NATIONAL QUALIFICATIONS SYSTEM IN RUSSIA – AN EPISTEMOLOGICAL PERSPECTIVE
Introduction. The article offers an insight into an epistemological perspective of establishing the national qualifications system in Russia that is critical for creating a “market of qualifications in the country that would be adequate to the current context.The aim of the research was to examine the context and factors underpinning the formation and evolution of the structure of knowledge relating to the national qualification system in Russia.Methodology and research methods. The methods applied comprise a mixed-method approach that included: review and analysis of official documents, semi-structured background and individual interviews with key stakeholders (line ministries, experts from industry and qualifications awarding bodies etc.) and desk research.Results and scientific novelty. The performed analysis of documents and of research data has revealed that the process of generating and enhancing knowledge about the national qualifications system semantically and methodologically is in broad terms contingent on the needs and objectives of social and economic development, and in narrower terms – on the development of occupational standards that underpin the standards of vocational and higher education, as well as on the enhanced institutionalisation of employers’ involvement in the training of personnel and in the search of effective mechanisms and instruments of independent assessment of qualifications.It also offers an insight into current key paradigmatic gaps in the NQF-related epistemology in Russia that account for the specificity of the observed state of play and for the key conceptual epistemological contradiction that has been identified by the authors as the opposition of and conflict between occupational and education qualifications. The authors propose ways out of the situation based on international practices in this area starting from aligning the terminology and conceptual approaches with international ones. The performed epistemological research and the conclusions made by the authors contribute to the national qualifications system discourse. It is stressed that core principles underpinning the development of the national qualifications system should be adapted to the social and economic and local factors.Practical significance. The material of the article can be used by policy-makers dealing with the NQS development and implementation of the NQS.Введение. В статье рассматривается эпистемологическая перспектива становления в России национальной системы квалификаций, необходимой для появления в стране отвечающего современным реалиям «рынка квалификаций». Цель публикации – обсуждение содержания, факторов формирования и эволюции знаний об аспектах функционирования национальной системы квалификаций в российских условиях. Методология и методики. В ходе работы применялись комплексный подход к объекту изучения, включающий такие методы исследования, как обзор и анализ официальных документов, полуофициальные опросы и индивидуальные интервью с представителями наиболее заинтересованных сторон – руководителями и сотрудниками отраслевых министерств, отраслевыми экспертами, специалистами структур по присуждению квалификаций и т. д.Результаты и научная новизна. Проведенный анализ документальных и научных источников показал, что процесс генерирования и совершенствования знаний о системе национальных квалификаций семантически и методологически связан в широком понимании с потребностями и целями социально-экономического развития; в более узком – с разработкой профессиональных стандартов, которые должны служить основой стандартов профессионального и высшего образования, а также с усилением институционализации участия работодателей в подготовке кадров и поиске эффективных механизмов и инструментария независимой оценки квалификаций и компетенций. Вскрыты имеющиеся в настоящее время парадигматические пробелы в системе знаний о национальной рамке квалификаций, порождающие проблемы дальнейшего развития данного направления в России. В качестве узлового концептуального эпистемологического противоречия выделено неправомерное противопоставление профессиональных и образовательных квалификаций. С опорой на зарубежный практический опыт предложены пути выхода из сложившейся ситуации, в частности, рекомендуется прежде всего согласовать терминологию и концептуальные подходы с международными нормами. Предпринятое в контексте эпистемологии исследование и сделанные авторами выводы расширяют дискурс о национальной системе квалификаций; подчеркивается, что при соблюдении общих принципов разработки национальной системы квалификаций важно учитывать специфические внутригосударственные социально-экономические и территориальные факторы. Практическая значимость. Материалы статьи могут быть использованы в практике отечественных специалистов, ответственных за создание инфраструктуры и внедрение национальной системы квалификаций и формирующих политику в этой области
Dissemination of blaIMP-8 among Enterobacteriaceae isolates from a Buenos Aires Hospital
