A Pharmacological and Toxicological Profile of Silver as an Antimicrobial Agent in Medical Devices
Silver is used widely in wound dressings and medical devices as a broad-spectrum antibiotic. Metallic silver and most inorganic silver compounds ionise in moisture, body fluids, and secretions to release biologically active Ag+. The ion is absorbed into the systemic circulation from the diet and drinking water, by inhalation and through intraparenteral administration. Percutaneous absorption of Ag+ through intact or damaged skin is low. Ag+ binds strongly to metallothionein, albumins, and macroglobulins and is metabolised to all tissues other than the brain and the central nervous system. Silver sulphide or silver selenide precipitates, bound lysosomally in soft tissues, are inert and not associated with an irreversible toxic change. Argyria and argyrosis are the principle effects associated with heavy deposition of insoluble silver precipitates in the dermis and cornea/conjunctiva. Whilst these changes may be profoundly disfiguring and persistent, they are not associated with pathological damage in any tissue. The present paper discusses the mechanisms of absorption and metabolism of silver in the human body, presumed mechanisms of argyria and argyrosis, and the elimination of silver-protein complexes in the bile and urine. Minimum blood silver levels consistent with early signs of argyria or argyrosis are not known. Silver allergy does occur but the extent of the problem is not known. Numerical modelling of liquid metal transport in partially molten H5 ordinary chondrite
An equation-based model of liquid metal segregation in Fe-bearing chondrite meteorites is presented. Textural data from natural samples provide the input conditions. Initial results confirm porous flow of Fe-Ni-S liquid alloy as an important metal segregation mechanism in planetary interiors
Immunohistochemical detection of ERβ in breast cancer: towards more detailed receptor profiling?
Oestrogen receptor (ER) is used routinely to predict endocrine responsiveness in patients with breast cancer. A second ER, ERβ has been described but its significance remains undefined; most studies have described mRNA levels rather than protein expression. Love, rights and solidarity: studying children's participation using Honneth's theory of recognition
Recent attempts to theorize children’s participation have drawn on a wide range of ideas, concepts and models from political and social theory. The aim of this article is to explore the specific usefulness of Honneth’s theory of a ‘struggle for recognition’ in thinking about this area of practice. The article identifies what is distinctive about Honneth’s theory of recognition, and how it differs from other theories of recognition. It then considers the relevance of Honneth’s conceptual framework to the social position of children, including those who may be involved in a variety of ‘participatory’ activities.
It looks at how useful Honneth’s ideas are in direct engagement with young people’s praxis, drawing on ethnographic research with members of a children and young people’s forum. The article concludes by reflecting on the implications of this theoretical approach and the further questions which it opens up for theories of participation and of adult–child relations more generally
Auto-driven Photo Elicitation Interviews in Research with Children: Ethical and Practical Considerations
Children can sometimes find it difficult to articulate their experiences if they have to rely solely on words. Giving children the opportunity to use arts-based research approaches can support their participation in research and create a bridge that enables them to express their perspectives and feelings. This paper focuses on the ethical and practical considerations when using photo elicitation interviews (PEI) in research with children. The discussion and examples provided are drawn from an international study that used auto-driven PEI, where photographs are taken by children themselves, to explore children’s experiences of living with a chronic condition and the impact condition management may have on their everyday lives. In this paper we critically explore the issues arising from our use of PEI including children’s participation and engagement, balancing power and control, and keeping children safe. The main areas of focus for the paper are how PEI provided a means of shifting control; how setting photographic boundaries influenced our PEI study with children; and how we addressed risks associated with the method. Our experience shows that PEI is an engaging and valuable research method, providing a powerful medium for obtaining rich data with children. However, PEI is challenging and it requires researchers to conscientiously address ethical and practical aspects that extend beyond those inherent to standard (words-alone) interviews
Longitudinal changes in intracardiac repolarization lability in patients with implantable cardioverter-defibrillator.
PMC3740232Background: While it is known that elevated baseline intracardiac repolarization lability is associated with the risk of fast ventricular tachycardia (FVT)/ventricular fibrillation (VF), the effect of its longitudinal changes on the risk of FVT/VF is unknown. Methods and Results: Near-field (NF) right ventricular (RV) intracardiac electrograms (EGMs) were recorded every 3-6 months at rest in 248 patients with structural heart disease [mean age 61.2 ± 13.3; 185(75%) male; 162(65.3%) ischemic cardiomyopathy] and implanted cardioverter-defibrillator (ICD) or cardiac resynchronization therapy defibrillator (CRT-D) [201 (81%) primary prevention]. Intracardiac beat-to-beat QT variability index (QTVINF) was measured on NF RV EGM. During the first study phase (median 18 months), participants made on average 2.4 visits. Then remote follow-up was continued for an additional median period of 3 years. Average QTVINF did not change during the first year after ICD implantation (-0.342 ± 0.603 at baseline vs. -0.262 ± 0.552 at 6 months vs. -0.334 ± 0.603 at 12 months); however, it decreased thereafter (-0.510 ± 0.603 at 18 months; P = 0.042). Adjusted population-averaged GEE model showed that the odds of developing FVT/VF increased by 75% for each 1 unit increase in QTVINF. (OR 1.75 [95%CI 1.05-2.92]; P = 0.031). However, individual patient-specific QTVINF trends (increasing, decreasing, flat) varied from patient to patient. For a given patient, the odds of developing FVT/VF were not associated with increasing or decreasing QTVINF over time [OR 1.27; (95%CI 0.05-30.10); P = 0.881]. Conclusion: While on average the odds of FVT/VF increased with an increase in QTVINF, patient-specific longitudinal trends in QTVINF did not affect the odds of FVT/VF.JH Libraries Open Access Fun
Student civic participation in school: what makes a difference in Ireland?
Article 12 of the United Nations Convention on the Rights of the Child provides for the views of the child to be given due weight in accordance with the child’s age and maturity. Legislation in Ireland recognizes the rights of children to have a voice in educational matters. Based on a sample of 2838 14-year-olds in Ireland and using questionnaire data from the International Civic and Citizenship Education Study, this paper uses a multilevel model to examine the student and school characteristics associated with civic participation at school. Results indicate that boys have lower levels of civic participation at school than girls, but among boys only, civic participation at school varies in accordance with levels of perceived influence on decision making. Findings are discussed in the context of the rights of children to participate in decisions that affect them and with reference to Bandura’s social cognitive theory
Childhood Labor in India: issues and complexities
It is estimated that more than 12 million children in India under the age of 14 engage in paid labor at least part time, due mostly to economic reasons. Dominant discourses about childhood however conceptualize childhood labor not only as unethical but as exploitation. This article explored will the tensions between Western notions of childhood (within which paid labor is considered taboo) and the realities of children's lives in India, arguing that childhood labor must be contextualized and understood not only as a colonial legacy but also as part of its socio-cultural context. The author argues that separating children from the world of work fosters a culture of childhood that emphasizes entitlement over participation and privileges the rights of the consumer over children's rights as citizens
