Appropriateness and cost-effectiveness in the treatment of invasive candidiasis in Internal Medicine Wards

    Get PDF
Invasive candidiasis (IC), including candidemia, is a major cause of morbidity and mortality among patients and the majority of cases of candidemia are documented in Medical Wards. Early identification of patients at risk, knowledge of local epidemiology and prompt efforts to define etiologic diagnosis are pivotal to ensure appropriateness. Start with an echinocandin and switch to fluconazole when possible, seems to represent a useful strategy for the management of IC. The choice between the three echinocandins should be based on the specific indications, pharmacokinetic/pharmacodynamic profile, clinical experience and cost

Remote sensing investigation techniques for the analysis of rocky slope stability in remote areas: a test from the Sierra Madre Occidental, Mexico

    Get PDF
Direct field survey to assess slope stability in steep and remote rocky cliffs is time demanding and highly consuming in term of human and economic resources. However, evolving technologies allow remotely sensed data integrated with GIS to theoretically provide equivalent information. Here we present a case study comparison of these methods applied to the Eastern valley-side of the Chinipas River, Sierra Madre Occidental, Mexico. Results show that remote sensing procedures provides the same discontinuity sets and equivalent attitude information with respect to the data acquired during field survey

Regio- and Stereodivergent Allylic Reductions of Bicyclic Piperidine Enecarbamate Derivatives

The particular nature of tetrahydropyrido[4,3-e]-1,4,2-dioxazines of type 1 allows the regio- and stereoselective obtainment of substituted N-carbamoyl tetrahydropyridines by common reducing agents. A completely novel, biologically active, bicyclic 1,3-diaza-4-oxa-[3.3.1]-nonene scaffold can be generated by the use of lithium triethylborohydride through unprecedented cascade syn-SN2′ reduction/carbamate reduction/cyclization reactions. The remarkable regioselectivity switches in the allylic reduction process have been rationalized with the aid of computational studies

Influenza della velocità di deformazionenel carico di rottura di moschettoni in lega di alluminio e di acciaio

    Get PDF
We analyze data on the deformability, the work on resistance to breakage and obtained evidence of a fall in variable strain rate (Torre CRASC) on snap aluminum alloy and steel, noting that with increasing strain rate characteristics resistance decreases markedly. These changes are made in relation to the different ranges of values of strain rate of its progression caving, canyoning, mountaineering, high strain rate (via ferrata)

Anisotropic straining of graphene using micropatterned SiN membranes

    Get PDF
We use micro-Raman spectroscopy to study strain profiles in graphene monolayers suspended over SiN membranes micropatterned with holes of non-circular geometry. We show that a uniform differential pressure load ΔP\Delta P over elliptical regions of free-standing graphene yields measurable deviations from hydrostatic strain conventionally observed in radially-symmetric microbubbles. The top hydrostatic strain εˉ\bar{\varepsilon} we observe is estimated to be ≈0.7%\approx0.7\% for ΔP=1 bar\Delta P = 1\,{\rm bar} in graphene clamped to elliptical SiN holes with axis 4040 and 20 μm20\,{\rm \mu m}. In the same configuration, we report a G±G_\pm splitting of 10 cm−110\,{\rm cm^{-1}} which is in good agreement with the calculated anisotropy Δε≈0.6%\Delta\varepsilon \approx 0.6\% for our device geometry. Our results are consistent with the most recent reports on the Gr\"uneisen parameters. Perspectives for the achievement of arbitrary strain configurations by designing suitable SiN holes and boundary clamping conditions are discussed.Comment: 8 pages, 6 figure (including SI

Predictors of intensive care unit admission in patients with Legionella pneumonia: role of the time to appropriate antibiotic therapy

    Get PDF
Legionella spp. pneumonia (LP) is a cause of community-acquired pneumonia (CAP) that requires early intervention. The median mortality rate varies from 4 to 11%, but it is higher in patients admitted to intensive care unit (ICU). The objective of this study is to identify predictors of ICU admission in patients with LP
