Is porto sinusoidal vascular disease to be actively searched in patients with portal vein thrombosis?
Porto sinusoidal vascular liver disease (PSVD) and portal vein thrombosis (PVT) are distinct vascular liver diseases characterized, respectively, by an intrahepatic and a prehepatic obstacle to the flow in the liver portal system. PVT may also occur as a complication of the natural history of PSVD, especially if a prothrombotic condition coexists. In other cases, it is associated to local and systemic pro-thrombotic conditions, even if its cause remains unknown in up to 25% despite an active search. In our opinion, the presence of PSVD should be suspected in patients with PVT especially in those with PVT "sine causa" and the active search of this condition should be included in their diagnostic work-out. However, sometimes the diagnosis of pre-existing PSVD is very hard. Biopsy cannot be fully discriminant as similar histological data have been described in both conditions. Liver stiffness may help as it has been shown to be higher in PSVD than in "pure" PVT, due to the presence of sclerosis in the portal venous radicles observable in PSVD patients. Nevertheless, comparing liver stiffness between PVT and PSVD has until now been restricted to very limited series of patients. In conclusion, even if it is still totally hypothetical, our point of view may have clinical consequences, especially when deciding to perform a liver biopsy in patients with a higher liver stiffness and suspending the anticoagulation in patients with PVT and no detectable prothrombotic factors
Angiotensin type-2 (AT-2)-receptor activation reduces renal fibrosis in cyclosporine nephropathy: evidence for blood-pressure independent effect
Compound 21 (C21), selective agonist of AT2 receptors, shows antinflammatory effects in hypertension and nephroprotection in diabetes. The aim of this study was to evaluate the effects of C21 in cyclosporine nephropathy, which is characterized mainly by tubulo-interstitial fibrosis. Ten days before and during the experimental periods, low-salt diet was administered to Sprague Dawley rats. Cyclosporine-A (15mg/kg/day, i.p.) and cyclosporine-A plus C21 (0.3 mg/kg /day, i.p) were administered for 1 and 4 weeks. Control groups was left without any treatment. Blood pressure (plethysmographic method) and 24 hour albuminuria were measured once a week. At the end of the experiments, the kidneys were excised for histomorphometric analysis of renal fibrosis and for immunohistochemical evaluation of inflammatory infiltrates and type I and IV collagen expression.After 1 and 4 weeks, the rats treated with cyclosporine showed a significant increase (p <0.01) in blood pressure, no significant changes in albuminuria, a significant increase (p <0.01) in glomerular and tubulo-interstitial fibrosis and inflammatory infiltrates as compared to the control rats. Treatment with C21 did not modify the cyclosporine dependent increase of blood pressure, which was higher than in control rats, but after 4 weeks of treatment significantly reduced (p <0.01) glomerular and tubulo-interstitial fibrosis, type 1 collagen expression and macrophage infiltration, as compared to rats treated with cyclosporine.The administration of C21 showed a protective effect on cyclosporine nephropathy, decreasing renal fibrosis and macrophage infiltration. These data suggest that C21 may counteract tubulo-interstitial fibrosis, the most potent predictor of the progression of renal diseases
Rough-conduit flows and the existence of fully developed turbulence
It is widely believed that at high Reynolds number (Re) all turbulent flows
approach a state of "fully developed turbulence" defined by a unique,
Re-independent statistics of the velocity fluctuations. Yet direct measurements
of the velocity fluctuations have failed to yield clear-cut empirical evidence
of the existence of fully developed turbulence. Here we relate the friction
coefficient () of rough-conduit flows to the statistics of the velocity
fluctuations. In light of experimental measurements of , our results yield
unequivocal evidence of the existence of fully developed turbulence.Comment: 5 pages, 2 figure
A search for gravitational lensing in 38 X-ray selected clusters of galaxies
We present the results of a CCD imaging survey for gravitational lensing in a
sample of 38 X-ray-selected clusters of galaxies. Our sample consists of the
