Hydrogen site occupancy and strength of forces in nano-sized metal hydrides
The dipole force components in nano-sized metal hydrides are quantitatively
determined with curvature and x-ray diffraction measurements. Ab-initio density
functional theory is used to calculate the dipole components and the symmetry
of the strain field. The hydrogen occupancy in a 100 nm thick V film is shown
to be tetrahedral with a slight asymmetry at low concentration and a transition
to octahedral occupancy is shown to take place at around 0.07 [H/V] at 360 K.
When the thickness of the V layer is reduced to 3 nm and biaxially strained, in
a Fe_0.5V_0.5/V superlattice, the hydrogen unequivocally occupies octahedral
z-like sites, even at and below concentrations of 0.02 [H/V]
Safety and tolerability of NXY-059 for acute intracerebral hemorrhage: the CHANT trial
<p><b>Background and Purpose:</b> NXY-059 is a free radical-trapping neuroprotectant developed for use in acute ischemic stroke. To facilitate prompt administration of treatment, potentially before neuroimaging, we investigated the safety of NXY-059 in patients with intracerebral hemorrhage (ICH).</p>
<p><b>Methods:</b> We randomized 607 patients within 6 hours of acute ICH to receive 2270 mg intravenous NXY-059 over 1 hour and then up to 960 mg/h over 71 hours, or matching placebo, in addition to standard care. The primary outcome was safety: the mortality and the frequency of adverse events, and the change from baseline for a variety of serum, imaging, and electrophysiological measurements. We also studied the overall distribution of disability scores on the modified Rankin Scale (mRS) and the Barthel index.</p>
<p><b>Results:</b> We treated 300 patients with NXY-059 and 303 with placebo. Treatment groups were well matched for prognostic variables including Glasgow Coma Scale, risk factors, and age. The mean National Institute of Health Stroke Scale score on admission was 14 in both groups. The baseline hemorrhage volume was 22.4±20.1 mL in the NXY-059 group and 23.3±22.8 mL in the placebo group (mean±SD). Most hemorrhages were related to hypertension or anticoagulant use. Mortality was similar in both groups: 20.3% for NXY-059 and 19.8% for placebo-treated patients. The proportion of patients who experienced an adverse event was the same for both groups, whereas for serious adverse events the proportion was slightly higher in the NXY-059 group. However, no pattern emerged to indicate a safety concern. Serum potassium fell transiently in both groups, lower in the NXY-059 group. There were no differences in 3-month function, disability, or neurological deficit scores. The odds ratio for an improved outcome in 3-month mRS scores in the NXY-059 group was 1.01 (95% CI 0.75, 1.35).</p>
<p><b>Conclusions:</b> NXY-059 given within 6 hours of acute ICH has a good safety and tolerability profile, with no adverse effect on important clinical outcomes.</p>
Convergence towards an asymptotic shape in first-passage percolation on cone-like subgraphs of the integer lattice
In first-passage percolation on the integer lattice, the Shape Theorem
provides precise conditions for convergence of the set of sites reachable
within a given time from the origin, once rescaled, to a compact and convex
limiting shape. Here, we address convergence towards an asymptotic shape for
cone-like subgraphs of the lattice, where . In particular, we
identify the asymptotic shapes associated to these graphs as restrictions of
the asymptotic shape of the lattice. Apart from providing necessary and
sufficient conditions for - and almost sure convergence towards this
shape, we investigate also stronger notions such as complete convergence and

Morphology of the earliest reconstructable tetrapod Parmastega aelidae.
The known diversity of tetrapods of the Devonian period has increased markedly in recent decades, but their fossil record consists mostly of tantalizing fragments1-15. The framework for interpreting the morphology and palaeobiology of Devonian tetrapods is dominated by the near complete fossils of Ichthyostega and Acanthostega; the less complete, but partly reconstructable, Ventastega and Tulerpeton have supporting roles2,4,16-34. All four of these genera date to the late Famennian age (about 365-359 million years ago)-they are 10 million years younger than the earliest known tetrapod fragments5,10, and nearly 30 million years younger than the oldest known tetrapod footprints35. Here we describe Parmastega aelidae gen. et sp. nov., a tetrapod from Russia dated to the earliest Famennian age (about 372 million years ago), represented by three-dimensional material that enables the reconstruction of the skull and shoulder girdle. The raised orbits, lateral line canals and weakly ossified postcranial skeleton of P. aelidae suggest a largely aquatic, surface-cruising animal. In Bayesian and parsimony-based phylogenetic analyses, the majority of trees place Parmastega as a sister group to all other tetrapods
Dilemmas and paradoxes in providing and changing antenatal care: A study of nurses and midwives in rural Zimbabwe
This paper describes the experiences of caregivers in a rural district in Zimbabwe, in caring for
pregnant women within a context of changing antenatal care routines. Data were generated using
individual interviews with 18 nurses and midwives.
The caregivers experienced their working situation as stressful and frustrating due to high staff
turnover, inconsistent policies, parallel programmes and limited resources, including time. They also
faced difficulties when implementing some of the proposed changes. Furthermore, the caregivers had
to deal with the pressure and resistance from the pregnant women, whose reasoning and rationale
for using care appeared different from those of the health professionals.
In light of the above, we stress the necessity for reflecting on and including the experiences and
perspectives of caregivers and the users of care, as well as their contexts and realities, when
implementing change
