    Cochrane Centralised Search Service showed high sensitivity identifying randomized controlled trials: A retrospective analysis

    BACKGROUND: The Cochrane Central Register of Controlled Trials (CENTRAL) is compiled from a number of sources, including PubMed and Embase. Since 2017, we have increased the number of sources feeding into CENTRAL and improved the efficiency of our processes through the use of APIs, machine learning and crowdsourcing. OBJECTIVES: Our objectives were twofold: (1) Assess the effectiveness of Cochrane's centralised search and screening processes to correctly identify references to published reports which are eligible for inclusion in Cochrane systematic reviews of randomised controlled trials (RCTs). (2) Identify opportunities to improve the performance of Cochrane's centralised search and screening processes to identify references to eligible trials. METHODS: We identified all references to RCTs (either published journal articles or trial registration records) with a publication or registration date between 1st January 2017 and 31st December 2018 that had been included in a Cochrane intervention review. We then viewed an audit trail for each included reference to determine if it had been identified by our centralised search process and subsequently added to CENTRAL. RESULTS: We identified 650 references to included studies with a publication year of 2017 or 2018. Of those, 634 (97.5%) had been captured by Cochrane's Centralised Search Service (CSS). Sixteen references had been missed by the CSS: six had PubMed-not-MEDLINE status, four were missed by the centralised Embase search, three had been misclassified by Cochrane Crowd, one was from a journal not indexed in MEDLINE or Embase, one had only been added to Embase in 2019, and one reference had been rejected by the automated RCT machine learning classifier. Of the sixteen missed references, eight were the main or only publication to the trial in the review in which it had been included. CONCLUSIONS: This analysis has shown that Cochrane's centralised search and screening processes are highly sensitive. It has also helped us to understand better why some references to eligible RCTs have been missed. The CSS is playing a critical role in helping to populate CENTRAL and is moving us towards making CENTRAL a comprehensive repository of RCTs

    Systematic Bias in Helioseismic Measurements of Meridional Circulation Arising from Nonlocal Averaging Kernels

    Meridional circulation in the solar convection zone plays a profound role in regulating the interior dynamics of the Sun and its magnetism. While it is well accepted that meridional flows move from the equator towards the poles at the Sun's surface, helioseismic observations have yet to provide a definitive answer for the depth at which those flows return to the equator, or the number of circulation cells in depth. In this work, we investigate whether the discrepancies regarding the nature of the return flow are intrinsic to how helioseismic observations are made. We examine the seismic signature of possible meridional flow profiles by convolving time-distance averaging kernels with the mean flows obtained from 3-D hydrodynamic simulations of the solar convection zone. At mid and high latitudes, we find that weak flow structures in the deeper regions of the convection zone can be strongly obscured by signal from the much stronger surface flows. This contamination is the result of extended side lobes in the averaging kernels and generates a spurious equatorward signal of 2--3 m s−1^{-1} at those latitudes, and at ≈70 Mm\approx 70~\mathrm{Mm} depth. At low latitudes, however, the flows in the simulations tend to be stronger and multiple cells across the shell depth can produce a sufficiently strong seismic signal to survive the convolution process. The signal associated with the deep equatorward return flow in the Sun is expected to be weak and in the same sense as the contamination from the surface. Hence, the return flow needs to exceed ∌2\sim 2--3 m s−13~ \mathrm{m~s^{-1}} in magnitude for reported detections to be considered significant.Comment: Submitted to AAS Journal

    Spatial Distribution of Petroleum Hydrocarbons in Sediment Cores from Blind Pass, St. Pete Beach, Florida