most X-ray luminous (Lx>= 2x10^{44} erg s^{-1}) clusters selected from the
Einstein Observatory Extended Medium Sensitivity Survey (EMSS) that are
observable from Mauna Kea (dec > -40deg). The sample spans a redshift range of
0.15 0.5. CCD images of the
clusters were obtained in excellent seeing. There is evidence of strong
gravitational lensing in the form of giant arcs (length l > 8'', axis ratio l/w
> 10) in 8 of the 38 clusters. Two additional clusters contain shorter arclets,
and 6 more clusters contain candidate arcs that require follow-up observations
to confirm their lensing origin. Since the survey does not have a uniform
surface brightness limit we do not draw any conclusion based on the statistics
of the arcs found. We note, however, that 60% (3 of 5) of the clusters with Lx
> 10^{45} erg s^{-1}, and none of the 15 clusters with Lx < 4x10^{44} erg
s^{-1} contain giant arcs, thereby confirming that high X-ray luminosity does
identify the most massive systems, and thus X-ray selection is the preferred
method for finding true, rich clusters at intermediate and high redshifts. The
observed geometry of the arcs, most of which are thin, have large axis ratios
(l/w > 10), and are aligned orthogonal to the optical major axes of the
clusters, indicate the cluster core mass density profiles must be compact
(steeper than isothermal). In several cases, however, there is also some
evidence, in the form of possible radial arcs, for density profiles with finite

Macroscopic effects of the spectral structure in turbulent flows
Two aspects of turbulent flows have been the subject of extensive, split
research efforts: macroscopic properties, such as the frictional drag
experienced by a flow past a wall, and the turbulent spectrum. The turbulent
spectrum may be said to represent the fabric of a turbulent state; in practice
it is a power law of exponent \alpha (the "spectral exponent") that gives the
revolving velocity of a turbulent fluctuation (or "eddy") of size s as a
function of s. The link, if any, between macroscopic properties and the
turbulent spectrum remains missing. Might it be found by contrasting the
frictional drag in flows with differing types of spectra? Here we perform
unprecedented measurements of the frictional drag in soap-film flows, where the
spectral exponent \alpha = 3 and compare the results with the frictional drag
in pipe flows, where the spectral exponent \alpha = 5/3. For moderate values of
the Reynolds number Re (a measure of the strength of the turbulence), we find
that in soap-film flows the frictional drag scales as Re^{-1/2}, whereas in
pipe flows the frictional drag scales as Re^{-1/4} . Each of these scalings may
be predicted from the attendant value of \alpha by using a new theory, in which
the frictional drag is explicitly linked to the turbulent spectrum. Our work
indicates that in turbulence, as in continuous phase transitions, macroscopic
properties are governed by the spectral structure of the fluctuations.Comment: 6 pages, 3 figure
Clinical significance of epithelial-to-mesenchymal transition in laryngeal carcinoma: Its role in the different subsites
Background: During epithelial-to-mesenchymal transition, cancer cells lose adhesion capacity gaining migratory properties. The role of the process on prognosis has been evaluated in 50 cases of laryngeal carcinoma. Methods: E-cadherin, N-cadherin, β-catenin, α-catenin, γ-catenin, caveolin-1, and vimentin immunohistochemical expression were evaluated using a double score based on staining intensity and cellular localization. Results: Cytoplasmic E-cadherin and α/γ catenin staining were associated with a decrease in survival, cytoplasmic β-catenin was associated with advanced stage, and N-cadherin and vimentin expression were associated with poor differentiation and tumor relapse. On the basis of cancer cells, epithelial or mesenchymal morphological and immunophenotypic similarity we identified 4 main subgroups correlated with a transition to a more undifferentiated phenotype, which have a different pattern of relapse and survival. Conclusion: The negative prognostic role of epithelial-to-mesenchymal transition has been confirmed and a predictive role in glottic tumors has been suggested, leading us to propose epithelial-to-mesenchymal transition as an additional adverse feature in laryngeal carcinoma
