Comparing surgically induced astigmatism calculated by means of simulated keratometry versus total corneal refractive power
Purpose: To evaluate surgically induced astigmatism as computed by means of either simulated keratometry (KSIM) or total corneal refractive power (TCRP) after temporal incisions.
Methods: Prospective observational study including 36 right eyes undergoing cataract surgery. Astigmatism was measured preoperatively during the 3-month follow-up period using Pentacam. Surgically induced astigmatism was computed considering anterior corneal surface astigmatism at 3mm with KSIM and considering both corneal surfaces with TCRP from 1 to 8mm (TCRP3 for 3mm). The eyes under study were divided into two balanced groups: LOW with KSIM astigmatism <0.90D and HIGH with KSIM astigmatism ≥0.90D. Resulting surgically induced astigmatism values were compared across groups and measuring techniques by means of flattening, steepening, and torque analysis.
Results: Mean surgically induced astigmatism was higher in the HIGH group (0.31D @ 102°) than in the LOW group (0.04 D @ 16°). The temporal incision resulted in a steepening in the HIGH group of 0.15 D @ 90°, as estimated with KSIM, versus 0.28 D @ 90° with TCRP3, but no significant differences were found for the steepening in the LOW group or for the torque in either group. Differences between KSIM- and TCRP3-based surgically induced astigmatism values were negligible in LOW group.
Conclusion: Surgically induced astigmatism was considerably higher in the high-astigmatism group and its value was underestimated with the KSIM approach. Eyes having low astigmatism should not be included for computing the surgically
induced astigmatism because steepening would be underestimated

Central Mini-plication of the Medial Rectus for Convergence Insufficiency
Five cases with a mean (± SD) age of 61 (12.02) years are described to study the outcomes of treatment with central mini-plication of the medial rectus (MR) muscles in adult convergence insufficiency with diplopia and near exotropia: mean preoperative deviation: 18 (± 2) pd. Surgical outcome was considered to be favorable when diplopia and symptoms were resolved and final exotropia at near was ≤8 pd at the end of follow-up.
A central mini-plication of the medial rectus (MR) muscles was performed in 5 patients (4 unilateral). Overcorrection at distance vision was recorded in 3 cases; this resolved by 3 months postoperatively. There was not overcorrection at near vision in any case. None of the cases operated on had overcorrection at the end of follow-up. The final horizontal deviation was ≤8 pd at near vision, except for 1 case. Symptoms and diplopia resolved in every case but 2/5 required reoperations. The mean follow-up was 8 (2.12) months. Central mini-plication of 1 or 2 medial rectus muscles can improve the symptoms and signs of exotropia associated with convergence insufficiency when exercises and the prisms are rejected by the patients and when these approaches have not solved the problem
Impacto del apoyo social y la morbilidad psíquica en la calidad de vida en pacientes tratados con antirretrovirales

therapy. The aim of this study is to analyse the existing relation between the psychic morbidity
and social support and the quality of life. Besides this, the paper analyses the buffer rol that social
support plays on the psychic morbidity in these patients. We studied 320 HIV+ patients in truatment
with antiretrovirals, who attended the infectious disease services of four hospitals of the Autonomous
Andalusian Community. Being associated a better quality of life to an absence of psychic morbidity
and to the presence of social support, it is observed the relevant buffer role that the social support like
shock absorber of the psychic morbidity in this one type of patients. These results show the importance

psÌquica en estos pacientes. Se estudiaron a 320 pacientes VIH positivos en tratamiento con antirretrovirales,
que acudieron a las consultas externas del servicio de infecciosos de cuatro hospitales
de la comunidad autÛnoma andaluza. AsÌ, una mejor calidad de vida se asociÛ a una ausencia de morbilidad
psÌquica y a la presencia de apoyo social, a la vez que se observa el papel fundamental que juega
el apoyo social como amortiguador de la morbilidad psÌquica en este tipo de pacientes. Estos resultados
ponen de relieve la importancia que los factores psicosociales tienen durante el transcurso de
enfermedades de car·cter crÛnico
