    The effect of chemotherapy combined with recombination mutant human tumor necrosis factor on advanced cancer

    BACKGROUND: Past studies suggested that tumor necrosis factor (TNF) assisted anti-tumor treatment and intensified the sensitivity of chemotherapy. However its clinical application has been curbed because of its low purity, high dosage, and strong toxicity. This research, through perspective random clinical control experiment, observed the therapeutic effect of the treatment of late malignant tumor through the injection of recombinant mutant human tumor necrosis factor (rmhTNF) combined with general chemotherapy and its adverse reactions. METHODS: 105 patients with advanced malignant tumor were randomly divided into trial group, 69 patients, and control group, 36 patients. Injection of rmhTNF 4 × 10(6)u/m(2 )was given to the trial group, from the 1(st )to 7(th )days, the 11(th )to 17(th )days combined with chemotherapy course. The chemotherapy plan was as follows: CAP for patients with the NSCLC; FAM for patients with gastric cancer; FC for patients with colorectal cancer. One treatment cycle lasted for 21 days and two cycles were scheduled. The control group was given only the same chemotherapy as the trial group. RESULTS: In the trial group there was 1 CR case and 12 PR cases, and the response rate is 13/69 (18.84%); in the control group 1 PR case, the response rate 1/36 (2.78%). The response rate of the trial group was significantly higher than that of the control group (P = 0.022). The response rate for NSCLC in the trial group was 8/17 (47.06%), and 1/6 (16.67%) in the control group. The response rates for gastric cancer and colorectal cancer in the trial groups also were higher than those of the control groups. After the treatment the KPS is 89.00 ± 9.92 in the trial group, and 84.17 ± 8.84 in the control group, with a significant difference between the two groups (P = 0.028). The adverse reactions of rmhTNF injection included: pain in the injection area, chill, hardening and swelling and redness in the injection area, fever, ostealgia and myosalgia, and cold-like symptoms. All these adverse reactions were mild and bearable. CONCLUSIONS: The administration of rmhTNF injection in combination with general chemotherapy is an effective and secure means in treating advanced malignant tumor

    Observations of discrete magnetosonic waves off the magnetic equator

    Fast mode magnetosonic waves are typically confined close to the magnetic equator and exhibit harmonic structures at multiples of the local, equatorial proton cyclotron frequency. We report observations of magnetosonic waves well off the equator at geomagnetic latitudes from −16.5°to −17.9° and L shell ~2.7–4.6. The observed waves exhibit discrete spectral structures with multiple frequency spacings. The predominant frequency spacings are ~6 and 9 Hz, neither of which is equal to the local proton cyclotron frequency. Backward ray tracing simulations show that the feature of multiple frequency spacings is caused by propagation from two spatially narrow equatorial source regions located at L ≈ 4.2 and 3.7. The equatorial proton cyclotron frequencies at those two locations match the two observed frequency spacings. Our analysis provides the first observations of the harmonic nature of magnetosonic waves well away from the equatorial region and suggests that the propagation from multiple equatorial sources contributes to these off-equatorial magnetosonic emissions with varying frequency spacings

    Particle energization in space plasmas : towards a multi-point, multi-scale plasma observatory

    This White Paper outlines the importance of addressing the fundamental science theme "How are charged particles energized in space plasmas" through a future ESA mission. The White Paper presents five compelling science questions related to particle energization by shocks, reconnection, waves and turbulence, jets and their combinations. Answering these questions requires resolving scale coupling, nonlinearity, and nonstationarity, which cannot be done with existing multi-point observations. In situ measurements from a multi-point, multi-scale L-class Plasma Observatory consisting of at least seven spacecraft covering fluid, ion, and electron scales are needed. The Plasma Observatory will enable a paradigm shift in our comprehension of particle energization and space plasma physics in general, with a very important impact on solar and astrophysical plasmas. It will be the next logical step following Cluster, THEMIS, and MMS for the very large and active European space plasmas community. Being one of the cornerstone missions of the future ESA Voyage 2050 science programme, it would further strengthen the European scientific and technical leadership in this important field.Peer reviewe

