Estudio observacional de complicaciones neurológicas a medio-largo plazo tras osteotomías del calcáneo.
Introducción. La osteotomía de calcáneo es un procedimiento ortopédico frecuente en
la patología del pie y tobillo. Su principal indicación es la corrección en el plano coronal
del mal alineamiento del retropié. Muchos son los estudios sobre las múltiples
complicaciones en este tipo de cirugías, pero son muy pocos los que cuantifican la
incidencia real de complicaciones neurológicas, que muchas veces no son
diagnosticadas. Hipótesis. La incidencia de lesiones neurológicas tras osteotomías de
calcáneo es mayor a la publicada en la literatura. Objetivos. Conocer la prevalencia de
lesiones neurológicas a medio-largo plazo tras realización de osteotomías de calcáneo y
estudiar si existe factor que favorezca su aparición. Material y métodos. Se incluyeron
todos los pacientes intervenidos de osteotomía de calcáneo en el Hospital Universitario
de Canarias entre 2010 y 2018. Se revisaron todas las historias clínicas, protocolos
quirúrgicos, radiografías pre y postoperatorias. Se realizó una exploración física
neurológica minuciosa y sistemática bilateral actual de pie y tobillo. Se incluyeron un
total de 69 pacientes. Resultados. La incidencia de complicaciones neurológicas tras
osteotomía de calcáneo fue de 43,47%. De éstas, el 8,7% fueron neuroapraxias, que se
recuperaron espontáneamente en menos de un año, mientras que el 34,7% presentaron
secuela neurológica permanente. El seguimiento medio postoperatorio fue de 3,8 años.
Mantener la distancia de seguridad en el abordaje disminuye las lesiones del nervio
sural (p = 0,035). No se encontraron diferencias significativas entre el lugar de la
osteotomía, el número de tornillos y el tipo de cierre y el aumento de complicaciones
neurológicas. Presentar déficit neurológico no condiciona peor percepción del resultado
funcional desde el punto de vista del paciente (p < 0,01). Conclusiones. Las
complicaciones neurológicas tras osteotomías de calcáneo son frecuentes y están
infradiagnosticadas. La "zona segura" en el lugar de la osteotomía descrita en estudios
en cadáver, no parece relacionarse con los resultados clínicos en vivo. No se
encontraron diferencias significativas en función del lugar de la osteotomía, el número
de tornillos y el tipo de cierre. Se debe explicar a los pacientes que existe un riesgo
considerable de presentar lesión neurológica tras osteotomía de calcáneo, si bien ésta
suele ser bien tolerada clínicamente y muchas veces pasa desapercibidaIntroduction. Calcaneal osteotomy is a frequent orthopedic procedure in foot and ankle
pathology. Its main indication is the correction in the coronal plane of the misalignment
of the hindfoot. There are many studies on the multiple complications in this type of
surgery, but there are very few that quantify the real incidence of neurological
complications, which according to many times are not diagnosed. Hypothesis. The
incidence of neurological injuries after calcaneal osteotomies is higher than that
published in the literature. Objectives. To know the prevalence of neurological injuries
in the medium-long term after performing calcaneal osteotomies and to study if there is
a factor that favors its appearance. Methods. All patients who underwent calcaneal
osteotomy at the University Hospital of the Canary Islands between 2010 and 2018
were included. All medical records, surgical protocols, pre and postoperative
radiographs were reviewed. A thorough and systematic bilateral current foot and ankle
neurological physical examination was performed. A total of 69 patients were included.
Results. The incidence of neurological complications after calcaneal osteotomy was
43.47%. Of these, 8.7 were neuroapraxias, which recovered spontaneously in less than a
year, while 34.7% had permanent neurological sequelae. The mean postoperative
follow-up was 3.8 years. Maintaining the safe distance in the approach reduces lesions
of the sural nerve (p = 0.035). No significant differences were found between the
osteotomy site, the number of screws and the type of closure and the increase in
neurological complications. Presenting neurological deficit does not condition worse
perception of the functional result from the patient's point of view (p <0.01).