Entre agosto de 2008 y diciembre de 2011 se detectaron en el Hospital Interzonal General de Agudos «Evita» de Lanús 6 aislamientos de enterobacterias productoras de metalo- β-lactamasas, distribuidos en tres especies: Enterobacter cloacae (4), Klebsiella oxytoca (1) y Citrobacter freundii (1). Los seis aislamientos presentaron un perfi l de multirresistencia y se confi rmó la presencia del gen blaIMP-8. Cinco aislamientos además expresaron la β-lactamasa de espectro extendido PER-2. El gen blaIMP-8 fue hallado como primer casete de un integrón de clase 1. Sin embargo, la secuencia 3´ conservada no pudo detectarse en tres aislamientos. En todos los casos, el gen blaIMP-8 fue transferido por conjugación a Escherichia coli resistente a azida. Mediante PFGE se observó que los cuatro aislamientos de E. cloacae no estuvieron genéticamente relacionados. Estos son los primeros hallazgos de metalo-β-lactamasas en la institución, que sugieren una posible diseminación horizontal del gen blaIMP-8 intra e interespecies.From August 2008 to December 2011, six metallo-β-lactamase-producing isolates, four Enterobacter cloacae, one Klebsiella oxytoca and one Citrobacter freundii, were detected at Hospital Interzonal General de Agudos “Evita” in Lanús. All six isolates showed multiresistant profi les and the presence of the blaIMP-8 gene. Five isolates also expressed PER-2 extended spectrum β-lactamase. The blaIMP-8 gene was found as the fi rst cassette in a class 1 integron. However, the 3´ conserved sequence could not be detected in three isolates. In all cases, blaIMP-8 was transferred by conjugation to azide-resistant Escherichia coli J53. PFGE analysis revealed that the four E. cloacae isolates were not genetically related. These are the fi rst metallo-β-lactamases detected in this institution and our results suggest a possible intra- and inter-species horizontal dissemination of blaIMP-8.Fil: Togneri, Ana M.. Calculating the true costs of protein sources by integrating environmental costs and market prices
The food sector is responsible for a great part of the environmental impact of our society (for instance, according to the UN, about a third of all human-made greenhouse gas emissions are linked to this sector) and protein sources, as one of the main food groups, have a particularly significant impact on the environment. Understanding the environmental and economic impacts of dietary choices is crucial, especially proteins choices, a main food source. Market prices alone do not comprehensively represent the true costs for society. True Cost Accounting is a methodology that quantifies the comprehensive economic, environmental, and social costs. Calculating the hidden environmental costs of the different alternative proteins helps to inform the public about the environmental consequences of their dietary choices. Adjusted prices, which integrate market prices with hidden environmental costs of protein alternative sources, including both animal-based and plant-based options, were estimated. In determining the true cost of proteins, assigning monetary values to environmental impacts is essential. Calculating environmental costs and adjusting the price of proteins provide a more accurate reflection of their true cost by accounting for the environmental externalities associated with protein production. A life cycle approach was applied, considering both a conventional mass-based functional unit and a proposed protein-content-based functional unit, which integrates the source´s efficiency to deliver protein nutrient. In a mass-based calculation, beef and lamb production consistently demonstrate the highest adjusted costs, amounting to 33.72€ per kg of source, while plant-based protein sources typically exhibit lower adjusted costs, with an average of 12.31€ per kg of source. In between, seafood ranks fourth at 26.41€ per kg of source. However, when the calculation of the adjusted price is based on real protein content, seafood commands the highest value, reaching 179.97€ per kg of protein, whereas beef and lamb drop to the third position at 131.76€ per kg of protein, and plant-based options Increase to 116.32€ per kg of protein. In summary, our study emphasizes the significance of informed dietary choices that account for both environmental sustainability and economic factors, and the need to use proper methodologies for the quantitative accounting. High Estradiol Differentially Affects the Expression of the Glucose Transporter Type 4 in Pelvic Floor Muscles of Rats
Purpose To characterize the relationship between serum estradiol levels and the expression of glucose transporter type 4 (Glut4) in the pubococcygeus and iliococcygeus muscles in female rats. Methods The muscles were excised from virgin rats during the metestrus and proestrus stages of the estrous cycle, and from sham and ovariectomized rats implanted with empty or estradiol benzoate–filled capsules. The expression of estrogen receptors (ERs) was inspected in the muscles at metestrus and proestrus. Relative Glut4 expression, glycogen content, and serum glucose levels were measured. Appropriate statistical tests were done to identify significant differences (P≤0.05). Results The pubococcygeus and iliococcygeus muscles expressed ERα and ERβ. Glut4 expression and glycogen content in the pubococcygeus muscle were higher at proestrus than at metestrus. No significant changes were observed in the iliococcygeus muscle. In ovariectomized rats, the administration of estradiol benzoate increased Glut4 expression and glycogen content in the pubococcygeus muscle alone. Conclusions High serum estradiol levels increased Glut4 expression and glycogen content in the pubococcygeus muscle, but not in the iliococcygeus muscle

Es importante establecer y visibilizar a los estudiantes los beneficios relacionados con
la formación en investigación, dentro de los cuales encontramos el fortalecimiento de
las capacidades de liderazgo así como el compromiso activo y las experiencias en
independencia y colaboración. Así mismo, la formación integral hacia una mayor
apreciación del valor de la literatura disciplinaria, generando de esta manera habilidades
de pensamiento crítico, indagación y análisis. Además, esto permite forjar la confianza
en sí mismo para presentar las propias ideas a la comunidad, permitiendo al estudiante
la preparación de futuras actividades académicas, incluidos estudios de posgrado.
