    Procedure for an effective quick and targeted distribution of product to final beneficiaries by a social food bank

    Humanitarian food banks throughout Europe periodically deliver baskets of food products to vulnerable families. These entities occasionally receive large, unexpected quantities of perishable products, sometimes very specific (e.g., gluten-free, vegan), to be distributed urgently. The highly random nature of these arrivals has hindered their study in literature. The purpose of this work is to define a procedure for an effective, quick and targeted distribution to beneficiaries, of large quantities of sometimes very specific perishable products. The research methodology starts with an in-depth on-site analysis of a real food bank, interviews with managers and beneficiaries, and definition of the procedure requirements. Next, the procedure is designed, defining how to select the appropriate beneficiaries for the targeted distribution, using free and common-use tools to announce the product distribution and allow the collection request by interested beneficiaries. It is technically easy to use, taking into account that food banks’ workforce is composed mainly of volunteers, with a high turnover. Training material is prepared for volunteers and beneficiaries to overcome the digital gap.     The network of entities for the social distribution of food in Terrassa "El Rebost" is dedicated to the periodic delivery of baskets of food products to beneficiaries who suffer from food insecurity. El Rebost must carry out urgent product distributions, for three reasons: i) an unexpected large donation, ii) the product is very specific (e.g., gluten-free food) or iii) perishable product. To address this problem, the project, implemented and validated during 2021, has developed a simple solution for personalized notices and collection reservations, considering budget constraints and inexperience with digital applications of some beneficiaries. In order to achieve an efficient and equitable distribution of the product, the solution defines a beneficiary notification procedure based on the characteristics of the product to be distributed, age, intolerances, specific diets (e.g., vegan) or time since the last receipt of the product. Specifically, the solution uses Whatsapp and Doodle, and includes training materials for beneficiaries and volunteers in charge of the daily operations of the project.     Lysinuric Protein Intolerance and Its Nutritional and Multisystemic Challenges in Pregnancy: A Case Report and Literature Review

    Lysinuric protein intolerance (LPI) is a rare inborn error of metabolism (IEM), classified as an inherited aminoaciduria, caused by mutations in the SLC7A7 gene, leading to a defective cationic amino acid transport. The metabolic adaptations to the demands of pregnancy and delivery cause significant physiological stress, so those patients affected by IEM are at greater risk of decompensation. A 28-year-old woman with LPI had experienced 3 early miscarriages. While pregnancy was finally achieved, diverse nutritional and medical challenges emerged (food aversion, intrauterine growth restriction, bleeding risk, and preeclampsia suspicion), which put both the mother and the fetus at risk. Moreover, the patient requested a natural childbirth (epidural-free, delayed cord clamping). Although the existence of multiple safety concerns rejected this approach at first, the application of novel strategies made a successful delivery possible. This case reinforces that the woman’s wish for a non-medicated, low-intervention natural birth should not be automatically discouraged because of an underlying complex metabolic condition. Achieving a successful pregnancy is conceivable thanks to the cooperation of interdisciplinary teams, but it is still important to consider the risks beforehand in order to be prepared for possible additional complications