    One hundred and one sediment cores were collected to characterize the spatial distribution of petroleum hydrocarbons within and just outside Blind Pass, St. Pete Beach, Florida. Twenty-five percent of the cores exhibited levels of petroleum hydrocarbons above detection limits of the gas chromatograph/flame ionization detector (GC/FID) (0.01 mg/Kg), but at generally low concentrations. Petroleum hydrocarbon speciation studies of these samples (gas chromatography/mass spectroscopy [GC/MS]) indicate above-detection level (1 ÎŒg/Kg) petroleum hydrocarbons are similar to the non-volatile petroleum hydrocarbons found in a Bouchard 155 reference sample collected after the 1993 oil spill in the area, but are in a much degraded and weathered state. Individual petroleum hydrocarbons were, in all but one case, below the threshold effective level (TEL) described in the literature (MacDonald, 1994). The petroleum hydrocarbons were primarily found at 100-300 cm depth in Blind Pass cores. Above-detection level petroleum hydrocarbons were generally found in samples from cores in the center of the channel, near the edges of the shoal, and just outside of Blind Pass. A second mixture of hydrocarbons, primarily phthalates, ketones, and ether, was found at relatively shallow core depths (0-99 cm) in the Mid- and North End Channel cores. These suggest a separate source of contamination, possibly storm water runoff. The fuel fluorescence detector (FFD) probe was investigated for its ability to detect petroleum hydrocarbons in marine sediments. When analyzed with the FFD, all sediments from the cores produced peaks of fluorescence, but none above the background levels of Blind Pass native sediments. All but two samples analyzed by GC/FID were below the detection limits (100 ppm) of the FFD. These samples were found in dark-colored sediments. The combination of the detection limits of the instrument, sediment color, and the degraded nature of the heavier weight petroleum hydrocarbons may have resulted in fluorescence outputs below background levels. These studies demonstrate that the distribution of petroleum hydrocarbons within Blind Pass sediments is generally low and patchy. However, 25% of the cores exhibited levels above detection using GC/FID/MS. These cores could be subjected to individual speciation studies which indicate generally below TEL levels and an association of some, but not all, with the 1993 oil spill in Blind Pass. Appendix A provides photographs and tables for sediment subsamples which exhibited total petroleum hydrocarbon concentrations above detection limits, while Appendix B presents the results from fuel fluorescence detector probe analyses. A discussion of the results of the study in relation to sediment quality guidelines and soil cleanup target level guidance documents is included as Appendix C. Some preliminary results using the above techniques on core samples from the nearby John’s Pass are presented in Appendix D

    Factors associated with self-care activities among adults in the United Kingdom: a systematic review

    Background: The Government has promoted self-care. Our aim was to review evidence about who uses self-tests and other self-care activities (over-the-counter medicine, private sector,complementary and alternative medicine (CAM), home blood pressure monitors). Methods: During April 2007, relevant bibliographic databases (Medline, Embase, Cumulative Index to Nursing and Allied Health Literature, Applied Social Sciences Index and Abstracts, PsycINFO,British Nursing Index, Allied and Complementary Medicine Database, Sociological Abstracts, International Bibliography of the Social Sciences, Arthritis and Complementary Medicine Database, Complementary and Alternative Medicine and Pain Database) were searched, and potentially relevant studies were reviewed against eligibility criteria. Studies were included if they were published during the last 15 years and identified factors, reasons or characteristics associated with a relevant activity among UK adults. Two independent reviewers used proformas to assess the quality of eligible studies. Results: 206 potentially relevant papers were identified, 157 were excluded, and 49 papers related to 46 studies were included: 37 studies were, or used data from questionnaire surveys, 36 had quality scores of five or more out of 10, and 27 were about CAM. Available evidence suggests that users of CAM and over-the-counter medicine are female, middle-aged, affluent and/or educated with some measure of poor health, and that people who use the private sector are affluent and/or educated. Conclusion: People who engage in these activities are likely to be affluent. Targeted promotion may, therefore, be needed to ensure that use is equitable. People who use some activities also appear to have poorer measures of health than non-users or people attending conventional services. It is, therefore, also important to ensure that self-care is not used as a second choice for people who have not had their needs met by conventional service

    Rapid generation of chromosome-specific alphoid DNA probes using the polymerase chain reaction