La investigación formativa tiene como propósito la difusión de la información existente
y permitir que el estudiante la integre como conocimientos, considerándolo como un
aprendizaje permanente y necesario. Uno de los principales problemas que debe
enfrentar la investigación formativa es el número de docentes con las capacidades
necesarias para generar en el estudiante capacidades investigativas, exigiendo al
profesor universitario adoptar una actitud contraria al objeto de enseñanza, generando

Clima y salud en la Argentina: diagnóstico de situación 2018

Departamento de Ciencias de la Atmósfera y los Océanos; Argentina.El presente diagnóstico de situación sobre Clima y Salud en Argentina fue realizado por la Mesa de Trabajo sobre Cambio Climático de la Secretaría de Gobierno de Salud.
Se realizó un estudio de tipo descriptivo, en el cual se presenta por un lado el comportamiento de las variables meteorológicas y por otro, algunos eventos seleccionados en salud. Asimismo, se desarrolla el estado del conocimiento sobre la relación entre el clima y la salud en Argentina en el período comprendido entre los años 2005 y 2017.
La descripción climática fue realizada por el Departamento de Climatología del Servicio Meteorológico Nacional (SMN). Las estadísticas se basan en valores medios (promedio) del periodo 1981-2010.
Los efectos del clima sobre la salud humana fueron estudiados a partir de su clasificación en efectos directos e indirectos, entendiendo como efectos directos aquellos ocurridos a raíz de la ocurrencia de eventos meteorológicos extremos tales como olas de frío y calor, inundaciones, sequías y vientos fuertes. Para el análisis de estos eventos se realizó el análisis de la frecuencia y distribución espacial de los eventos extremos para el período 2005-2017.
Para el estudio de las patologías sensible al clima se priorizaron las patologías más relevantes para ser descriptas en base a la evidencia científica. Se realizó una descripción de la evolución temporal de los casos entre 2005 y 2017, a partir de los datos obtenidos de la vigilancia clínica (a través del módulo C2 del SNVS).
Por último se presentan las tendencias y retos que afrontará el sistema sanitario debido al cambio climático
Acceptance of living liver donation among medical students: A multicenter stratified study from Spain
AIM:
To analyze the attitude of Spanish medical students toward living liver donation (LLD) and to establish which factors have an influence on this attitude.
METHODS:
STUDY TYPE:
A sociological, interdisciplinary, multicenter and observational study.
STUDY POPULATION:
Medical students enrolled in Spain (n = 34000) in the university academic year 2010-2011.
SAMPLE SIZE:
A sample of 9598 students stratified by geographical area and academic year. Instrument used to measure attitude: A validated questionnaire (PCID-DVH RIOS) was self-administered and completed anonymously. Data collection procedure: Randomly selected medical schools. The questionnaire was applied to each academic year at compulsory sessions.
STATISTICAL ANALYSIS:
Student´s t test, ?(2) test and logistic regression analysis.
RESULTS:
The completion rate was 95.7% (n = 9275). 89% (n = 8258) were in favor of related LLD, and 32% (n = 2937) supported unrelated LLD. The following variables were associated with having a more favorable attitude: (1) age (P = 0.008); (2) sex (P < 0.001); (3) academic year (P < 0.001); (4) geographical area (P = 0.013); (5) believing in the possibility of needing a transplant oneself in the future (P < 0.001); (6) attitude toward deceased donation (P < 0.001); (7) attitude toward living kidney donation (P < 0.001); (8) acceptance of a donated liver segment from a family member if one were needed (P < 0.001); (9) having discussed the subject with one's family (P < 0.001) and friends (P < 0.001); (10) a partner's opinion about the subject (P < 0.001); (11) carrying out activities of an altruistic nature; and (12) fear of the possible mutilation of the body after donation (P < 0.001).
CONCLUSION:
Spanish medical students have a favorable attitude toward LLD

A Simple Frailty Score Predicts Survival and Early Mortality in Systemic AL Amyloidosis
Systemic AL amyloidosis is a challenging disease for which many patients are considered frail in daily clinical practice. However, no study has so far addressed frailty and its impact on the outcome of these patients. We built a simple score to predict mortality based on three frailty-associated variables: age, ECOG performance status (<2 vs. ≥2) and NT-proBNP (<8500 vs. ≥8500 ng/L). Four-hundred and sixteen consecutive newly diagnosed patients diagnosed at ten sites from the Spanish Myeloma Group were eligible for the study. The score was developed in a derivation cohort from a referral center, and it was externally validated in a multicenter cohort. Multivariate analysis showed that the three variables were independent predictors of survival. The score was able to discriminate four groups of patients in terms of overall survival and early mortality in both cohorts. Comorbidity was also analyzed with the Charlson comorbidity index, but it did not reach statistical significance in the model. A nomogram was created to easily estimate the mortality risk of each patient at each time point. This score is a simple, robust, and efficient approach to dynamically assess frailty-dependent mortality both at diagnosis and throughout follow-up. The optimal treatment for frail AL amyloidosis patients remains to be determined but we suggest that the estimation of frailty-associated risk could complement current staging systems, adding value in clinical decision-making in this complex scenario.Peer reviewe
