excessive talkativeness and overfriendliness to absence of verbal language and poor social relationships. Appreciation of the possible co-occurrence of WBS and autism challenges the common view that WBS represents the opposite behavioral phenotype of autism, and might lead to improved recognition of WBS in individuals diagnosed with autism
Preparing a Community Hospital to Manage Work-related Exposures to Infectious Agents in BioSafety Level 3 and 4 Laboratories
Construction of new BioSafety Level (BSL) 3 and 4 laboratories has raised concerns regarding provision of care to exposed workers because of healthcare worker (HCW) unfamiliarity with precautions required. When the National Institutes of Health began construction of a new BSL-4 laboratory in Hamilton, Montana, USA, in 2005, they contracted with St. Patrick Hospital in Missoula, Montana, for care of those exposed. A care and isolation unit is described. We developed a training program for HCWs that emphasized the optimal use of barrier precautions and used pathogen-specific modules and simulations with mannequins and fluorescent liquids that represented infectious body fluids. The facility and training led to increased willingness among HCWs to care for patients with all types of communicable diseases. This model may be useful for other hospitals, whether they support a BSL-4 facility, are in the proximity of a BSL-3 facility, or are interested in upgrading their facilities to prepare for exotic and novel infectious diseases
Challenges and perspectives for integral bridges in the UK: PLEXUS small-scale experiments
This study focuses on the investigation of the factors that have limited, so far, the development of a consistent design and assessment approach for integral bridges (IBs). This paper presents a review of previous research and current design practices for IBs, followed by an overview of monitoring studies in the laboratory and in the field. As part of the UK Collaboratorium for Research on Infrastructure and Cities–Priming Laboratory EXperiments on Infrastructure and Urban Systems experimental campaign, a small-scale 1 g physical experiment is described. The test aimed to simulate the soil–structure interaction arising from seasonal expansion and contraction of the bridge deck and assess the performance of different monitoring techniques. Pressure cells were used to measure the lateral stresses behind the abutment wall, particle image velocimetry was employed to monitor the soil behaviour behind the abutment and linear variable differential transformers were used to monitor the backfill surface movements. By combining the data from these instruments, a preliminary assessment of the soil–structure interaction behaviour of the idealised integral abutment under seasonal thermal loading has been obtained. These monitoring methods and the associated understanding of IB behaviour gained from the tests provide definitive evidence for the development of monitoring systems for larger-scale physical tests and field monitoring systems for IBs
The Special Pathogens Research Network: Enabling Research Readiness.
The 2013-2016 epidemic of Ebola virus disease (EVD) that originated in West Africa underscored many of the challenges to conducting clinical research during an ongoing infectious disease epidemic, both in the most affected countries of Guinea, Liberia, and Sierra Leone, as well as in the United States and Europe, where a total of 27 patients with EVD received care in biocontainment units. The Special Pathogens Research Network (SPRN) was established in the United States in November 2016 to provide an organizational structure to leverage the expertise of the 10 Regional Ebola and Other Special Pathogen Treatment Centers (RESPTCs); it was intended to develop and support infrastructure to improve readiness to conduct clinical research in the United States. The network enables the rapid activation and coordination of clinical research in the event of an epidemic and facilitates opportunities for multicenter research when the RESPTCs are actively caring for patients requiring a biocontainment unit. Here we provide an overview of opportunities identified in the clinical research infrastructure during the West Africa EVD epidemic and the SPRN activities to meet the ongoing challenges in the context of Ebola virus and other special pathogens
