    The effect of a multisensory approach to Spanish instruction on the foreign language proficiency of students with learning disabilities

    The purpose of the study was to investigate the effect of a multisensory approach to Spanish instruction founded in Orton-Gillingham on the (1) foreign language writing, and (2) foreign language reading comprehension of students with learning disabilities in the middle school Spanish classroom. Additionally, the study inquired about the students\u27 satisfaction with the Orton-Gillingham approach to learning to read in Spanish. Four middle school Spanish students with SLD, three male and one female, participated in the study. A single subject ABAB design was used. During the baseline phases, students received Spanish instruction in the traditional mainstream classroom. During the intervention, students received instruction through an alphabetic based multisensory program for teaching reading and spelling in the target language. Daily assessment scores were collected on reading comprehension and written expression across all phases. Results show all four participants improved their reading comprehension and writing scores from baseline data. The student satisfaction surveys suggest that students enjoyed the use of a multisensory approach to Spanish instruction. Further research is suggested investigating the effect of a multisensory approach on Spanish students with learning disabilities

    Breast metastasis and lung large-cell neuroendocrine carcinoma: first clinical observation

    The lung Large-cell neuroendocrine carcinoma (LCNEC) is a very rare aggressive neuroendocrine tumor with a high propensy to metastasize and very poor prognosis. We report an atypical presentation of lung large-cell neuroendocrine carcinoma was diagnosed from a metastatic nodule on the breast. Our patient is a 59 years-old woman that presented in March 2014 non productive cough. A CT scan showed multiple brain, lung, adrenal gland and liver secondary lesions; moreover, it revealed a breast right nodule near the chest measuring 1.8 cm. The breast nodule and a lung lesions were biopsied and their histology and molecular diagnosis were LCNEC of the lung. To our knowledge, this is the first documented case of breast metastasis from LCNEC of the lung. Furthermore, breast metastasis from extramammary malignancy is uncommon and its diagnosis is difficult but important for proper management and prediction of prognosis. Therefore, a careful clinical history with a thorough clinical examination is needed to make the correct diagnosis. Moreover, metastasis to the breast should be considered in any patient with a known primary malignant tumor history who presents with a breast lump. Anyhow, pathological examination should be performed to differentiate the primary breast cancer from metastatic tumor. Therefore, an accurate diagnosis of breast metastases may not only avoid unnecessary breast resection, more importantly it is crucial to determine an appropriate and systemic treatment

    FOREIGN LANGUAGE LEARNING OF STUDENTS WITH LANGUAGE LEARNING DISABILITIES: AN ACTIVITY THEORY PERSPECTIVE OF THREE MIDDLE SCHOOL STUDENTS

    The purpose of this study is to contribute to the existing literature on the foreign language learning of students with LLDs by conducting a comprehensive investigation of the performance of three students in middle school foreign language classes in a suburban school district in southwestern Pennsylvania. More specifically, this study documents (a) student writing over time, (b) student and teacher perceptions, (c) classroom interactions, and (d) the instructional practices that teachers use to accommodate these three middle school students with (LLDs) during whole group class instruction. As previous research has demonstrated a relationship between classroom interactions and student performance in a foreign language, this study also examines how the writing performance of three students with LLDs relates to the classroom interactions and instructional practices that their teachers implement to accommodate students with LLDs. To conduct this study, several data sources were collected and analyzed including student writing assessments over time, student and teacher interviews, and classroom observations. To link these multiple sources of data that were collected, an Activity Theory was applied as an analytical framework. This sociocultural theoretical framework was applied to this study because it provides a method to explain the multiple aspects of a child's environment that contribute to his/her learning. By referring to Activity Theory, relationships between classroom components such as classroom interactions, student and teacher perceptions, mediational tools, and student performance were established and explained. Although this study examines a small group of three students and its specificity prevents generalizing to all students with LLDs who participate in foreign language programs, the findings and implications contribute to a grounded conceptual understanding of students with LLDs and their abilities and challenges in learning a foreign language. By contributing to the conceptual understanding of the foreign language learning of students with LLDs, the findings of this study assist school districts in making decisions regarding enrollment of students with LLDs in foreign language programs, designing curricula and instructional accommodations for students with LLDs, developing professional development for foreign language teachers, and informing the public on the issue of students with LLDs and foreign language learning

    A CLINICAL STUDY OF PATIENTS WITH IDIOPATHIC THROMBOCYTOPENIC PURPURA

    OBJECTIVE To study the incidence, age and sex distribution among adults, various modes of presentation, correlation between thrombocytopenia and bleeding manifestations and various modalities of treatment of Idiopathic Thrombocytopenic Purpura in a tertiary care hospital in India.MATERIALS AND METHODSProspective study consisting of 4o cases of ITP admitted to Kasturba Medical College Hospital, Manipal from November 2005 to March 2007. Patients above 14 years of age admitted with thrombocytopenia in this institution were screened based on detailed clinical history, physical examination and laboratory investigations.RESULTSThe maximum incidence was in the 3 rd decade of life accounting for 27.5% of the patients. Ratio of male to female was 1:1.9 with female preponderance. Females in 3 rd decade had maximum incidence while males in 4th decade have maximum incidence. Majority 12 (85.71%) of male patients and 15(57.69%) of female patients  developed purpura during the course of disease. The mean count was 17.8X109/L with range between 2.0 X109/L to 76 X109/L. Cutaneous bleeding spots were found to be assosiated with counts above 25000X109/L and hematuria with lowest counts around 4000 X109/L. 20 (50%) of the above 40 patients responded to corticposteroids alone and did not have any further relapse. Among the remaining patients 3 (7.5%) had a relapse of symptoms within next 6 months and responded to repeat prednisolone started at 1mg/kg. 2 (5%) patients were given steroids tapering dose with IV Immune globulins for initial 5 days as therapy to which they responded.CONCLUSIONIdiopathic thrombocytopenia is 1.9 times more common in females than males. Most common presentation is bleeding spots over body. Bleeding manifestations are more common with thrombocytopenia less than 30000/mm3. Corticosteroids are the mainstay in treatment. Complete remission is seen in up to 57.5% of the patients. Splenectomy is the second modality of treatment in ITP. Complete and sustained remission is seen 75% of patients. Â