    Non-isotopic in situ hybridization of chromosome-specific alphoid DNA probes has become a potent tool in the study of numerical aberrations of specific human chromosomes at all stages of the cell cycle. In this paper, we describe approaches for the rapid generation of such probes using the polymerase chain reaction (PCR), and demonstrate their chromosome specificity by fluorescence in situ hybridization to normal human metaphase spreads and interphase nuclei. Oligonucleotide primers for conserved regions of the alpha satellite monomer were used to generate chromosome-specific DNA probes from somatic hybrid cells containing various human chromosomes, and from DNA libraries from sorted human chromosomes.     Boynton-Delray Coastal Water Quality Monitoring Program

    This report discusses a sequence of six cruises in the vicinity of the Boynton-Delray (South Central) treated-wastewater plant outfall plume (26°27\u2743 N, 80°2\u2732 W), the Boynton Inlet (26°32\u2743 N, 80°2\u2730 W), and the Lake Worth Lagoon, Palm Beach County, Florida. The sampling cruises took place on June 5-6, 2007; August 28-29, 2007; October 18-19, 2007; February 14 and 18, 2008; May 19-20, 2008; and July 11-13, 2008. Water was sampled at 18 locations at the surface, middle, and near the seafloor (where there was sufficient depth) for a total of 45 samples; these samples were analyzed for a variety of nutrients and related parameters. The water sampling unit contained a conductivity-temperature-depth (CTD) instrument from which data were obtained at each sampling site. Synchronal ocean current data were measured by a nearby acoustic Doppler current profiler (ADCP) instrument. The inlet measurements were consistently lower in salinity and more acidic (lower in pH) than the coastal ocean and were warmer during the May and, especially, during the February cruises. For most analytes, viz., nitrite+nitrate (N+N), total suspended solids (TSS), chlorophyll-a, silica (Si), and total dissolved nitrogen (TDN), the lagoon concentrations were significantly higher than the coastal ocean; the inlet concentrations appeared to be consistent with lagoon water with partial mixing with the coastal ocean, as expected. Estimates of the nutrient flux to the coastal ocean were computed: approximately 1,500 kg of dissolved nitrogen (N), 2,350 kg of silicate (Si), 33 kg of orthophosphate (P), and 59 kg of ammonium (NH4) per day were delivered to the coastal ocean through the inlet. The outfall boil at South Central outfall (the smallest in volume of the six outfalls in southeast Florida) is only visible under ideal conditions. In the six cruises described in this document, the outfall boil could be found in only one cruise (August 28-29, 2007). Elevated concentrations of nutrients (N+N, P, Si, and P) at the outfall vicinity were measured, and these concentrations decreased rapidly away from the outfall for most analytes, to become undistinguished from the background within 3 km or less. Not finding the boil, however, in five of six cruises meant that the waters with the highest concentrations were probably missed. When the boil was sampled in August 2007, N+N, P, and total dissolved phosphorus (TDP) concentrations at the boil were roughly the same as from the inlet. For other analytes (chlorophyll-a, TSS, Si, and dissolved organic carbon [DOC]), the concentrations at or near the outfall were significantly less than those from the lagoon and inlet on most of the cruises. The coastal ocean appeared to be significantly impacted by the Boynton Inlet and less so from the inlet. A suggestion of a source to the south was seen in some analytes. Measurements from the Gulf Stream Reef area were the lowest in the study, and may provide “background” concentrations for this region. As expected, the coastal ocean was warmer and more stratified in the summer compared to the winter, e.g., whereas no thermocline was noted in the CTD data from February 2007, a strong thermocline was observed in most casts during July 2008. In certain cases (e.g., N+N in June 2007, pH in July 2008), an increase in the concentration (decrease for pH) from north to south implied a source from the south, e.g., the Boca Raton Inlet or Boca Raton outfall

    Social work students as community partners in a family intervention programme

    Summary: This paper describes the findings from the evaluation of a UK initiative which engaged social work students as community partners within an educational based family intervention programme. Fourteen social work students in the first year of a BA (Hons) were placed in the programme to meet the volunteering requirements of their 'Community Project'. By engaging with the community-based family programme at an early stage in their education, students experienced the benefits of interventions, focussing on sustainability, citizenship and parent participation. We describe the approach and discuss the evaluation outcomes to illustrate the potential of utilising co-learning with families in social work education. In taking up the role of community partners, students observed first-hand the value of incorporating horizontal relationships into their learning and experienced direct knowledge exchange with service users at an early stage in their training. Findings: The findings suggest that this experience enabled students to connect with conceptualisations of macro alongside micro practice in their professional development as social workers. Further, connecting critical social theories with the direct experience of families promoted the learners own empowerment and conscientization. Application: This provided a means of embedding social work values and aspirations towards social justice in their future practice orientation