Conclusions. Neurological complications after calcaneal osteotomies are frequent and
underdiagnosed. The "safe zone" at the osteotomy site described in cadaver studies does
not appear to be related to live clinical results. No significant differences were found
depending on the location of the osteotomy, the number of screws and the type of
closure. Patients should be explained that there is a considerable risk of presenting
neurological injury after calcaneal osteotomy, although this is usually well tolerated
clinically and often goes unnoticed
Optimization of Drug Permeation from 8% Ciclopirox Cyclodextrin/Poloxamer-Soluble Polypseudorotaxane-Based Nail Lacquers
Cyclodextrin/poloxamer-soluble polypseudorotaxane-based nail lacquers have demonstrated significant capacity for promoting the permeation of drugs into the nail plate. Furthermore, previous studies have shown that the use of hydroalcoholic blends as vehicles promotes drug permeation. The work described herein studies the effect of the type of alcohol used in the lacquer preparation, and the composition of the vehicle is optimized to obtain soluble doses of 8% and to promote the diffusion of ciclopirox base and olamine across the nail. Permeation studies on different types of alcohols show that optimum results are achieved with short-chain alcohols, and that results become less satisfactory the higher the number of alcohol carbons. In addition, solubility and penetration studies on the bovine hoof have enabled the composition of the lacquer to be optimized for both forms of ciclopirox. The results suggest that optimized lacquers have better ciclopirox diffusion and penetration properties than the commercial reference lacquer. Lastly, in vivo studies in which optimized ciclopirox olamine lacquer was applied for 45 days to the nails of healthy volunteers showed that it caused no negative effects or changes to the nail surface. A topical microemulsion for the prevention of allergic rhinitis symptoms: results of a randomized, controlled, double-blind, parallel group, multicentre, multinational clinical trial (Nares study)
Background: Since barrier protection measures to avoid contact with allergens are being increasingly developed, we assessed the clinical efficacy and tolerability of a topical nasal microemulsion made of glycerol esters in patients with allergic rhinitis. Methods: Randomized, controlled, double-blind, parallel group, multicentre, multinational clinical trial in which adult patients with allergic rhinitis or rhinoconjunctivitis due to sensitization to birch, grass or olive tree pollens received treatment with topical microemulsion or placebo during the pollen seasons. Efficacy variables included scores in the mini-RQLQ questionnaire, number and severity of nasal, ocular and lung signs and symptoms, need for symptomatic medications and patients" satisfaction with treatment. Adverse events were also recorded. Results: Demographic characteristics were homogeneous between groups and mini-RQLQ scores did not differ significantly at baseline (visit 1). From symptoms recorded in the diary cards, the ME group showed statistically significant better scores for nasal congestion (0.72 vs. 1.01; p = 0.017) and mean total nasal symptoms (0.7 vs. 0.9; p = 0.045). At visit 2 (pollen season), lower values were observed in the mini-RQLQ in the ME group, although there were no statistically significant differences between groups in both full analysis set (FAS) and patients completing treatment (PPS) populations. The results obtained in the nasal symptoms domain of the mini-RQLQ at visit 2 showed the highest difference (−0.43; 95% CI: -0.88 to 0.02) for the ME group in the FAS population. The topical microemulsion was safe and well tolerated and no major discomforts were observed. Satisfaction rating with the treatment was similar between the groups. Conclusions: The topical application of the microemulsion is a feasible and safe therapy in the prevention of allergic symptoms, particularly nasal congestion

El presente trabajo se ha realizado en el Departamento de Filosofía y Ciencias de la educación, en concreto en el área de métodos de investigación y diagnóstico en educación (MIDE) a la que pertenecen los profesores implicados. El objetivo principal del proyecto es desarrollar materiales didácticos relacionados con la investigación y el diagnóstico en educación pertenecientes a la titulación de Psicopedagogía. Se ha desarrollado siguiendo los siguientes fases: primero preparación y revisión, segundo diseño, tercero aplicación, cuarto, evaluación y quinto modificación del trabajo en grupo, para el diseño y la perspectiva de la investigación evaluativa para el establecimiento de los resultados. La utilización del material elaborado ha supuesto el favorecimiento del aprendizaje de los alumnos y ha potenciado la eficacia de las prácticas de forma que los alumnos han adquirido competencias en el ámbito de la Intervención Psicopedagógica y de la Investigación Educativa.