    Abstract The commercial space industry seems to draw worldwide attention in the recent two decades to respond to the increasingly broad market demand. Compared with Western countries, the development of China’s commercial space industry is still in its infancy and faces many daunting challenges. Few studies have considered the dual perspective of government and market of commercial space industry in China. Aiming at exploring the influencing factors and future directions of the development of China’s commercial space industry, this paper proposed the theoretical analysis framework consisting of Policy, Innovation and Economics, and conducted a synergistic analysis of government and market in China’s commercial space industry. Specifically, based on the interview data, assisted by a machine learning approach, we have conducted an industry analysis of the influencing factors of commercial space in China. The results show that China’s commercial space industry is rapidly developing with many characteristics. Specifically, at present, there is a lack of specific and operational policies at the implementation level. Despite limited disruptive innovation, commercial space companies have achieved breakthroughs in many key areas. The overall industry is exploring more stable and sustainable profit models. The findings of this study contribute to the topic of industry development in business management literature by underpinning the policy discussion, technological analysis and direction for the future development of China’s commercial space industry. The industry practitioners may also benefit from the practices that were discussed in this research

    Incidence and risk factors of in-hospital prosthesis-related complications following total shoulder arthroplasty

    Background The occurrence of prosthesis-related complications after total shoulder arthroplasty is devastating and costly. The purpose was to determine the incidence and risk of in-hospital prosthesis-related complications after total shoulder arthroplasty utilizing a large-scale sample database. Methods A retrospective database analysis was performed based on Nationwide Inpatient Sample from 2010 to 2014. Patients who underwent total shoulder arthroplasty were included. Patient demographics, hospital characteristics, length of stay, economic indicators, in-hospital mortality, comorbidities, and peri-operative complications were evaluated. Results A total of 34,198 cases were capture from the Nationwide Inpatient Sample database. There were 343 cases of in-hospital prosthesis-related complications after total shoulder arthroplasty and the overall incidence was 1%, with a more than 2.5-fold decrease from 2010 to 2014. Dislocation was the most common category among prosthesis-related complications (0.1%). The occurrence of in-hospital prosthesis-related complications was associated with significantly more total charges and slightly longer length of stay while less usage of Medicare. Risk factors of prosthesis-related complications were identified including younger age (<64 years), female, the native American, hospital in the South, alcohol abuse, depression, uncomplicated diabetes, diabetes with chronic complications, fluid and electrolyte disorders, metastatic cancer, neurological disorders, and renal failure. Interestingly, advanced age (≄65 years) and proprietary hospital were found as protective factors. Furthermore, prosthesis-related complications were associated with aseptic necrosis, rheumatoid arthritis, rotator cuff tear arthropathy, Parkinson’s disease, prior shoulder arthroscopy, and blood transfusion. Conclusions It is of benefit to study risk factors of prosthesis-related complications following total shoulder arthroplasty to ensure the appropriate management and optimize consequences although a relatively low incidence was identified

    Electric fields associated with dipolarization fronts

    Electric fields associated with dipolarization fronts (DFs) have been investigated in the magnetotail plasma sheet using Cluster observations. We have studied each term in the generalized Ohm&apos;s law using data obtained from the multispacecraft Cluster. Our results show that in the plasma flow frame, electric fields are directed normal to the DF in the magnetic dip region ahead of the DF as well as in the DF layer but in opposite directions. Case and statistical studies show that the Hall electric field is important while the electron pressure gradient term is much smaller. The ions decouple from the magnetic field in the DF layer and dip region (E + V-i x B not equal 0), whereas electrons remain frozen-in (E + V-e x B = del p(e)/n(e)e).Astronomy &amp; AstrophysicsSCI(E)[email protected]