    ‘Does My Haltung Look Big In This?”: The Use of Social Pedagogical Theory for the Development of Ethical and Value Led Practice

    The aim of this article is to set out how the use of social pedagogical Haltung can support the exploration of values and how this informs and shapes a practitioner’s direct work. Haltung is a German concept that has no direct English translation but means ‘mind set’, ‘ethos’ or ‘attitude’ (Eichsteller, 2010) and relates to an individual’s value base. MĂŒhrel’s (2008, cited in Eichsteller, 2010), sets out that a social pedagogical Haltung is based on the two concepts of empathic understanding and regard. This paper argues that the use of a social pedagogical Haltung gives practitioners a philosophical framework to support the reflection of core values and ethics held on a personal level. It also supports an understanding of how these influence practitioners and students when using ‘self’ in relationship based practice. The understanding of Haltung is important but for social pedagogical practice to be undertaken it also has to be demonstrated by actions. The reflective activity Values Alive in Practice, set out in this article, provides a tool for social workers, practitioners and students to critically explore their own values and practice and make more meaningful connections between their Haltung and their behaviours demonstrated in their everyday work. In the UK, values and standards for social work practice are set out by British Association of Social Work and Social Work England. Arguably, these have, at times, been reduced to a checklist for students and practitioners and can lack more in depth and explicit links to practice. The analysis of practice is more likely to focus on the skills and abilities of practitioners rather than the value base that underpins these. Whilst the understanding and key application of core knowledge and skills is essential for competent social work practice (Forrester et al., 2019), this article argues that it must also be supported and shaped by ethical principles. This article seeks to explore how social workers can be supported to adopt value led approaches to complex work within an outcome focussed culture

    The use of complementary and alternative medicine products in preceding two days among Finnish parents - a population survey

    <p>Abstract</p> <p>Background</p> <p>The use of complementary and alternative medicines (CAM) has been extensively studied globally among adult and paediatric populations. Parents, as a group, had not been studied to assess their knowledge and attitude to CAM and general medicine use. This study is necessary since parents' attitude to medicine use is known to influence their child's attitude to medicine use later in life. We therefore aim to assess the extent and types of CAM use among Finnish parents, and to determine the factors that promote the CAM use. Also, we aim to determine parents' attitude to general medicine use.</p> <p>Methods</p> <p>Children less than 12 years old, as of spring 2007, were identified from the database of the Finnish Population Register Centre and were selected by random sampling. The parents of these children were identified and a questionnaire was sent to them. Only the parent who regularly takes care of the child's medicine was requested to fill the questionnaire. Cross-tabulations and Chi-square test were used to determine the associations between categorical variables. CAMs were defined as natural products that are not registered as medicines, such as homeopathic preparations, dietary food supplements, and traditional medicinal products.</p> <p>Results</p> <p>The response rate of the survey was 67% (n = 4032). The use of CAM was 31% in the preceding two days. The most commonly used CAM products were vitamins and minerals, followed by fish oils and fatty acids. Prescription and OTC medicines were used concomitantly with CAM by one-third of the parents. CAM was frequently used by parents over 30 years (33%), female parents (32%), highly educated parents (35%), and parents with high monthly net income (3000-3999 euros, 34%). The users of CAM had more negative attitudes towards medicines than non-users of CAM.</p> <p>Conclusions</p> <p>Our findings are in accordance with those of previous studies that women over 30 years of age with a high education and income typically use CAMs. Finnish parents seem to use CAMs as complementary rather than alternative to medicines. Health care professionals should take into consideration both the concomitant use as well as the negative attitudes among CAM users in encounters with the parents.</p
